Based on a book titled "Johnny Got His Gun" story about a WWI soldier who was wounded in the war and lost his limbs, sight, hearing, and speech. So he was trapped in his mind- he was One. yes, very dark
The speaking voice, background voice is the soldier( on the bed, face covered) trying to make sense of what has happened to him. Well he can't speak, he can't hear, he can't walk, he can't eat/ drink, he can't move, he has none of these anymore. So he's trying to figure out how to tell them he is alive and coherent, not just a lifeless body, he has brain activity. In the beginning it said they doubt he'd have any form of pain or brain activity, but he does. So he used the Morse Code, which most back then knew. To tell them I am thinking and have feeling. The kindest person was the Nurse trying to end his suffering, but his dad stopped her. So now he just has to wait till he dies naturally, via starvation. Back in the 1950s no-one understood medical stuff that well, let alone the brain. It was still common to have a Lebotomy(stitch in forehead) for difficult people( hole drilled into scalp and brain, add electrical stimulation to subdue the person, which caused alot of brain damage, which then made them 10times worse). Shows how far medicine has come in the 70years since.

The man laying in the hospital bed also had his face torn up so they put a mask on him so nobody could see the damage. It is like a tent so its not laying against his face or what is left of it. Great reaction!!!!

Metallica wanted it to be dark. James Hetfield was obsessed with the book and film, so he wrote the song. They then bought the rights to the movie so they could use it for the video. The movie is a tough watch, but a poinant anti-war piece.
