    If you’re new to my channel, my name is Dr. Drew Timmermans, ND, RMSK. I’m the founder of Regenerative Performance in Gilbert, AZ, specializing in interventional orthobiologics, which is just a fancy way of saying we help people reclaim their life from chronic pain without steroids or surgery.     How I got here…
    22 yrs old: Graduated with a Bachelor of Science in Kinesiology from the University of Western Ontario.
    23 yrs old: Competed as a long sprinter, anchoring the 4x400m relay to a silver medal at the Canadian national championships, contributing to Western's first national title in men's track and field.
    23 yrs old: Started working as a personal trainer for GoodLife Fitness in Canada, quickly becoming one of the Top 10 Personal Trainers nationwide.
    23 yrs old: Herniated two discs in my lower back, ending my track and field career and sending me into a multi-year battle with chronic pain.
    24 yrs old: Enrolled at the Southwest College of Naturopathic Medicine (now Sonoran University of Health Sciences) after discovering my passion for naturopathic medicine.
    26 yrs old: Still battling chronic back pain, I finally became pain-free through a platelet-rich plasma injection.
    28 yrs old: Graduated and become a licensed naturopathic doctor.
    29 yrs old: Completed the first-ever residency in regenerative medicine and integrative pain management at the Neil Riordan Center for Regenerative Medicine.
    29 yrs old: Co-founded Regenerative Performance to help people with chronic pain just as I had been helped.
    Today: We help people who have chronic joint pain reclaim their life with cutting edge treatments like platelet rich plasma, and stem cell therapy.

    So nice to see someone who knows what they are talking about.
    My issue is a bilateral burning sensation around the upper buttock, superior Cluneal nerve areas on the skin after sitting. I had this slightly for some time but would adjust my seating position so that that area would not come into contact with what ever I was sitting on and it would then be manageable.
    However after getting a new and very hard sofa it flared real bad to the point I was having a constant burning of the skin like a sunburn.
    Now though after not sitting or driving for 3 weeks the burning has lessoned quite a lot.
    I have to say also around this time I was doing a lot of stretching for my prostatitis/cpps which I think might of made it worse as the nerve may have been inflamed.
    All scans confirm no other problems with my back
    Can I ask in your expert opinion Dr, can conservative treatment like not sitting help heal the nerve?

      Conservative treatment can be helpful yes, but if it's not significantly improved or gone with 6 weeks of conservative treatment, you need to do something stronger such as PRP nerve hydrodissection

    This is almost certainly what i get. It's on my left side, often starts as pain/tightness in my lower glute. Lower back then spasms and gets very painful and puts me in bed for days.
    Never heard of it until now. Ill ask my physio. Thanks

      Definitely ask about it! That also sounds like it could be an annular fissure in one of your lumbar discs

    This was very helpful. Thank you! Are there any exercises that can help with this? I’ve been suffering from Cluneal Nerve Pain for several years from playing Ice hockey. My doctor diagnosed me but didn’t offer the solutions you mentioned here. I asked for injections but he stated he didn’t want to go that route because I was still young and healthy.

      I'm not a physical therapist so I don't recommend exercises. I use interventional orthobiologics like PRP and stem cell therapy and then we refer to good physical therapists

    I am having what appears to be back mice removed from my left iliac crest area. It has taken 8 years for anyone to listen and actually investigate the lumps there that seem to be the cause of a lot of pain. They are at the subcutaneous level and while no vessels or nerves are there, my theory is the lumps push on cluneal nerves beneath them causing inflammation and pain. Doctors I have seen seem confused by this theory but I think it's fairly logical. Also to note a lot of Doctors including specialists know absolutely nothing about cluneal nerves. They just look at the spine and nothing else. I have been getting steroid shots which help short time. Here is hoping the removal of these lumps brings relief. If not I will be looking into flying to your clinic for an appointment.

    This is definitely what is wrong with me it actually is so severe it causes critical BP/HR. Unfortunately patients are dismissed like drug seekers and left to suffer inhumanly now because of the opioid epidemic.

      I'm terribly sorry to hear that. If you ever want to explore treatments that treat the root cause, explore nerve hydrodissection with d5w or prp

      I agree and it's infuriating, I challeneged a Doctor on that point by saying I know you Doctors are being trained to ignore the cries of pain from your patients but some actually really do need the pain medication. She denied that but I said I respectfully disagree, I am addicted to being pain free, not drug addicted.

    Can you dry needle this area?

      Yes and sometimes that's enough to calm down the muscle tension that's potentially causing the nerve entrapment. If it doesn't work, we recommend nerve hydrodissection

      @@DrewTimmermansND Thanks I have a couple of clients with pain in this area. I have been doing joint mobes at L4 along with long leg traction and cupping and the Dr. McGill Big 3 exercises (Trunk Ext, Side Planks and Bird Dogs) to help.

    May I ask where you practice, what city & state? This is exactly what I have. Kaiser repeatedly sends me to physical therapy which causes more pain.

    I have a New Pain Mangement . What a Joke they are. My First Apt. they really didn't let me tell them where my Lower Lumbar Pain was. NOW without a Physical Exam this so Quote Doctor told me I need this ' Superior Clunreal Nerve Block.. WHAT ? WHY ? I Pray this doesn't MESS ME UP in the Long Run.. I have a Bone Protruding down by the Scrum, Illiac area. I"m VERY NERVOUS.. Doctors that are Arrogant, think they know it all.. And are NOT Thorough should be FIRED.

      @@DrewTimmermansND I'm on Whidbey Island.. Can you at least tell me what would be the Signal's or Symptoms if you need a Superior Cluneal Nerve Block.. I have no Groin pain.. No Sciatic nerve down leg.. little bit of Buttock pain...I have Coccyx , Sacrum, S.I pain..

      @@eaglewind77 pain doctor here. I do a lot of cluneal blocks. They are very low risk so i wouldn't worry too much. Middle cluneal is tender inferior to PSIS (..hand on hips and bone under your thumb lower down is PSIS -round there). Superior is tender over iliac crest (hands on hips location). Get ice and looked for altered sensation to ice (feels less or more cold compared to other side if one sided pain or to above if both sides). Altered sensation = nerve issue. Nerve pain is constant so often worse at night (most pains better when lying still at night).
