  • Radiofrequency ablations, also called rhizotomy, is a nonsurgical, minimally invasive procedure that uses heat to reduce or stop the transmission of pain. Radiofrequency waves ablate, or “burn,” the nerve that is causing the pain, essentially eliminating the transmission of pain signals to the brain.
    This procedure is most commonly used to treat chronic pain and conditions such as arthritis of the spine (spondylosis) and sacroiliitis. It is also used to treat neck, back, knee, pelvic and peripheral nerve pain. The benefits of radiofrequency ablation include: avoiding surgery, immediate pain relief, little to no recovery time, decreased need for pain medication, improved function, and a quicker return to work and other activities.
    The patient lies on an x-ray table. Local anesthetic is used to numb the treatment area. The patient experiences minimal discomfort throughout the procedure. The patient remains awake and aware during the procedure to provide feedback to the physician. A light sedative can be used during the procedure.
    The technique for nerve ablation is similar to that used for diagnostic blocks. With the aid of a fluoroscope, the doctor directs a needle into the area responsible for the pain. Fluoroscopy allows the doctor to watch the needle in real-time on the fluoroscope monitor to make sure that the needle goes to the desired location.
    Once the needle is in place, the patient receives an anesthetic medication through the needle. Then a radiofrequency current is passed through the needle to create a small and precise burn, called a lesion, about the size of a cotton swab tip. The current destroys the portion of the nerve that transmits pain and disrupts the pain-producing signal. The burn takes approximately 80 seconds for each site, and multiple nerves can be burned at the same time.
    Radiofrequency nerve ablation is a relatively safe procedure with minimal risk of complications. The complications reported in the literature include: temporary increase in nerve pain, neuritis, neuroma, localized numbness, infection, allergic reaction to medications used during the procedure, and/or lack of pain relief (in less than 30% of patients).

    I had one done in May 2019. My procedure was completely pain free, took about 15 minutes and lasted an amazing 5 years. I’m getting another procedure today.

    I had this done today…. Let me tell you, I cried! The numbing injection hurt, the probes being inserted hurts… now I’m incredibly sore. Not going to work tonight. Praying for a long term relief.

      I had mine done 4 months ago. Already scheduled for another next month. I have CRPS in/around my Ribs. I've had a 10lb 12oz, 24" long baby boy and that pain, I would rather go thru multiple times than CRPS pain. It is a extremely painful procedure thing to endure. I had the needles in between my 10/11/12 Ribs in back. I am going to try an get some type of sedation or something to relax me. I felt the nerves all the way to the front of my stomach, feel as if they were bubbling. Good luck with your journey.

    I would never have this procedure without sedation. I went to sleep and didn't know a thing until it was over.

      I have a friend with chronic migraines, 6-8 on a pain scale daily. She can't take sedation. They ablation her neck wide awake, with maybe numbing her skin.
      I can't imagine, even being aware. She can't take Valium either😢😢😢
      I just had one today on my lumbar/si.

      I’ve done several times. No sedation and the needle pain meds don’t work. But in the long run it has worked well.

      I've had nerve injections, and nerve blocks done and they've never worked and the nerve block was so painful. Now my Dr wants me to do the ablation and said it's a 3 procedure thing over months. I'm terrified to be honest. I didn't know sedation was an option for these because I thought you had to tell the Dr how it feels etc. But I noticed at my apt they had a sign that they can be sedated for $50. I hope this is an option for me because I'm nervous and last time with the nerve block I passed out and when I came to and felt more pain my heart rate went through the roof.

    I work for pain management doctor and thank goodness I work with him cause I was having horrible pain in my neck due to being rear ended twice and putting dumb bar with heavy weights on my neck.
    I have see a lot people do good with this procedure first hand

    Just had my first RFA session today. Three spots targeted in the lumbar region. And mother of god, words do not describe how excruciating it was lol. Not the act of burning away the nerves, hardly felt that. But the insertion of the probes through all the muscles to the bone… holy crap. If full sedation is an option for you, take it. If not I would highly recommend an Ativan or something like that before hand.
    Been 12 hours and no lingering numbness. The sensation now is hard to describe. Some intense residual internal pain from the procedure that comes and goes, but the “normal” chronic pain I have has been greatly reduced. They also injected me with a steroid which can often give huge symptom relief within 24-48 hours.
    The experience was hell, not gonna lie. But if it frees me from the shackles of chronic and blinding back pain, then I’ll be going back as needed with a smile on my face.
    I’m tired of being a prisoner in my own body. And if 30 minutes of pure torture is the price I have to pay to get out of jail… it’s worth it.

