For dental work try Agha khan hospital, it is expensive but worth to try and may be you will get some good dentist plus good cleanliness and will feel comfortable to get work done.

Not an easy decision specially when young kids are involved. Depending on where in Pakistan you will decide to live in, some parts of Pakistan are better than others. If you ever move back then try to find a better place to settle as your family will get used of western world comforts. Get the passport for sure. All the best.
