The problem isn't with the relay. It's with the Adapter which has capacitor that slowly drop voltage keeping the relay from switching for half a second. Either remove the capacitor from adapter or use 2x In5822 Schottky diodes instead of relay.
@@StagLabs Thank you. After reading this discussion I am able to solve the problem of my FTTH modem. During Mains outage It always restarts even though my home battery backup inverter is in UPS mode. I changed the 12V adapter of modem and it solved the issue.

You definitely need a diode after the TP5100. You don't want current to flow back to the TP5100.

why you use relay?
Add big capacitor on the output to prevent restarting when switching for example 470 microfarad/25 volt
