Top Mistakes to Avoid When Applying Hormone Creams

  • Hi, This is Dr. Raj Singh. I practice here in Las Vegas Nevada.
    We do a lot of hormone management in our practice and I am seeing a lot of mistakes being made and that is the purpose of this video correct those mistakes.
    What are some of Top mistakes when applying hormone creams?
    1. Applying cream on the outside skin of the body where the skin is thick. Always apply it to the inner skin because the outside skin is too thick so the cream will not get absorbed.
    2. Applying the cream to a very small area. Make sure that apply the cream least a 6-8 inches or at a minimum apply to area bigger than the size of your hand. if you have unusually tiny hands, then apply to an area the size of the iPad.
    3. Applying at other times when the skin is not clean. Best time is to apply after a shower.
    4. Applying estrogen creams to breast area. This can breast tenderness and also stimulate breast cancer. applying estrogen creams to the breast are contraindicated and should never be done.
    5. Applying cream to forearms. I have seen some videos recommending this approach, however I am not a fan of applying hormones to forearms. High risk of transferring hormones to those around you by direct skin contact. Inner thigh, inner biceps, torso are better choices.
    6. Using a cream every single day. Testosterone creams always skip a day a week. When using estrogen cream always apply the cream from day 1 to day 25. It can be the first day of the period and easier way is to start using on the 1st day of the calendar so if you started on January 1st apply till every day till January 25th and then do not apply for the next 5-6 days and then start again February 1st. This does minimizes any possible side effects to it keeps her receptors hungry and maintains a natural rhythm of the hormones.
    7. Try to save the cream by not using the prescribed amount. Remember that this a medication so you should use the prescribed amount. if you are running low on your medication changes, call your doctor to get a refill
    8. Getting blood drawn to monitor hormones. Medications applied on the skin travel to tissues using lymphatics and are quickly used by the cells. Blood hormone levels are not unchanged. Saliva Hormone testing is ideal. Another alternative to saliva is capillary blood spot testing
    9. Not rotating site. Please do not apply to same area every single day. Rotate the sites for best results
    10. Not rubbing enough. At least rub it for 1-2 minutes for optimal absorption.
    11. Exposing the cream direct sunlight. The hormones may get deactivated and may not get the optimal results. Keep your cream in a dark area.

      Hi Suzie, Ideally all hormone should be cycled especially in pre-menopausal state. After menopuase cycling is not required. Progesterone is a good balancing hormone, helps with sleep and calmness by binding to GABA receptors. I prefer giving one day a week hormone break to all my patient to keep the receptors hungry.     Thank you for the excellent information, Dr. When applying bio-identical estrogen and progesterone to the face, should they be used on alternate days? Does using progesterone intermittant with estriol keep the receptors "hungry" as you put it? Will the receptors in your face stop responding if you apply hormones 7 days a week (even if you are altering estrogen and progesterone)? If so, how long of a break does one need to reset the receptor sensitivity? Thanks again for your advice!

      Thank you for your comment. All these creams tend to be quite greasy and can cause clogging of facial skin pores. I generally advise my clients to apply to the face only once a week. And the other days, it should be applied to one of the recommended areas as I pointed out in the video. most of my clients will skip 1 day a week to give their bodies a break from all hormone applications. I do not have much experience or data to recommend for or against every other day application of estrogen alternating with progesterone. I would recommend speaking to your doctor who prescribed such a regimen. I hope I was able to answer your questions

      @@pancakepancake3789Progesterone downregulates the estrogen receptors but does not decrease the number of estrogen receptors. so even if estrogen is high it has a difficult time expressing its tissue effects in presence of progesterone. I hope this answers your question

    Hello, and thank you so much for this video. I am considering starting DHEA or estrogen cream to help several symptoms of aging. I am not under a doctors care at this time, just trying a low dose cream to see if it's beneficial. What is your opinion on DHEA cream & it's effectiveness on post menopausal symptoms vs estrogen creams?

      thank you so much for your comment. you have to be extremely careful when using hormones such as estrogen as they can stimulate certain cancers. A doctor's consultation is highly recommended. DHEA creams are expensive and not very useful as our skin absorption of creams goes down with age. Best is to take DHEA supplementation in a capsule form. Again you want to know if you DHEA levels are low before starting this supplement. DHEA can get converted to estrogen or testosterone and can cause acne problems and hair growth if excessive doses are used. Again, highly recommend seeing a physician before starting these treatments

    Not to be redundant, but thank you so much for offering such helpful information in a concise manner. I work for one of the top 10 hospitals in the US and cannot get one doctor to talk with me about the information you covered in your video/ comments. My PCP says it's useless to test hormones because they vary day-to-day and my gynecologist says that menopausal symptoms are just a “natural part of life" and wont prescribe HRT. What kind of clinic would I search for in my area to explore nutritional deficiencies, prolactin, cortisol levels and saliva testing and potentially prescribe HRT? Your clinic sounds outstanding!

