I can see how the "agitated" part is demonstrated here. What about psychosis? The woman seems to have delusions about a terrible overwhelming guilt, as well as paranoia ... "a bit low" concerning her mood underestimating it, in my opinion. Also, the psychiatrist is sometimes a bit cold; "can I move on" is pressuring a poor woman, isn't it?

i also think that this sounds like psychosis (paranoid delusions, that are congruent to her mood?) secondary to depression.