      I’m going back for my 3rd procedure in a few months. The pain of the procedure cannot be understated or sugar coated. It fucking hurts lol. Like… a lot. BUT the benefits also defy words. Chronic pain is gone, I have full mobility, and no need for potent pain killers or muscle relaxants.
      A few minutes of blinding pain for 6 months of relief is a price I’ll happily pay every single time 😎

      Well, had my 3rd yesterday. I did end up popping an Ativan prior which helped me relax a little. Maybe helped with the pain too, as it didn’t seem quite as bad this time around. An 8.5/10 instead of the usual 9/10 lol. Or maybe my pain threshold is increasing 🤷‍♂️
      But you know what they say… no pain, something something 🤣

    I’m having this done Monday on my lower back in 4 places. After reading the comments I’m afraid now. The nurse asked me if i wanted sedation or to be out cold 🥶 I told her knock me out, if not, I’m going to pass out because i hate needles and pain.

    Just had this procedure today after an unsuccessful Anterior Disc Cervical Fusion 5 months ago. The procedure took 45 minutes in 4 areas right side of neck and shoulder. Excrutiating doesn't quite cover the experience I had, pain,pressure and shock. But if it relieves the non stop relentless pain then I would do it again Not to scare anyone but it's a lot .Fingers crossed and respect to the doctors who try their best to help people

    Got one done on 2/1/23 and this was the best experience for me I was first given sedition then propofol and I didn’t feel or remember a thing it worked for me that night.
    I had this before back in 202 and it didn’t work I was given a light sedation and it did jack I was very hesitant to do it again, but I am so glad I pursued it with better pain management team and being out completely.

    I have had this done several times in the lumbar area. Thankfully I always have heen given the option to have anesthesia . The anesthesiologist uses propofol and I am completely knocked out feeling nothing. I have never had pain coming out of the anesthesia nor after the procedure.

    Considering doing a second RFA with my VA providers AUG 2022. The first RFA was about 1.5 yrs ago & it offered some minimal improvement as it relates to feeling pain or numbness in my right leg but my back spasms and numbness on my left leg were still present then and today. Valid concerns are in regards to many folks whom have lost mobility and feeling to legs… I have lived in so much pain for 15 plus years that the RFA procedure didn’t hurt has much as I’ve read… on a scale from one to ten I rate it a 5. It did hurt, trust me. You need to be awake to answer doctors questions as to where you feel the pinch prior to the laser durns of nerve roots. I have got to do more research to determine if a second round of RFA is worth the risk of failure or complications. I value dearly the little mobility I have left.
    Good luck to all seeking pain relief, Lord knows we need it!!!

      Cbd oil has worked for my brother but the strong stuff is thc / cbd with very low thc no stoneing effects just painreif.bit expenzive

      I have never been awake and have had it done successfully several times. The doctor can see the nerves via the xray machine as s/he is doung the procedure.

      Why is it my outside Dr put me to sleep. But the VA left me awake and in pain??

      @@karynpazandak6579as per my doctors you need to be awake to feel which nerves are going to be burned to confirm prior to burning them. If your sleep then there’s no way for them to verify…. They would touch the nerve and my legs will move, adjust as needed.
      I have done about four sessions with no issues. I have moved on to a better procedure since which is tens times better then a RFA..
      It’s called a sciatic joint injection. It’s more effective then the RFA and works specifically with lower back inflammation which is the root of the pain squeezing the nerves.
      Hope this helps you folks out there dealing with chronic pain like me. The meds last about six months. Much longer then RFA’s.

    I had this done 7 weeks ago with a local anesthetic. The first 2 weeks, I had to go back because I was still in pain and was given a couple of trigger shots in the area, which kept the pain at bay for another few weeks. Let's say I'm on my way back to the Dr. to find out why I'm still in pain.