Hormones are definitely difficult to manage and lot of providers tend to dismiss hormone issues. hormone balancing if done properly can make such an impact on quality of life. Probably look for clinics in your area that offer hormone replacement therapy would be a good start.

      I had a Dr who gave me the same dismissive behavior too. I left their practice and went to an OBGYN practice who has several specialist in HRT on staff . It's sad that women have to advocate so hard for medical relief against physicians who like to dismiss our needs. Hope you found someone to help you.

      Look for a compounding pharmacy in your area & then call them & ask what gyn or physicians they are filling HRT prescriptions for and get an appt there.

    Would it be worthwhile trying a compounded estradiol cream rather than Divigel because not sure if I'm absorbing it well. Would that be twice a day ?

      thank you for your comment. Generally we like to avoid synthetic estrogen creams as the estradiol source is from horse urine and tends to be significantly more potent than our natural estrogen. A bioidentical hormone replacement therapy might be more beneficial. If your using the hormone cream appropriately, then absorption is rarely an issue.
      If your symptoms are not better, then it is likely that your hormones are not balanced. Most of the woman are not low in estrogen. A lot of the vasomotor symptoms of Peri or postmenopausal are related to issues with progesterone, testosterone or DHEA. Since I do not have access to your health history, I am unable to advise you on the best dosing. Please discuss this with your doctor.

    If I'm in anticoagulation therapy with warfarin is sure use these creams?
    I think i can do it if I adjust the doses.

      Thank you for your comment. It depends on the reason you are on anticoagulation. If he have multiple blood clots it is not a good idea to be on estrogen therapy as these can increase the risk of future blood clots.

    I can't find any information on applying moisturizers over topical hrt creams. I use a retinol moisturizer - is that okay?

      HRT cream should be applied on clean surface. once it has been absorbed ( 2 -3 hours) you can apply other creams over that area.

      thank you for your comment.Hormone creams Should never be applied on the lower abdomen as they can increase risk of atrophy of testis in men and uterine or vaginal growth in females. Apply the hormones around the belly avoiding the lower abdomen area. Hope this was helpful

    When to apply estrogen and progestron creams. I heard estrogen in the morning and progestron at night.

      estrogen can be applied anytime of the day. Progesterone use is recommended at nighttime as it binds to GABA receptors in the brain which helps us fall asleep. you can use progesterone during the day, however it can sometimes cause grogginess and drowsiness

    Is it o.k right after a hot bath or shower? Skipping one week a month is also recommended after menopause when using bi-est and DHEA and or testosterone?

      Thank you for your comment. it is recommended to apply it after a shower on clean skin. One week skipping is not required post-menopause we recommend to skip 1 day a week to maintain receptor sensitivity

      Thank you for your comment. Hormones are very safe when used appropriately. Any time your on estrogen therapy it should always be balanced with progesterone to reduce risk of estrogen induced risks. since I do not have any knowledge of your medical problems, age and symptoms I am unable to answer this question properly. Progesterone creams are always safe, however when comes to using estrogen there many factors that have to be considered.

    Can different hormones be blended and used together on the same area? I’ve seen some info say they have to be used separately on different areas of the skin. If so, why? What happens if they are mixed?

      thank you for your question. Yes different hormones can be blended and used together in the same area. Only restriction is that if the cream contains estrogen it can not be applied to the breast area as this can stimulate or increase the risk of breast cancer. Women also can have breast engorgement and nipple tenderness when applying estrogen cream directly on the breast area

    Thank you so much for this information, I recently went to a new hormone doctor, and he took me off of estrogen because he believes that it may cause breast cancer, but estrogen has so many benefits and I feel I should continue to take it. it's about balancing hormones, so if I am not taking estrogen, then my hormones aren't going to be balanced. I think I need to go to an endocrinologist, I think they would be able to give me the information and proper care that I need, as it can be confusing when every doctor has a different opinion, which I don't need, I only need the facts, not what you feel, but what is based on my body needs. Perhaps, I should make a appoint with you Dr. Raj Singh.

