- I’ve just finished my FIRST MONTH as a resident doctor! For this rotation, I was on the gastroenterology (GI) service, the subspecialty of medicine that deals with our "guts".
In this video i’ll update you about what residency's been like so far, the best parts, the worst parts, the craziest things i’ve seen and my most embarrassing moment! I even have a short "soap box moment", so let me know what you think by commenting below!

I’m so happy you touched on how alcohol can destroy the body. I’ve buried 3 friends as a direct result of alcohol abuse. I wish they would put warnings on the bottles like they do with cigarette packages. I haven’t touched a drop of alcohol in over 28 years
Meanwhile, theres people like my grandpa who are in their 60's, been drinking their whole lives, and hardly has gray hairs yet. I don't get it sometimes.
@@Daedroh my father got gray hair since the age of 30, and he drinks like 5 times per month. On the side note, I only drink like 5 cups per year.
I hope you and Doctor Mike will do a collab someday 😊

If I had watched this 5 years ago, I would’ve went back to college to go to medical school. Perfect time in my life too. Now I can’t stop working because I won’t be able to afford to live lol. Go Siobhan! You’re such an inspiration!

Stumbled upon your channel. I have Crohn’s disease and have had managed stays in the hospital here and the US and I was a nurse in GI dr office. I love GI. Crohn’s not so much lots of surgery and hospitals and meds and pain. Thanks for your wonderful help and I wish all my residents I have see over the years we’re like you. I’m starting from the beginning you’re so beautiful too. Thanks.

My dad was for almost 13 years. And the rift was Rocky for a few years after. Alcoholism sucks. It affects not only the alcoholic, but also the family. The worst part is, he doesn't remember what he did cus he was always drinking. People just push alcohol like its a lollipop, but it's dangerous. It's deadly and traumatizing. I don't know how you're faring with it, but there are a lot of things that you come out of those situations with that not many people understand. Blessed be!
Hey super video! Im a nurse in a female gastroeneterology ward .. What you said about alcoholism and the effects it has is so so true! I had no idea how horrible it is to watch the effects of liver failure on the human body. We have many hepatology patients so i also agree with you when you said that its shocking how much asicites can be drained from the abodmen .. I had a patient once undergoing tapping who drained around 10,000 lr too .. Super shocking ! Keep up your good work and videos :)

This was actually really interesting to watch as usually when I am in the hospital I am just on the psych ward but last time was a really bad OD so was followed by the GI team because of my kidneys so knowing what goes on/was going on gives me more insight
It’s so cool seeing you go running when code blues occur I like seeing the action since my old mom crushed my dreams of doing this. I’m going to prove her wrong though

I love how you addressed alcohol abuse. My uncle has been drinking everyday all day for many, many, many years now. But thankfully, he has decided to stop and is on his road to recovery now!

My uncle passed a few weeks ago from alcohol abuse...it was years we tried to help him, but you can only help those who want to be helped. It was rough to watch him decay all the while saying he didn’t have a problem. It’s so important for people to understand what happens when you abuse any substance

Siobhan, I am soooo sory for 1 month late reply. Interestingly your comment was in my mind almost every day, but I always delayed commenting. Sorry.
I have just graduated from a med school in Turkey. Some differences in our system: There is an extremely difficult exam after graduation for residency, more difficult than USMLE Steps but after you got the score you need you will directly go to the residency you aimed. No reference system, no interviews, no nothing, just the score.
Sometimes The US/Canada and Europe/The UK publish their own guidelines for the same medical scenario and I guess because of Turkey's position, we had to learn both of them throughout the medical training.
Internal medicine is full of rotations just like yours, but some departments like Pulmonary Ds, Cardiology, Infectious Ds are divided from IM years ago. They are different departments now. Still you have to do rotations there too though.
Also I am curious about the difference between residencies in Canada and the US in terms of getting accepted, evaluation during the residency and working conditions from a resident's perspective (If there is any difference)

Love your videos! I am a new grad veterinarian so I work long, stressful hours too- do you have any tips on how to stay positive and maintain balance? Any suggestions for things to do in your time off to recuperate?
I can relate to the gentleman with a piece of steak stuck. I had a piece of hot dog stuck, after 3 days, had an edg procedure done to remove it. Made me wonder, if something like that had happened to someone 120 years ago, did medical science have the ability to deal with it.
My son was in the hospital for 3 months, he was only 5 months. He has had a rare case over the summer, he was also seen by the GI team, he’s had a hole in his bile duct. They still don’t know if it was caused by a gallstone or cysts. The leakage that led to his abdomen made his skin turn yellow and made his abdomen very distended. They had no choice to insert a stent in, so far so goods, it was inserted in August, he gets in removed in February. :)
I had to see a Gastrologist. I've been suffering constipation all my life. I actually didn't like my Gastrologist, I need another referral. Prune juice works amazingly well!! The problem is I don't want to take my laxatives. I do like prune juice, and last Friday it worked sooooooo well.