    I had to be tested first on each side of my back in separate appointments and those were horrible to me…I just had my procedure today and I was sedated and the procedure was great….cant wait see the results !

      I had the first test last week , it really helped. Going to have another test next week. They didn't knock me out and I hollered like crazy😢

      Had this done with just local anesthetic. It was no big deal. It did hurt, but it was only for very very short time and it was a lifesaver as chronic pain gone.

    Thank you for this video. I get RFA on my neck in a couple of weeks. Previously, no one (even a neurosurgeon) told me it existed, the VA has had me jumping thru hoops for 18 months trying different therapies that failed. I'm so glad I found out about it!

    For those who are talking about the pain involved, you can ask your doctor to inject extra local anesthetic and have them wait an extra 10 minutes and it’ll take some of the edge off.
    The key when it starts is to breathe slow deep breaths and to concentrate on the breathing. The other key to this is to stay as relaxed as possible. The more tense you are, the more it will hurt.
    You also can ask for a sedative and codeine to take an hour or so before. People have said that made a big difference.
    Best of luck to those who came here to know what it is. It’s worth getting

      I really appreciate you posting this I'm supposed to have this done and that's my biggest concern is what pain is involved I don't do well with pain, does asking for sanative really take the edge off and on a scale from 1 to 10 what pain would you still feel?

      @@catherinewalton6839 just remember to breathe slow deep breaths and to focus on any area of your body that tenses up. You just have to close your eyes and relax. I won’t sugar coat it and say it fixes everything but just do what I mentioned and it will absolutely make a difference. The first 30 seconds are the toughest after that it gets progressively better.

    After a 'nerve burning' of two lumber facet joints to relieve intermittent sciatica, three weeks later I'm in more pain than before plus the symptoms are omnipresent. I now regret my decision to have the procedure as now I'm in constant pain and whereas before I could play some gentle tennis from time to time...now I cannot play tennis...at all.

    I have had several to my cervical, thoracic and lumbar with relief that lasts a year to 18 months. Finally this year I had a spinal cord stimulator implanted because the sciatic nerve pain got so bad I could hardly walk. After that I had an ablation of the thoracic area and I had always done these with no sedation and tolerated them well. The last one, however, I screamed and have never felt such pain. I've had the same Dr. For over10 years, she has never hurt me. It shocked her as much as it did me. The only thing we could figure out SAS that maybe the stimulation of the nerves from the SCS had made the nerve endings terribly sensitive.

    I just had this done yesterday on left and right L3, L4, and L5 through Kaiser Permanente Tacoma. The first two test procedures are very painful; I had six injections each side...a few F-bombs and friendly banter with the doctor and staff helped 😊🤭. The rhizotomy was surprisingly mostly painless. You are sedated, but you can still communicate with the doctor. Its been 36 hours and I only feel a mild ache in the injection site. The doctor said to be prepared for it to take about a month for total healing. It's important to find a doctor who you feel comfortable with and has a good reputation. My doctor has been amazing. He was upfront about the level of pain for the test procedures and was very caring and supportive throughout each procedure. I'm hoping that the ablation gives me quality relief so that I can just get back to doing the basics again. I'll see how well this works and may get my neck done also. I have RA, OA, fibromyalgia, and ME/CFS so severe chronic pain has been a way of life for over 20 years. Hoping this gives me some relief.

    I've had this procedure more times than I can remember for over a decade I'd say. Works well for me. I usually get one once a year or so.

    Today I had my second RFA. My first one for whatever reason I was given general anesthesia. Today I was not. I have fibro on top of arthritis in my spine and other things going on there. My doctor must have been great or there were other factors not affecting me that has affected other posters. There was discomfort. Sometimes sharp pain for a moment, but not the excruciating pain others are talking about. I wonder what the difference(s) are. The nurse rubbed my back when it was obvious I was in discomfort/some pain. That was great. I would definitely go through even the pain others are talking about just to lower my pain scale from 8-9 to 4-6. (That is what happened the last almost two years.) It is definitely worth it.

      the degree of pain is likely due to whether the peripheral nerve is compressed closer to the nerve root coming directly out of the spinal cord or distally at the facet joints. some people have compression and stenosis in both places and their pain may be increased

    I live with degenerative disc desease. Numbing the pain in your back is bad. The reason is... you can not feel how much more you are grinding your lumbar down, destroying the disc( if any left) even more without feeling the pain.