      Thank you for your comments. whether or not estrogen therapy is needed depends on your blood tests and symptoms. Because we are exposed to so many environmental estrogens, most woman do not have estrogen deficiency. We use estrogen replacement in patients who have extremely low estrogen levels, have osteoporosis not responding to traditional treatment with bisphosphonates and some other indications. Most menopausal symptoms can simply be treated by replacing testosterone. Hormones are always tricky to treat as we had so many conflicting clinical trial in the last 20+ years. Your can always schedule a virtual appointment with our clinic in Las Vegas.

      First definitely get rid of that doctor (assuming you did that by now.) There is a brand new major study out & HRT is not cancer causing in most instances & estrogen replacement is absolutely necessary for so many aspects of women’s health including: heart (#1 killer of women) bone & brain (Alzheimers) not to mention estrogen cream for vagina which can atrophy (shrink!), extreme dryness causing uti, painful intercourse, etc. estrogen is vital to your health. No ifs, ands or buts about it.

      Thanks for your comment. Oral progesterone is meant to be taken orally and will not absorb when applied to the vulva.

      thank you for your comment. Generally gels or patch are not recommended options to help transition as the dose absorbed is low and unpredictable. Best option would be to consider pellets, injection or pills.

      thank you for your comment. Synthetic testosterone is man made based on testosterone formula. Bioidentical testosterone comes from soy bean oils and sometimes yams. I hope that answers your question

Hormone creams can contain estrogen, progesterone, testosterone, DHEA and other ingredients. the best combination of these medication can only be choose after careful review of history and diagnostic testing.     I have been prescribed Oestrogel & utrogestan capsules everyday continuous..i'm 54 & post menapausal. i'm not sure if your advice to apply the gel for 25 days & have a break applys if your post menapausal too ( not just peri menapausal) & if so do I take the utrogestan capsules for only 25 days aswell ?

      Hi Kathryn, thank you for your comment. You are currently prescribed estrogen and progesterone therapy. Unless you have a documented estrogen deficiency or symptoms of estrogen deficiency such as severe osteoporosis, estrogen therapy is not helpful and can cause many negative effects. My advise regarding hormones is because when we use hormones every day, our receptors get down regulated and become less responsive. Giving a body a little break from hormone therapy has better results and fewer side effects. I would definitely encourage you to revisit the need for taking estrogen therapy at your age.

      Thank you for your reply Dr. Raj Singh. I was going to take it for normal menapausal symptoms..hot flashes, joint pain etc. I dont have an oestrogen deficiency as u have suggested. I thought the benefits would outweigh the side effects & the risks but i do have my doubts. I will heed your advice.

    Which in general has better antiaging property? Estradiol cream or estriol cream?

      Estradiol aka E2 is the strongest estrogen so will have the most effect at lower concentrations. The other two estrogens Estriol and estrone are very mild and have limited effects. There is not much data supporting one estrogen over the other when it comes to anti aging. most creams will have either two estrogens (Bi-Est) or all three estrogens (Tri-Est) in different ratios.

      People use premarin on face.
      But everywhere it's written it's for vaginal use.
      Do people use premarin vaginal cream on face?
      OR
      There's a separate premarin 'facial' cream?

      @@titirmajumdar3730 I have read some articles showing the benefit of anti aging with premarin cream on the face. This is not a common practice and not something I utilize in my practice. Use your best judgement

      Thank you for your comment. For men, it is always recommended to apply the cream in the morning as testosterone levels tend to peak in the a.m. for males. For females the testosterone levels are relatively consistent throughout the day so nighttime application is okay.
      - make sure the skin is clean before application, application after shower is best
      - give the cream at least 2 hours for Full absorption before getting into bed
      - Be extra careful if you have young children as the testosterone cream can get transferred to pillow covers and bed sheets and then can be transferred to children from the bedding.

    Is premarin (conjugated estrogen) cream safe for face ?