Hi Viola - great questions, it definitely varies depending on the rotation. I'll try to make a vlog in the next few weeks so you can see what kind of hours we have!
I just finished a placement on a GI ward in Scotland as a student nurse.. absolutely loved every minute of it, and so sad to leave! Got to take out a lot of those ascitic drains you were talking about. Also worked with some of the best doctors of my training so far, the consultants would let me follow their ward round and actually include me and ask the med students questions based on the nursing perspective of people's care. Also got to follow the 'bleep-holder' one night - that's what we call the person with all of the pagers :) and spent a day at the scope clinic - definitely very cool seeing inside of people!

Aw yeah the doctors on that ward were amazing :) normally they just deal with RNs but they made such an effort with us students too it was really nice! Looking forward to your next videos!! xx
Now there’s this thing on WhatsApp where you can delete a message on a group after sending it. ( limit is 7 minutes right after sending ) . Having that would’ve saved youuu trouble 😂😂😂🙌🏻

I have a question if you see this. So you so 4 years of med school and then you do residency which is like rotations? All different rotations? I’m in PA school not MD and we do 15 months didactic and then 12 months rotations at various places but we don’t specialize. We can go into a specialty but we learn on the job that hires us.
My question is because I thought in American med school you do your 4 years and then you pick your specialty and do both residency and internship in the specialty you pick (or family practice or something). I am not sure if that’s how it works or not though. So I was just curious.
Also I’m interested in the GI and i May do that in one of our elective rotations because I have crohns so I’m very aware of how people feel with it. And I absolutely love my GI doctor I’ve had for 6 years now. He is amazing and I believe he really cares. I have apparently an interesting disease process i guess or something. He actually took my case to a convention to ask some other doctors what we should do

Doctor I been suffering with GERD recently this year its gotten worse and I found out I have a weak LES. I can’t even have fruits. I’m on ppis and have been for 5 years but its not helping anymore. Any recommendations?

do internists deal with patients coding? (i know they don't happen too often and usually in the icu, right?)
Hi Caleb! great question - short answer is yes! Different hospitals are set up slightly differently, but typically there is a whole team that shows up to codes - internists, residents, anesthesia, ICU nurses etc. Where I work, on call it's the senior medical resident who runs the codes. There certainly are more codes with sicker patients (and ICU patients are very sick), but you would be surprised how many codes are called on the ward or even in the lobby of the hospital! In other words, when I'm on call, I carry a "code pager" and as an internal medicine resident, I go to all the codes.

Wait, I'm actually a bit confused. So you're a resident now, doesn't that mean you're beginning to specialize? I know the curriculum at your medical school in particular is a bit different, but I thought you did rotations while still in medical school (after the didactic years) before you graduate? Could you maybe explain it a bit? I'm Dutch so North-American systems are different anyway, but I don't know if the terms differ between Canada and the US.
Hey Mariza! Great question - it is a bit confusing! So you're right, in medical school we do a very BROAD group of rotations to get exposed to the different areas of medicine (i.e. surgery, paediatrics, obstetrics, family medicine etc.). When you choose a residency program, you still do different rotations but they are designed to enrich your learning as a specialist. So for me, I'm in internal medicine, so I do rotations in internal medicine SUB-specialties (i.e. gastroenterology, hematology, cardiology, rheumatology, even ICU!). So in other words, the set up is similar to medical school with different rotations, but they are more tailored to your particular residency program than the whole medical field. Does that make sense?
Violin MD Ohhh so it's actually quite similar to the Dutch system (and basically every other system in the world)! I want to go into emergency medicine and during my specialization I have to do cardio, IM, peds, neuro and pulmonology rotations as well as paramedic training. I saw your video about being on call and I heard emergency room and stuff but I guess it didn't really register properly that you had to be there as a part of your IM specialization. Thanks for your quick response and clear explanation! I really like your videos, so keep it up! :)
