Thank you for the insightful message👍. We also studied the significance of ACEIs and ARBs as renoprotective drugs during our PharmD program. It is both alarming and perplexing that some physicians do not consider these medications as first-line treatment options in Pakistan. Not only in the US or UK, these drugs are also extensively prescribed in Norway, particularly for patients with CKD and HTN, especially when Diabetes is present.

Today, I talked to my cardiologist doctor , that I am worried that my egfr has come down and less than 51. He did not reply and asked for the kidney ultrasound. I am taking Iberstan, which I think is an ARB. Thank you so much for your guidance. But one thing good , he regularly check my potassium. But he talk much less.

Assalamualaikum. Doctor saab you are correct in this regard, but what about people who ace inhibitors don't suit? I myself was a transplant survivor since 2012, having my creatinine at 2.3-2.4 stable in the last couple of years of transplant which were from 2016-2022. Since my blood pressure was usually high, the doctor prescribed ramipril, and just by taking it for two days my creatinine shot up to 5.1. we were never able to recover it back and which eventually lead to my transplanted kidney giving up just 6 months later. Alhamdullilah, I had a second one. And for that same reason I'm afraid to start on ace inhibitors again. Currently using Amlodipine.
Good point. Sometimes you can’t give it. I am talking about those who can get it and are not given.

On urine test pus cells are 10_15 rbcs 1_2 epithelial cells +++ and bacteria + but the naphrologist said it's normal and can't suggest any antibiotics both kidney sizes 9.0

Sir if our GFR is leas 35 than we can use ARB medicine. My creatinine is 2.3 I was using valsartion medicine and a doc asked that medicine reduced the blood circulation. He recommended me Amlodipine. One doc recommended one medicine and other recommended other. What will we do I am polycystic kidney patient
