    I would totally have to agree with everything you said. I share your unpopular opinion with residency’s should be standard for at least a year for new NPs. I’m in an APRN residency right now as an FNP in DC and I absolutely love it and definitely think it’s super beneficial! I can go more in detail if anyone needs!

    I think you are absolutely right about the arrogance of new NPs, I know that its controversial, but just because you can do something doesnt mean you should. I hope that we see the education of NPs standardize and increase training because ultimately its about the patients. And there is a reason why MD/DOs have as much training as they do. There are absolutely fabulous NPs out there, but the license alone tells me nothing about your qualifications to be my provider

      Absolutely! The problem facing the NP profession is that it has accredited far too many schools that are simply put “diploma mills” with zerooooo or bare minimum requirements, and there’s no standardization in clinical requirements; this leads to subpar providers,and tarnishes the good name of the profession-it also causes PAs and Physicians to doubt competency’s of NPs :/

    • @LemansSunset350
      @LemansSunset350 2 роки тому

    • @naturallynursing5392
      @naturallynursing5392 2 роки тому +1

      @@LemansSunset350 I disagree. All PA programs require around 2000 hours or more of direct patient care clinical experience. Many PA students have more than that due to how competitive these programs are. The bedside nursing role is NOT the same as a medical prescribing role. So, although bedside nurses have valuable experience, it is not at all the same as working as a medical prescriber.

    I want to hear more on your thoughts of nurse residency programs. I graduate with my ADN next year and i’ve been trying to decide between going into a residency program or to just start working. I’m still a baby nursing student but I definitely feel like I could benefit from some handholding. Especially since my patient load will increase at a job vs clinicals. My long term goal is to be an NP and I definitely think residency is the way to go there, but I still have some years ahead of me before I get to that. Love you and your videos 😁

      Hi! I can definitely make a video on it. But basically you'll be fine either way. Really. I applied to a bunch of residency programs and didnt get into any of them. I had friends that did and they said it was basically just like an extended orientation with a few classroom days that weren't necessarily helpful. If you find a job that is willing to give you an appropriate orientation for a new grad, then you really don't need a residency program in my opinion. But either way, you really will be ok!

    Liz - As I've said before, I've followed your journey for a number of years. I'm wondering - have you ever considered becoming a nursing coach? (Or, if you wanted to keep doing straight NP work, looking into NP concierge medical services?) You have so much wisdom to share, and I know you're absolutely burnt out from insurance companies. Coaching nursing students might be a way to provide really targeted, individualized services for a good fee. (You could even get a group of nurses trained under you to coach even more students.) And concierge medicine, while generally for the wealthy, would allow you to give recommendations and move health care outcomes in a way that felt meaningful to you. I wish you a lot of luck, my friend.

    I graduate in less than a year and I will absolutely be looking into residencies. There’s two in my area. I have networked with someone who completed it recently and she’s going to put my name out there, so fingers crossed.
    A big draw back, is that these programs often don’t guarentee you a job. You won’t have one lined up at the end, so you’ll only be delaying your job search. Good news is that one year of residency makes you more marketable and more likely to get hired for your first job, at a higher salary than a fresh newbie. The residencies in my area pay in the mid to high 80s, and then you can expect to make 90s+ after that year. Totally worth it to me, and I’m all about earning my pay. 80ishk is not even a bad salary when you’re new and inexperienced. Especially compared to other starting salaries

    Imagine going through a master's or doctorate level program and having years of relevant work experience only to be looked down on to the extent of some of the people in these comments. The most unsafe and unethical practices I have personally encountered were at the hands of physicians. Most NPs are highly committed to providing exceptional care and always refer for anything outside of their scope of practice. Physicians aren't worried about patient safety. They are worried that they will be replaced by someone making way less.

    I love the idea of APP residencies/fellowships and getting the extra training for sure . The thing that I see that’s risky about it is , if more people are doing APP residencies to the point where there is tons of competition for fewer residency spots, and then the residency trained NPs are the ones getting the jobs first because of their extra expertise and training, where does that leave all of the NPs that went through school and maybe couldn’t get into one of those residencies because there’s obviously tons of NPs graduating out there and there aren’t tons of APP residency programs? So lots of NPs will be going through school and then will have massively hard times getting jobs, struggling financially for months until they find jobs, tons of stress and feeling inadequate , taking out tons of loans and having a terrible time finding a job etc ?

    Thank you for saying that all NP’s should go through a residency. I don’t understand the sense of entitlement and ego from NPs that claim to be exactly the same as physicians and want full autonomy and higher pay, yet attend an online school with only 700 hours of clinical and claim their years of nursing experience makes them just as prepared as physicians -.- I heard an NP student the other day at my work (ER) refer to another NP as a physician. I thought it was wildly inappropriate. We all need to remember our scope of practice and actual extent of training for patient safety. Nursing experience is not the same as running a differential, taking FULL responsibility for an order and prescribing meds etc….and it’s certainly not the same as 7-10 years of medical school/medical residency at 60-100 hour work weeks. I love NPs, but we need to stop blurring safety lines, especially in hospital settings like ICU or ER.

