What went wrong is obvious, the barge wasn't forward enough in the lock when the water level was lowered and it snagged on the cill. Operator error simple. Luckily it looks to have slid off the cill as it sunk and looks to be sitting even on the lock floor. If it had remained snagged on the cill you would have a job on your hands.

Allowing the rear of the boat to "hang" on the cill is the main danger when descending a lock, and the position of the forward edge of the cill is usually marked on the lock side by a white line.