      With age, our skin holds less moisture which leads to the development of fine lines and wrinkles. Estrogen helps promote water retention in the skin and can help with reducing fine lines. Estrogen creams are not recommended for daily use on the face, however, about once a week use can be considered provided no history of skin cancers, or any suspicious lesions on the face. Estrogen causes tissue growth and can stimulate the growth of skin cancer. The best option is to discuss with your doctor.

    Would estradiol or estriol cream to scalp help with menopausal hair thinning and hair loss? Many meno women with normal androgen levels suffer loss of hair and hair becomes thin, dry and brittle. It is devastating.

      Thank you for your comment. At this time it is not clear if androgens playing a role in female pattern hair loss as most females with hair loss tend to have normal androgen levels. Even if serum antigen levels are normal, some patients may have elevated conversion of dihydro testosterone conversion in the scalp so Topical application of dihydro testosterone blockers combined with minoxidil, red light therapy and platelet rich plasma can be beneficial. We also check for additional testing such as nutrition levels, thyroid hormones, prolactin levels, insulin. Certain medication may also be contributing to female pattern hair loss. In my opinion, Estradiol or estriol creams do not help with female pattern hair loss. I would recommend seeing your doctor to get a full workup to diagnose the underlying cause of hair loss and start treatments early before significant hair loss has occurred for best results.

I've been taking topical estradiol (Divigel 0.75mg) and Prometrium 100-200mg as well Minoxidil 1ml 2%/day for a year and that has not helped. 2 months ago my doctor started me on Finasteride 5mg po qd to see if that helps decrease DHT from attacking my hair follicles. Nothing seems to be working. I feel like meno turns women into old men even if they compensate for loss of ovarian function. 😥😭🥺

      @@crepesuzette5540 Hi, Hair loss treatments can be challenging sometimes. Might I suggest looking at your saliva cortisol levels. sometimes unmanaged stress can be overlooked as a potential cause. you can also consider getting PDO threads for hair follicle stimulation. Hope this helps

      Yes, occasional use of estrogen on the face can help with increasing water content in the skin, which can help improve the appearance of the skin and improve some of the fine lines.

    For menopausal women who can't shower everyday due to dry skin, is it okay to apply on dry skin?
    Were also going to need all available skin areas when we use more than one pump.
    It's confusing that you say to rub it in as the package insert days to not rub gel in or not massage the skin area.

      Hi Laura thank you for your comment. There are many ways to apply topical medications. If you already have dry skin you should not apply medications containing alcohol based gel as this was only further exaggerate the dry skin condition. Our patients are given compounded medication which is non alcohol based formula and has to be rubbed in for proper absorption. If your medication is in alcohol based gel, these are rapidly absorbed from the skin and do not need to be rubbed in. Your dry skin condition could be secondary to lack of thyroid hormone, lack of estrogen or other hormone imbalance. Topical medication generally are not an ideal way of hormone replacement for postmenopausal woman as with age, are skin has less blood supply and topical medications are poorly absorbed.

    Mayo clinic , NIH and other reputed health organizations state these creams DO IN FACT enter the bloodstream. So you need to site your source on saying it does not.

      Thank you for your comment. point I was trying to make was that when applied topically these creams are rapidly absorbed as they travel through lymphatics. Some of the hormones will still make into bloodstream but for accurate monitoring salivary hormone testing is preferred especially when using topical creams.

    Where can I rotate the places when using 2 different creams testosterone and estradiol ? And still keep them away from my arms and breasts? I am 61 being treated for menopause issues and tanked hormones. I am also prescribed progesterone oil capsules.
    This is very confusing! Do I use them continuously or take a break and how do I rotate when using multiple creams? Do you do zoom visits.

You can use the inner side of both forearms or arms and the inner side of both thighs and the area above the belly button. hormone cream should be kept away from breasts as they can induce breast cancer and also cause breast tenderness. since you are 61 you can use the creams continuously.       thank you for your question. If the hot flashes are caused by elevated estrogen or estrogen dominance than progesterone may be helpful. Most hot flashes are caused by imbalance between estrogen, progesterone and testosterone. You can certainly give this a trial for 4 weeks to see if your symptoms get better

      Thanks for your question. In order to transition we need much higher levels which are difficult to obtain with estrogen gels. In our practice we primarily use injections, oral medications or estrogen pellets.

    You say in the video to use estrogen cream in the morning but than in the comment section you say it can be applied anytime of the day. I have been using it at night because that's when I take my showers and my skin is clean. When is the best time to use it?