    WOW! I read about a half dozen comments about the horrible pain of this procedure. This has NOT been my experience. The initial test run hurts more than the abalation. Still i would say it is highly uncomfortable versus the horrible pain some people have described. I have had multiple ablations done over the years on 12 nerves. 3 on each side of the lower back and 3 on each SI Joint. The test procedure hurts more than the ablation. For the ablation, i have been given IV Xanax. I have a fantastic Doctor and staff.

    I just had this done yesterday in my lumbar area. They gave me local anesthesia but because i talked nonstop they sedated me. I also had bursitis on the two sides of my lumbar area so they did something about them too. Its been around 20 hours since the treatment and I dont feel any pain/numbness on my legs^^

      @@its_ari5458 hi!! The first two months were great but now I`m experiencing pain again. It is not as severe before though. I used to wake up from my sleep because of it but now it`s just a uncomfortable pain. If your condition directly affects your daily life I would recommend this procedure.

    Was diagnosed with spinal stenosis and was preparing for a MILD procedure; then diagnosed with osteoarthritis of my lumbar spine and scheduled for nerve ablation. I have low pain tolerance and would like to opt for profafol.

    I had a bilateral ablation on May 1st of this year. It was lumbar and three on each side. The three on the right side were completely painless. As was the lower one on the left side. The center and top left hurt like I've never felt before. My Dr had to use an additional 4 Lidocaine injections just to able to finish. I don't think i will ever do this again. I trust my Dr and shecis extremely attentive. I have five different conditions in my lumbar area. This helped maybe two of them. I was out for five days after the procedure due to being in pain. I was out of work last week four out of five days with severe pain. My Dr is very concerned that i didn't get as much relief as she had hoped. She now ordered a new set of X-rays to see if anything else is going on.

    Had the RFA on my right knee about ten weeks ago; the pain is SO MUCH WORSE! It utterly failed, and now I can hardly walk, even at home on days off. I will never go through that again! Pain meds only make you "not care", they don't take away pain (at least for me). I walk on concrete floors a lot, every day, constant long distances. I finally told them to fill my position at work until I am able to find some other solution. I understand that most people have good results; my nurse was kinda shocked that it made it worse, and almost everyone got good results. But I, personally, am ten times worse than before the procedure. Any input on a better method is welcome and appreciated. I want to get back to work, ASAP... TY ahead of time.

    I just had this done yesterday. There was no pain at all. The nerve block was more painful. Had no pain after numbing wore off, and no burning sensation. It was great!!

    I had a lot of success with this procedure on my L5S1 and L4S2. I didn’t do the sedation but I did squeeze the stress ball they gave me 😆 My team was amazing. They would have music on and would include me in the daily gossip to help keep me focused away from the pressure. The pain is pretty quick, most of the time is spent getting everything in position. I did it about once a year for 10 years. The first couple years we would have to go back in after maybe 6 months but I would be pain free in between shots if I was careful. Sometimes i would only get a facet injection the 2nd time. It took playing around until we found the right pattern. I definitely didn’t go back to school after it though. My dad would take me out to the diner nearby after to eat and then I was in bed with ice for the rest of the day. I am SO thankful these helped for so long. I did end up having to get a laminectomy and discectomy in 2021 but it was a blessing. We always knew these were bandaids and we didn’t expect them to work as long as they did. Sending my love to everyone trying to find some relief 💕

