My interpretation of the reason why Lee's face is so messed up when he kills O'Hara is a bunch of things. 1. It's not easy to kill a man. When I first saw this with my uncle I said the exact same thing, "Why is his face so pained?" And that's what my uncle said. 2. Obviously the character of Lee is very good at combat but he's also a monk. One wonders if he went there BECAUSE of the death of his sister. So as a monk, killing O'Hara is like breaking a solemn promise he made to himself or maybe god. 3. The temptation to kill O'Hara now that he knows he's the one who kills his sister has to be a struggle. "Don't think. Feel." comes to mind. He's already going to a tournament where he gets to unleash his powers, unlike the monastery, on Han's island he can become a monster, so his challenge is to NOT kill O'Hara. So when he wins the game, he successfully doesn't kill him, just embarrasses his ass. But when O'Hara forces his hand out of pride, he ultimately fails that spiritual challenge. So yeah, he already knew he could kill O'Hara, but when he finally does it. It's sad for him. He gets revenge for his sister, it doesn't feel awesome. And he's a little angry and sad because it didn't have to be that way.
Once Bruce hit Jackie Chan on the set of this movie, said he was sorry, and Jackie pranked him a little, telling him every time they've met that it hurts, Jackie afterwards said that Bruce Lee apologized every time after that.

Did you know Bruce was so fast they had to adjust the camera to slow his movements.. he's was a very rare human being In which his muscles ( fast twitch) respond like electricity..on top of that he was very!.very! powerful for a man only weighting 130-35 lbs..he was wayyyy! He tells people that yes, Bruce was around 135 lbs., but he could hit like a heavyweight. One of the largest bags he trained with weighed 300 lbs. Dan Inosanto still has it lying around. The core of it was packed with scrap metal. There are videos of this monstrosity hanging in Bruce’s backyard while he ran kicking drills on it.
