    This three-dimensional animated reconstruction shows the accident sequence for a collision between a combination vehicle and a 15-passenger van on Interstate 65 (I-65) in Munfordville, Kentucky.
    This animation illustrates the overall accident sequence from just before the combination vehicle left the roadway until the vehicles were at their final rest positions. The motions of the vehicles shown in the animation are based on the reconstruction of the accident. Computer simulation models were used to calculate the motion of the vehicles prior to the collision between van and the tractor of the combination vehicle. These computer simulations used the initial speeds (calculated to be near the posted speed limit of 70 mph) along with the three-dimensional survey data of the physical evidence to reconstruct the motion of the vehicles. The post-collision motions of the vehicles could not be simulated precisely because of limited physical data and model limitations, and the motions shown in the animation are based on the physical evidence of impacts and a general reconstruction of the post-collision trajectories of the vehicles. The animation does not depict the weather or visibility conditions at the time of the accident. The animation includes audio.
    The animation sequence shows the combination vehicle departing the left lane of southbound I-65 at a shallow angle and entering the 60-foot-wide depressed earthen median between the southbound and northbound roadways. The combination vehicle then travels across the median, striking and overriding the four-cable, alternating post median barrier adjacent to the left shoulder of northbound I-65, and then crosses the left shoulder and enters the travel lanes of northbound I-65. At that time, a 15-passenger van is traveling northbound in the left lane. As the combination vehicle crosses in front of the 15-passenger van, the 15-passenger van strikes the tractor of the combination vehicle. The 15-passenger van rotates clockwise, becoming engaged with the combination vehicle, which continues across both travel lanes and the right shoulder of northbound I-65. As the combination vehicle and 15-passenger van travel across the right shoulder, the van separates from the combination unit, strikes a vertical, cut rock slope, and rebounds back into the travel lanes, coming to rest in the left lane of northbound I-65, facing south. The combination unit tractor strikes the rock slope beyond the shoulder and rolls onto its right side. This animated sequence is approximately 11 seconds in duration.
    The animation is shown from three different viewpoints. The first viewpoint is a chase view following the combination vehicle with the label "Combination vehicle view", the second viewpoint is a chase view following the 15-passenger van, with the label "15-Passenger van view" and the final viewpoint is an overall view that is focused on the combination vehicle as it crosses the median and overrides the four-cable, alternating post median barrier, with the label "Median crossover and impact view". The three views are presented twice divided by the title "Repeat of Animation Sequence".

    Crossover accidents happen in the blink of an eye, for all of your brilliant commenters wondering why the van didn't just "swerve out of the way" at 70 mph. The TRUCK DRIVER was distracted and fatigued. The company Hester, Inc was shut down three months later, but the owners just did a Bad Accident Chameleon and resurfaced with a new DOT number. They even had the nerve to keep Hester in their name! Only one passenger of the Mennonite family traveling to a wedding survived, a baby strapped in a carseat that was in the rear most seat row.

    Each going 65 mph it happens in the blink of an eye. I suspect this is a case of truck driver looling at the inside of his eyelids....sleeping.

    I'm captain on towboat,we have Deadman alarms. If no movement in so many seconds while moving it sets off alarms on boat. What im saying is that could also on a truck shutdown engine and apply brakes after so many seconds of no driver movement.

      only if it's a semi. they'll go through cement barrier. it stops most normal cars though.

      Pretty sure the all or most of the van occupants died. If I remember correctly they were amish or something similar.

      This accident was a example of the freedoms that conservatives celebrate.
      1. Freedom from proper oversight of the commercial trucking industry (get the government out of the way of business):
      Truck driver had a terrible driving record, yet was allowed to keep driving. Employer had a terrible safety record, falsified logs, ordered drivers to commit illegal actions, and engaged in deceptive business practices in order to retain its license to transport freight, yet the punishments they faced were negligible, amounting to a few tens of thousands in fines.
      2. Freedom from safety laws (don't need no gubmint to coddle us):
      Driver was using his cell phone and lost control of his vehicle. (Road conditions and mechanical issues were not a factor.) Driver only had 4 hours of sleep the night before. Crossover barriers in that great, free conservative state of Kentucky were inadequate and cheaped out. Passenger vans with more than 10 seats (this one had 15) were exempt from Kentucky seatbelt laws. 3 out of 12 people in the van survived, most of the dead weren't wearing their seatbelts (guess it mussed up their clothes). (note - truck driver died, too.)

      However what is depicted here is no response by the van at all.

    I'm not blaming the van driver in the least, but if this animation is accurate, it doesn't look like he/she applied the brakes or took any evasive action whatsoever. Maybe texting? There was time to do *something* if he/she was paying attention.

      well the simulation doesn't show everything. for instance, the crash happened early in the morning (5am) at the end of march. definitely would have been harder to see than the simulation makes it out to be.

      When driving at higher speeds you get tunnel vision, the drivers attention is facing forwards and only forwards with limited field of view. Most events happening to the side are not processed by the brain. When the truck entered her active FOV to late to react

      At that time of day, the van driver would've seen headlights coming towards them. Probably thought the truck was just another passing vehicle instead of a vehicle coming straight at them.

      The van drivers choices were as follows: 1. Turn right to avoid collision with truck and crash head on into granite wall. 2. Turn left to avoid collision with truck and crash head on into on coming traffic at 70 mph. 3. Crash into truck.
      I would say the van driver didn't stand a chance!

      @@chrisstromberg6527 that's a wide median. The van driver wouldn't necessarily hit oncoming traffic. However, being as how it was still dark out I don't think the driver would have seen the problem in time to react.

      Most likely right in line with the a pillar. My work van requires i do a double take looking left every intersection i stop at. Sucks but had too many close calls because of it

      First, no one expects to see a semi coming right at them. It takes a moment to get past that surreal aspect. Then what? Dodge left or right? Could you tell where the semi was going to end up while it was still bouncing around in the median? And yeah, like someone else said, no brake lights in the animation.