      Just want to add after reading comments because If I had never gotten this procedure and then read these comments I’d be terrified! My experience was not at all like the comments. I never got the sedation because I was so desperate to just get on the table as fast as possible and have them do the procedure to get pain relief from my back. The pain during the procedure is mainly a pressure. You don’t feel the nerves being burned at all. You just feel the numbing shot which isn’t fun but is fine. It’s a thin sharp sting. The worst part for me is them getting to the nerve. The pressure pain feels easier for me to deal with then the disc pain but I definitely had to breath through it. I had a stress ball in each hand that I squeezed, and I usually needed to squeeze my eyes shut and let out a “eeeuumm” pain whine at the end of this part. They prep you with “okay, you’re going to feel pressure, I’m going to need you to be still and focus on breathing.” Then they talk you through it. “Okay, 1, 2, pressure-pressure-pressure…..good job you’re doing great…(the assistants are chatting at this point and include me in conversation in a way that I can talk if I want to or I can stay quiet and focus) breath, a little more pressure Chelsea…relax your back, nice we’re almost done. Pressure, (this is about the time a whine comes out of me as I feel the pressure and am really hoping we are almost done because the idea of doing this for another couple minutes feels overwhelming) breath, good! Pressure for one more second we’re almost done-almost done. Goood! Exhale. You did great Chel! You’re going to feel my fingers, I’m just moving you this way a little and I’m going to get us set up for the next one, no pain.” And then the pressure part repeats if you have multiple spots it needs to be done. I can’t remember for the life of me how long the pressure part is. My guess is 10 minutes but it probably feels longer than it actually is.

      I had it done on my neck.Started withOUT Pain meds because I had had some coffee with cream so they said not pain meds, butsaid people do it without pain meds & it didn't hurt them too much. So I said I would try it. Big mistake. It was so painful.i was screaming squirming.They stopped after just 1 side of my neck Had me come back a week later & do BOTH sides. Well it believe it was too soon for the sid they had already done.
      My neck pain before the procedure wasn't too bad so I thought do it 1st.
      Big mistake!1 Horrible,horrific pain.
      anyhow after they did both sides( 1 side 2 times in 2 weeks) I had pain worse then any I ever had in my neck so on serious pain meds I was given. Oxicontin & oxicondone going on 3 yrs or 4 yrs.now..
      My pain is constant 24/7. I can't look up or tilt my head back now to gugarle. When standing & walking the pain is unbearable!!
      I also have a military neck autonomically fusion of 3-4 & can no longer look up or tilt my head back to gugarle..
      I feel they did something wrong.maybe over burned 1 side of my neck doing the side 2 times in 2 weeks..
      It was also fellows doing it not the experienced doctor at UCLA!!
      I don't know what to do.i am so pissed.
      I am now considering disc surgery, but can't find a top doc who would take my insurance for the procedure or want to do it on an older woman such as myself!!
      So They were speaking of fusion or the pain doc. said unable neck. So I did a moving x-ray. That doc. said no NOT unstable. I think the pain doc was trying to get off the hook.
      Anyone heard of this procedure going South & causing worse constant pain ever??
      Please if anyone did or has any ideas as to what I can do. please let me know.
      I think I should SUE they at UCLA in Ca.
      Anyone know any medical accident lawyers working on contingency? Can I do it after 2 or 3 or even 4 yrs??

      I had one done in May 2019. My procedure was completely pain free, took about 15 minutes and lasted an amazing 5 years. I’m getting another procedure today.

    I didn't have a lot of pain. Watch out for afterward because sometimes the numbing can go down in the leg a little. Just be mindful of how you walk

    I've had this twice. I'm due for my third. The pain during the procedure is unbelievable. But, darn it, it works. I'm hoping today that I have the courage to endure it and to be able to control it for the few hours after it is done. Last time, I hurt for two hours after to the point of uncontrollable non-stop crying . I guess it was my body's way of handling the pain from the burning.

      The worst pain you should have during the procedure is from the initial numbing. Everything else should be a cake walk.

      I'm going in for this in 2wks time & the other side 2wks after that cervical. See what happens, Dr is going to give me an epidural on top of knocking me out. I have fybromyalgia, myofascial pain, osteoarthritis, degenerative disk disease, compression on nerves. Migraines chronic. I have had the pain 19yrs. I hope it works, that is it.

    I’ve had it don’t twice. It lasted me 8 months. It’s excruciating but the benefit outweighs the pain. Suck it up and have it done. Worth it!!!

    Wait. Did I just hear you say that you don't even need to take a day off because of this procedure? Have you ever had one of these procedures done to yourself? The pain that comes with healing from this is just awful. Even with pain meds, it is awful. I have had these done every year for 4 or 5 years and yes, the end result is good. The pain however from having this done takes up to 8 weeks to heal from. My chronic pain is in spinal levels (L/3-L/4 and L4/L5)

    I’ve had the ablation for my back and I always get an anesthetic, don’t feel, don’t recall any. So, that’s fine with me.

