The Lost Levels which was first released in the US on Mario All Stars for SNES. Its not much different from the original Super Mario Bros, but the difficulty is way up there.

For some reason the starman music kinda reminds me of Kid Icarus. Crazy how they took this and passed it off as SMB2, none of us knew the wiser.
