How sensitive is MRI for detecting this? Also, is it true that the disk will always be less than 100% after repairing an annular tear?
Finally, do you have any opinion on Discseel? Thanks!

Thanks so much for posting this! I was a PT with neuro specialty, also did backs but not my specialty. I think I have an annular tear. I am doing some PT with neutral spine and avoiding lifting pushing pulling bending and twisting. I started doing extension exercises on my own that seemed to let me sit longer than 5 minutes without getting stuck... now with neutral spine I'm seeming to get worse... or was it that I had to sit a long time...

Thank you so much for the clarity ..... This is the most info I have heard since I had my L4/L5 annular tear diagnosed through an MRI I waited 16 months for. It's a martial arts injury. I have received no physio on the NHS and was told at my results meeting to get back to what I was doing before as no sign of herniation was visable. I still have great weeks and really bad weeks. Yoga therapy seems to support for a few weeks then flare ups happen like I am in the middle of. Would you recommed a private physiotherapist ? as the chiro & massage only alleviate for a short while.

I think a physiotherapist who specialises in back rehabilitation is the way to go. Massage is palliative and will feel nice but not address the issue. Manipulation may also give ease but you need to address the muscles to reduce the disc loading

Dublin Physiotherapy Clinic what do you mean address the muscles? As in strengthen them?

Thank you for taking the time to make this video👍🏻The M.R.I confirmed that have a bulging disc with an annular tear and I also had a lumbar lateral shift with it. My spine is still not midline after 6 months and it looks like it’s gonna stay like this for the foreseeable. Isn’t life grand!

Dr. , I have been experiencing lower back pain due to annular fissures for the past 3 weeks. But prior to 3 weeks ago, I was working out regularly-alternating PiYo and rebounding on the JumpSport (bungee chords). Both workouts are low impact. The rebounding healed my Achilles’ tendon from an injury two years ago and I would especially like to resume that one. Would you recommend that I resume those workouts and how soon? Thank You in advance.

This was very helpful to me. I have two minor disc bulges. Both have annular fissures with small central disc protrusion. My doctor thinks the one at L5-S1 is what is causing my discomfort. It has been frustrating to me because I have been dealing with this for about 6 months. After about a month I was feeling much better and I gradually felt better every month until I hit the 4 month mark. Since then I have had a few set backs (which I did not expect). Both my chiropractor and doctor told me that the disc needs to heal and that I would likely have set backs until it does heal. I could not see how that would happen. I am wondering if the healing will be gradual and I will slowly feel better or will I wake up one day 6 months or more from now and just feel better? In the meantime, it does make sense to try to find the balance of working on my core while not irritating the disc/nerves.

What is the best minimally invasive procedure to treat annular tear today?

    • @DublinPhysiotherapyClinic
      @DublinPhysiotherapyClinic  4 роки тому +3

Stem cell injections would be an attempt to repair with minimal invasion but is very much in the trial phase. Epidural's sometimes tried but its not a direct treatment of the annulus. Usually managed non-invasively

10 years this year.. Misdiagnosed with neurological disorder i questioned from the start & the trauma that began it all after a fall affecting the sacrum directly completely ignored.
Later fall caused cervical issues . Chiari malformation type1 found but they were not interested in investigating further & being proactive and an active person they said i was doing the right things however the right things, martial arts, dancing, seated meditation & yet things got worse to the point of being unable to work. I couldn't work any longer in practical conservation with heavy tools & lifting. Hill walking, Dancing, grocery shopping causes daily issues. I still try to do it all as I'm not afraid and can't sit or lay for tooo long before spine hipps seize up.
Huge impact on mental health & after pushing further with new gp my recent mri report has annular tear l4/l5 & I also have bilateral sacralised l5 which i think is also causing symptoms. GP told me there is nothing to report on the MRI February so again thinking I'm crazy but I accessed the report & there it is. Livid again.
I have a connective tissue disorder which was diagnosed after the first fall but they have ignored my pleas that something biomechanical was going on. I finally have a good P/T and we've been working on neck as have incredible pressure headaches but maybe need to work from the bottom up after investigating.
Thank you, excuse the rant. These videos are incredibly helpful as I've found most P/T to be. Some Gp & specialists arrrgh 😠

I'm 47 and in this same boat, except compounded by several problems simultaneously. L4-L5 has a left disc bulge with a left posterior lateral annular tear and a compressed nerve root. L5-S1 has the exact same set of problems, but on the right side. To top it off, L4-L5 has signs of bilateral facet disease, and L5-S1 has signs of disc dessication. Add on the fact that I can't take opioid analgesics, and you're left with one hell of a mess. I'm not sure what doctor's are going to do about all of this, because it's not just one or two issues, it's several across multiple critical lumbar. The doctor in this video talks about people losing faith in their spine and that is exactly where I'm at, in addition to only being able to take acetaminophen and anti-inflammatories until physicians can figure out what to do with me. My faith in recovering from this is quite low.

If I have an annular tear that you showed up on the Mri and it is absolutely positively causing extreme pain on both sides of my sacroiliac joint, is it safe to slowly build my core gently when I have an annular tear from L5-S1 also there are some bulging discs and some arthritis and other discs all in the lower lumbar that all show up in the most recent MRI plus some spondylosis and disc degenerative disease, I was bowling a year and a half ago and then I got depressed and spent two months in bed which made my core totally horrible, I cannot be on my feet more than five hours or I am in excruciating pain the only thing that has helped me right now Is having four oblations done Under twilight sleep which actually next month will be the fourth one, is it dangerous to slowly work on my core now slowly and carefully???? I have done physical therapy and I t didn’t do anything And I am doing acupuncture and the only time that helps for a little while is when he hooks up some electricity to the needles during acupuncture I have wasted 13 visits and now he is trying the electricity and it helps for a little bit, thank you in advance you are absolutely phenomenal

Annular tears never heal despite claims they do. PT, massages, chiropractic care, steroid injections, etc are worthless for this condition. They are designed to ransack our wallets. We have a physical tear inside our bodies that will never heal because there is no blood supply in our spine discs. The only real option is fusion or
artificial disc replacement. That's it. Unfortunately spine tech is still in the stone ages and those are currently the only two stone age procedures that can potentially resolve the pain

Well thats a very general request but you need to Create the environment for repair ie
      Control static loading
      regular change of positions
      reduce flexion rotation torsional load
ensure segmental motion throughput the kinetic chain
      optimize muscle function
      address contributory ergonomic factors.