    I have hesitated to get an RFA but now that a knee replacement is on the horizon I am hoping that an RFA can be a stop gap solution. My hesitation is in regards to the amount of radiation one is exposed to. Does anyone know how many millisieverts are emitted during an RFA? If it can be done with an ultrasound, I would be on board.

      Ask for antivan. I take two before the procedure. And I ensure I have a fan on my, my face unobstructed, and a cold cloth on my lower back. And it’s bearable.
      For my first test, I couldn’t complete. So those tricks helped me a lot.

    RFA is an old procedure and is outdated. I had it done 12 weeks ago and it hasn't helped me much. There is a new procedure that is done endoscoplicly where the nerves are visualized and permanently abladed with a laser. The relief is long lasting and I am getting this procedure done. RFA is outdated like RK surgery for the eye. An endoscopic laser rhyzodomy is the new way.

      Will insurance cover it? I am scheduled to get an ablation done on both of my hips 2 weeks apart. I didn't know until now that it is only temporary and that the nerves can grow back. I don't want to cancel them if it can provide even temporary relief. But if insurance will cover the new more permanent procedure then I definitely want to get it! Chronic nerve pain sucks!

    Just got my rfa done today... I don't feel any pain but I feel intense numbing because of the numbing agent they used and it's been 12 hours... not going to even say where the areas that's extremely numb... how long does this wear off?

    Yeah! I just had 3 levels lumbar rhizotomy ablation. Pain is gone in lower spine but now my right leg doest work. I've fallen 4 times in 2 wks. You need to explain about nerve root damage! This is my 4th ablation. Never had problems before! But, now I can't walk without a brace locking my leg in a neutral position!

      they are the same procedure except RFA specifies the method by which the nerve is destroyed . there are 3 methods and rhizo is now mostly a laser procedure.

    There's more to be said about radiofrequency ablation (RFA) this is the 10th video that I've watched three of which was from patient perspective. The six video's were all different Dr's and pretty much stated the same information, except time wise for pain relief. I heard from 6 months up to 1year or so, for pain relief because the nerves can grow back. I advise anyone watching this video to watch several more please do your home work and be well informed on this matter. Because it's not that simple there is quite a bit of pain after the procedure may be up to three or four weeks before you feel any relief because the nerves take time to die. And you will feel pain possible even more pain this I was told from the Dr. to perform my (RFA).

      I'm sorry to hear that you have been going through that but, I had been doing this backwards but now that I have been through so much as pertaining to my back and the many procedures. I check out UA-cam on the front end now because there really are some good drs and people who have gone through the experience that can offer beneficial information.       @@joshlampe3458 it was a little more painful than I thought it would be, but they said it would take up to 6 weeks before I felt the full effects of it. What's crazy is I just had bunion and mortons nueroma surgery and I am in an air cast. It causes me to be uneven and adds a lot of disproportionate weight distribution so I still have lower pain in my back and hip. I'm hoping it will go away once I'm out of the cast next month. Keto me posted on how yours goes, I'm interested as well. Good luck!

      I have pretty severe scoliosis. My spine is in the shape if a backward s. I have this done every six months. I am great from a week after the procedure till the beginning of the 5th month. I have never been put out for this procedure, as I just don't think there is a lot of pain. In fact I had it done this morning and my blood pressure was higher before vs. after.

    I’m 21 I am scheduled to have one of these for my low back in a couple weeks I’ve had 2 injections in my SI joints prior and that shit hurt so bad I bout came off the table. My dr office doesn’t do sedation. Reading these comments has me TERRIFIED. I’m about to cancel my apt…

      Yes! The nerves regenerate so this procedure can be done numerous times once pain returns assuming it was successful for you.

      I did it like three weeks ago I regret my decision I’m still in pain that no amount of pain meds stop and the burning sensation is through the roof. The pain meds only brings the pain down to manageable levels. It’s unbelievable non stop pain. Im just praying it will get better.

      Whoever thinks this procedure is terribly painful is not getting enough lidocaine injected. My Doctor gave me a lot and if it started to hurt she injected more. Because of this mine was painless!
