Ferrule Effect - A different point of view of the preparation of endodontically treated teeth.

  • The success of crowns placed on endodontically treated teeth is often associated with the presence of the so-called ferrule effect of the crown that should encircle 2mm of existing sound tooth structure at the axial walls, in order to reinforce the tooth and prevent future fractures.
    Is it so?
    Or could there be another simpler explanation to this problem that has been puzzling dentists all around the world for many decades. Sometimes the different way that we look at things may help us override difficulties, as it is often the case with endodontically treated teeth.

    Such a good point of view! Have a patient tomorrow and was thinking about how to mantain the ferrul.
    On the other hand what about your other video, where you prepped subgingival, filled with flowable, than prepared for crown. What is happening with the resistance (and sound tooth structure) on that case?

      +aron dombi when there is no sound tooth structure at the cervical area, it is there that I think that it is essential to use a glass-fiber or carbon fiber prefabricated post and use flowable composite for the build up (actually this is the technique that I have been using for over 10 years in my private practice). In this way that core has some flexibility and the least possible forces are transferred to the root and the problem of post documentation is practically solved. After the post and core build up, the preparation follows the classic rules. If there is no tooth around the margin I do not intentionally do crown surgery or deep sub gingival preps. The flexibility of the post and core saves the day... But this is the topic of another video...

      The patient had the upper center incisor fractured, probably due to short metal post and conventional cement, but she thought that the lateral incisor (#12) we had restored, was fractured!

      The doctor who made the Central incisor restoration used metal lab made post, as I mentioned before, and porcelain crown (PFM) over it. We made the Lateral incisor root restoration, using fiber post and dual cure resin cement and on top of it lab made porcelain crown@@HAZEMDENTAL