      Thank you so much for your comment. We prefer hormone creams to be applied after shower during daytime. When applied at nighttime there is potential for the cream to be spreading to bed sheets and pillows and any other person coming in contact with bedding can also get exposure to these hormones

      Thank you for your comment. Estrogen therapy results in increased fluid retention and bloating and abdominal cramps or discomfort is a common symptom of elevated levels of estrogen. In my opinion, you may be getting higher doses of estrogen and that may be the reason you are getting these negative side effects. Dscuss this with your doctor, consider adding progesterone and rechecking hormone levels. Most woman do not need estrogen replacement therapy as we have enough environmental estrogens to stimulate our estrogen receptors

      Blood tests are important for monitoring hormones. That being said, saliva tests are more accurate and more reliable when topical hormone therapy is used

    This Dr. Is saying complete opposite to many other sites...I am not suggesting he's wrong I am just saying there's many different opinions out there...
    I don't get it. Don't know who to believe...
    I started today and my pharmacist said to place it on wrists 2" site and on neck too.
    Didn't mention to rotate sites although rotation makes sense.
    Also, no other Dr. I've seen talks about having a rest day (which kinda makes sense but other people have said the key is to be consistent!!
    Super confused 😕

      Thank you for your comment. Medicine is all about interpretation, and we all have different ways of approaching certain conditions and hormones are no different. There are no large clinical trials for compounded medications which leads to a lot of different and sometimes opposing opinions. I completely agree that a site rotation is important. My opinions are formed based on almost 15 years of practicing medicine and managing hormone-related disorders. Trust your medical provider and your body, if your symptoms are improved then you are doing things the right way.

      Thanks for your comment. Hormones, if done properly can change your life. At the same time, excessive amounts of hormones will lead to negative effects and complications. Goal of hormone therapy is to treat negative symptoms associated with hormone imbalance. When we replace hormones, we use extremely low-dose which is just meant to bring the hormones to physiologic level for that partcular age. If your hormones are low, it is likely that your body is already not making them. However, if very high doses of hormones are used, your own glands may stop producing its own natural hormones

Your doctor may have his own protocols, it would be best to discuss with them if it is okay to skip 1 day a week of hormone therapy.     I hope you’ve updated your opinions since this video was posted.
    It’s irresponsible _at best_ to suggest skipping multiple doses of estrogen each month. The levels aimed for are base levels similar to those at lowest point in the cycle to create a “floor” that no longer exists. Plunging women back into perimenopause or menopause on a monthly basis is terrible advice. Consistency is key.
    Applying estrogen to inner arms could increase the risk of transferring to the breast area. Unless there is outer arm skin damage, applying it there is not a problem.
    Also, you weren’t clear that hormones don’t *cause* breast cancer but may stimulate growth of *some* types of _existing_ cancers. If mentioning cancer, it would have been a good opportunity to remind women to stay up to date with breast screening and, if they still have a uterus, to use progesterone if taking estrogen. There is a lot of unfounded fear about breast cancer with HRT so it’s important to not inadvertently add to the fear.
    Testing - this is a symptoms-based diagnosis and efficacy is “tested” by making sure you know all the potential symptoms and asking your patients how well they are being managed. Hormones vary _wildly_ in perimenopause and not at all in menopause. With the exception of testosterone , they don’t need to be monitored. Testing is generally an unnecessary expense.

      Thank you for your comment. creams applied to the vaginal wall are absorbed very quickly because of excellent blood supply in that area. I would recommend waiting about 90 minutes after the cream application before sexual activity to prevent cross spread.

    Mistake by you estrogen causes uterine cancer progesterone causes breast cancer. And they are vica versa on protective qualities. If a menopausal woman still has a uterus she needs progesterone along with estrogen. If she no longer has a uterus she doesn't need progesterone.

      Thanks for your comment. Estrogen is tissue growth hormone and responsible for many cancers. Progestreone is protective and reduces negative effects of estrogen. Progesterone does not cause breast cancer but rather protects agianst breast cancer risk.
      even if a women has no uterus and has high estrogen levels progestrone is indicated.
      most women have enough estrogen produced by adrenal glands to protect against vaginal dryness, hot flashes etc. Even if a women has no uterus, progesterone therapy is recommended along with estrogen therapy as estrogen therapy can cause breast cancer, increase risk of blood clots and breast cancer.