    Completed NP residency a while ago in Hawaii, and was one of the best decisions in my nursing career. Our Cohort comprised NP's from all over the country, over 30 NP's, some from Yale and Columbia just in case anyone thinks Ivy leaguers don't need more excellent training. Every Thursday, we had lectures and presentations from MDs that were fantastic and we presented on various topics as well. We spent 4 days in Connecticut with some skills training including Nexplannon insertions and when we got back we had suture training/digital blocks etc. At our various clinics, we had our own patient panels, took care of everyone including walk-in's,(seeing up to 15-18 patients/day), and then there were 7-8 week specialty rotations for the entire year with the endocrinologist (took care of the diabetics, graves disease, hypo/hyperthyroidism, Hashimoto's, etc), the nephrologist (assessed dialysis patients, ordered labs/meds), dermatologist (one on one shave/punch biopsies), orthopedic surgeon (joint injections), general surgeon (minor sutures), podiatrist, pediatrics, internal medicine, family medicine, women's health, and emergency room. This was a huge plus when applying for an NP job. The pay sucked while in residency but after completion, the compensation was beyond my wildest dream, signed the job offer so fast my pen almost broke. I hope this inspires those who are willing to learn, learning never stops just because we have 20 years of experience. Best wishes ya'll.

    • @CrushOnYourCurls
      @CrushOnYourCurls Рік тому

      In nursing there isn’t really a difference. In medicine, a residency is mandatory, a fellowship is not. Residencies are broad to their specialty and a fellowship is specific. For example, a family medicine residency, and an addiction management fellowship.

    Feel the opposite. FNP's I think need more training out of school than Acute Care NPs. Acute students have a background and training that is far closer to that of a clinician than an FNP student.

      Due to COVID, I have recently seen ad regarding NP/PA starting their own independent telehealth or telemedicine with full practice authority. I assume it is not as easy as not all states have full practice authority. But yes, working in one would be lovely to learn. So, I know what I would be going into.

      I believe for NPs they are pretty much the same thing! Just different language. I imagine maybe a fellowship has more of a specialty focus and a residency is for a more broad area like primary care? maybe?

    1. Nurses are not realistic about becoming NP and pay.
    2. Yassssssss… there should be a required residence program.

    I wish if nurses stop playing doctors. No residency years will compensate for lack of foundation knowledge that going to medical school provide. Nurse practitioners can go to any specialty and practice without training in this speciality. Do you know how many years physician has to spend in training before becoming a cardiologist for example?! 3-4 years . This is after 3 years of internal medicine residency. This is 6 years at least for supervised training, and after 4 years and in other countries, 7 years of medical school, All supervised by senior physicians. NP training does not come 1/10 close to this training. If you are reading this message , please make yourself a favor and see a medical doctor even if they sign you up with a nurse practitioner. It is your right.

    I think standardizing NP programs and making the standardized curriculum more rigorous should be priority for NPs. That being said additional training options are not a bad idea. However calling this additional training "residency" or "fellowship" is just another blurring between nursing practice and medical training, which is undoubtedly much more rigorous and time consuming. When someone says they went to residency or that they are fellowship trained, the layman assumes you are a physician. These additional training programs are probably more consistent with certifications. Medical residents are licensed as well, with an independent license possible after intern year of residency, and I think comparing any non-physician practitioner training to medical training cheapens the training medical trainees have to complete.

    I honestly can't believe people would actually have the entitlement to get mad over something like the NP residency salary. Oh, just because you're not making what a full-practice NP makes? Like bro, you're a legit BABY NP. You can't do jack sh!t compared to a full-practice NP right out the gates. Freaking be glad you actually HAVE a residency program to utilize and that you actually managed to get into one. Like do these people even know about all the sh!t that new med school graduates have to go through in THEIR residencies? Utterly inhumane and nightmarish hours, at EVEN LESS pay than NP residents, and often for an even longer commitment of time than just 2 measly years. And that's not even including fellowship if they decide to do that, which is probably just more punishment but with learning greater scope of practice. Honestly imo, NPs are utterly SPOILED compared to doctors: getting to do a lot (though not all, and rightfully so) of the same things doctors can do, at 20x less stress and burnout, much shorter (AND cheaper) education time and cost, and for a pretty damn good salary at the end of the day. Like be grateful, you little brats. :| Might get some uber woke snowflakes tantruming over this, but I got my popcorn ready to enjoy the dumpster fire show. 😎🍿

      That said tho, I wanna join a residency program now when I get to be an NP, God-willing. I would love nothing more than to be able to get the full experience and training likened to what a new doctor would receive (though at hopefully much less painful hours x_x). I will literally take as much mentorship and experience as I can get before just being thrown to the wolves.

      Well I’ll tell you what my beef is. I am in a residency so I won’t address any of the peripheral stuff but I will say that pay wise, I see plenty of positions where they ask for 5 years of experience, suturing skills, minor procedural skills and then they pay 125k yearly and that’s in a high COL state. I think that is insulting but it is status quo and so I can understand why people think it’s a waste of time, financially, to do a residency. At least an MD will get something back in terms of pay, NPs typically do not.

    Kind of like medical school? Medical students have 5000-6000 hours of clinical experience thinking like a physician, acting as one, and observing them upon graduation. The minimal experience as a nurse, acting as a NURSE, after non-standardized NP training isn’t anything “like medical school graduate”. And “you need a little bit of help?” Please stop using the word “residency”. It’s not anything like a residency. It’s not 15,000+ hours of extraordinarily supervised training. “Baby NPs” are wildly under qualified to do most anything without direct physician supervision. The notion that there is anything remotely similar to mid level extenders and a medical school graduate is insulting. You pandering to those skeptical by saying “I know some nurses want to do everything…” makes you no different. The only reason the AANP has had success in pushing NP independence is because major medical corporations love to pay you half or a third the salary and get rich off of your shotgun labs, imaging, and Google search practicing while patients know no difference. Poor patients.. they see you all in your white coats introducing yourselves as “doctor” with your DNP degree and they think you’re actually qualified to be caring for them independently. It’s truly pathetic.
    Call it postgraduate training. It’s not a residency.
