Yeah, there was a bug in an initial version of te AD v28 related to the "character extents" font sizing (shown at 1'31''), but it's been fixed in updates.
Yes, of course you can do both - extrude and recess the text. And if you, for example, extrude the writing and make shell of the main body, the text will be recessed on the inside maintaining the wall thickness depending on the defined thickness and font size.

Can I give my text a depth in the surface or is that something I can do in meshcam?
To control depth, simply exit the sketch and use extrude cut or extrude boss to add/remove material.
Can you extrude the type so it is raised? Of cut out?
