This is NOT their first official single, as a matter of fact, this song was never a single. Their first single (digital) was SPARK and their first official single was OTNK. This was however the first MV they released. The last single was Bye-Bye Show, of which the MV is a call back to the Hoshi-ga MV.
I don't know the whole story, but they were only allowed at some festival in 2019(?) if they didn't play this song, so as a giant F-U to the organizators, they played it 5 times in a row. There's a clip of it somewhere and it's intense, to say the least.
It became kind of a running gag to play Hoshi ga Matataku Yoru ni 5 or... 13 times in a row at concerts after their debut live, where they didn't have enough songs yet to fill the 90 minute slot they were booked for and played it 4 times. They did it a few times at their own concerts (especially at the beginning) and always at the Tokyo Idol Festival, where the "accident" you were talking about happened in 2015 at their TIF debut. Because it's an IDOL Festival, they have strict rules about not being too roudy and behave in an orderly manner. Contrary to that BiSH fans started crowd surfing, etc, and, while staff tried to calm down the situation, the members actually got fired up by it and the energy between fans and the stage kind of let it escalate into a situation where the staff almost cut power (at least that's what was being said afterwards). You can see the staff in the background waving and almost panicing during the performance. The year after, in 2016, they were only allowed to perform if they'd apologize on stage and discourage the fans if it happens again. So, Hashiyasume Atsuko, who wasn't even part of BiSH the year prior, gave an apology that absolutely reeked of sarcasm and like she didn't even know why she was apologizing in the first place and if she'd know she wouldn't care anyways, but the fans (kind of) behaved so there wasn't really a problem after that.
