I think he is just playing with the pressure, you know, pressing hard, the softer, then hard again. If you double click on the brush tool in AI you can adjust the fidelity towards "accurate" so the software doesn't try to pretty it up, allowing the stroke to look blobby (assuming you pressed the stylus with some variation of course)
