Knee and hip pain | How does ankle stiffness effect the kinetic chain?

  • A case study this week about a chap who came in with pain to his right calf/achilles, knee and hip. We find out in the assessment from the "knee to wall" test the root cause of the pain which is stemming from the stiffness in his right ankle. We find out the compensation patterns during the gait assessment.

    Why don't all PT analyze the body like this to determine problem or problems of their clients? My experience has been that they only look at the area you day hurts.

    I had a trimalleour fracture 6 weeks ago. I had ORIF surgery. I was just given the ok to full weight bearing. My knee is killing me. I do start PT tomorrow so hopefully they can address this.

    Great video, thank you. What was the time period between the first section demonstrating the issue and the final result, and how many sessions did it take to resolve?

      Hey Utsav, thank you for supporting the channel. With starting any exercise or mobility program, it will likely take 4-6 weeks to start to see changes to your tissues and joints. Even for someone healthy starting a gym program, it likely takes that amount of time to see physiological changes in muscles, tendons and joints.

      Haha good suggestion Garman. It's difficult to film while treating though. Perhaps I need to invest in a tripod!

    Hello. I got tibialis anterior inflammation from overuse. It does not hurt anymore but I can feel it to be tight. Can I still ride my bike on an indoor trainer?

      Hey Martin, it's tough to give individualized advice over the internet without having a look at your leg, but I'll cover some global concepts that may help you. The first step is to figure out what is causing the overuse? (downhill running, loose laces/heavy footwear, improper training load/poor body mechanics etc). Second, since it doesn't hurt anymore, it generally means there is no more inflammation and you should gradually start loading it again. So cycling on your indoor trainer should be a good way to get motion and loading through your ankle again. But if you're not sure, consult with your health care practitioner for more specific care and advice.

      @@ecphysiotherapy8012 I just trained too hard and too intensive 4 times a week on my bike - so overuse. But I feel it to be irritated it doesnt hurt. So can I ride my indoor bike very slow and for how long?

      The action of tib ant is to pull up on the ankle. So it could be something about your pedaling technique or bike fit (?saddle height) that may be overloading your tib ant. Or it could be as simple as too much too soon as you've mentioned. If that's the case, then I think it is safe to progress your training load in a slow controlled fashion. A good rule of thumb is if you're sore for half a day to a day after your training, you're probably good. But if you're sore for 3-4 days following an effort then you may be overdoing it, or need to have a professional look at it.

    Recovering from ankle fracture, removed the cast .. Currently walking better without aids, still ankle is stiff, limping a bit, making a load on my knee and it hurts 😕 What's the solution till my ankle heals?!

      Hi Engy, keep working on trying to normalize your gait pattern, try to focus on heel-toe pattern of walking.
      Hard for me to say where you're at for exercise prescription without an assessment. But I would start trying some terminal knee extension exercises with a band, and also triple flexion (hips/knees/ankles) with your affect leg elevated on a chair.
