I've never seen this concept explained so well. What's missing though is another effect of accumulation of potassium in the cell and its subesquent diffusion into the tubule - this causes positive membrane potential which causes Ca2+ and Mg2+ to be secreted, an important effect of loop diuretics.
As a 3rd year student I've had many patients taking furosemide and it helped them a lot, but I had not done a more detailed study into it than now. Thanks a lot, this video is very helpful

I don't have heart failure, but my left kidney can become big sometimes and my chest can be full of liquid, I can hear a weazing sound from my left lung. My legs are big and my left arm as well. To the contrary, my right arm becomes skinnier every day. I wonder if furosemide would help me.

Lasix is used to treat hyponatremia in SIADH by increasing serum sodium and getting rid of more water than sodium but the fact that lasix increases urine sodium while decreasing urine osmolality seems contradictory. I appreciate your time in answering this or directing me to a resource in advance.

Ive been taking Lisinipril, Coreg & Isosorbide for over a year and generally BP was out of control even though I lost 80lbs, exercise & fruits, vegetables, nuts & ocasional chicken & fish. I want to ween off the Furosemide to see if my hearing improves. Does anyone know of a natural alternative? I'm also taking Vitamin C, Iron, B12, Ashwagandha, CBD Oil & Lions Mane Mushroom Extract.
Education is important: It's good if it makes you pee a lot! This is the intended effect! Better to pee a lot than to end up in the hospital because you can't breathe. As a registered nurse, I have seen many patients with heart failure get admitted to the hospital. My goal is to never have to see you again, despite how friendly and pleasant you are. Take your meds; check your weight daily; watch your sodium intake; visit your doctor. If you can't do these things, I can help you with that, too! Stay safe and healthy.
But, people report that it damages their (dogs) kidney and cause failure, so what are the chances?? My dog have CHF pulmonary edema and has a hard time breathing, but I hesitant to give him furosemide with people reporting kidney failure.. i really need to help him feel better.. 😥

Hi Adil. Thanks for your comment. That is a great question! To be honest I am not sure of the answer. I always thought since the loop of Henle is where the majority of sodium and water is reabsorbed into the blood from the filtrate and the convoluted tubules have a much smaller role, when there is renal impairment the effect size of the other diuretics is too small to make a clinical difference whereas loop diuretics can still have an effect. I'm not sure if that is accurate but that is how I think about it.

I would like to mention that most of the sodium reabsorbed from proximal convulated tubule approx 65%, where the carbonic anhydrase inhibitor and osmotic diuretic can act, so why cant we use that option if we have low blood flow through kidney. In my opinion you are right but not sure, because if we use drugs that act on proximal tubule the large amount of Na can still reabsorb from later part, but after loop of henle no part can absorb that much Na, while in compromised renal function blood does not flow that far , which makes loop diuretics last option..
Renal blood flow is the main conceren. And retention of fluid (water and electrolyte) causes renal imparement. So Furesemide increases intrarenal prostaglandin production (e.g., PGI2), which increases renal blood flow. Synthesis of prostaglandins also may cause vasodilation of tissues and increase renal flow..thereby decreseing warer ans electrolyte retention and improve overall renal profile

I undrestand that after using loop diuretics, the natrium and water just stay in the tubule. But why aren' t they reabsobed further in the distal tubule, where the transport mechanism is not affected?
Good question. there will be some that is reabsorbed in the distal tubule, however this is only a small amount compared with what is reabsorbed in the loop of Henle. The distal tubule does not have the capacity to reabsorb a large amount of sodium and water
how much time lasix take to work its been 1 week result are very slow my both leg were swollen and ankle and cafs are swollen are still there .... please tell me how much time it take to reduce proper...and after workout my foot is Normal ... so please tell me what to do

this medicine does not work in reducing fluid, it gives renal failure and damages the kidneys. Some Doctors are stupid.

Almost all of the contraindications and side effects you listed are primarily seen in thiazide/congener diuretics, not loop diuretics.
