    Just ran a call for a Covid pt in res distress with the FD. Hadn’t seen one of the NRB in awhile and they had him on 10 LPM. Def helpful to get the refresh thanks.

    I’m using this for cluster headaches for the first time and it’s important that I get as much o2 as possible and this was very helpful.

    I just got out of OR/PACU as an EMS Instructor and during my first time i got (southern slang) to use a low high flow nasal cannula at 13 LPM. i was given Lorazepam 4mg + 4 (8) mg mixed with Midazolam 2mg x 4 (8mg), Ketamine 15mg over 10 min for nerve pain very slow IV push, post-op down to 9 lpm on methadone 10mg PO for pain with 12.5mg of Diphenhydramine IV. i was having Cystoscopy & Renal Biopsy, could not get spinal do to my severe kyphoscoliosis. 2 Ansthologist tried plus 2 ARNA tried. also i hate the NR/PR mask Oxymask can use 1/4 liter to 25 lpm

    then we tried an experimental type of a mask United kingdom style even though this was down in southern US at 60% at 22 LPM around 60 lpm but i performed ok with the high flow non-heated high flow nasal cannula 7-15 lpm ; and my range was 10 - 13 liters. back to air entrainment mask at 60%, some of our PACU/ICU Flowmeters go up to 25 LPM before flush.

    would this be ok to use at home for my mom when she gets a bad copd attack she is on a constant 2 liters of o2 now up her nose but sometimes when she gets up and moves around she gets out of breath thought about buying one of these from Amazon to help her get over the attack? thanks for any info .

    Would you put a humidifier on the non rebreather since the oxygen delivery is more than 4mL

      Thanks a great question Tahmina. I must have overlooked it 4 mths ago. 😄
      You can add humidity in the form of a large volume nebulizer attached to a mask, but with air entrainment it will yield a lower Fio2. There are two reasons we do not use humidity on a non rebreather. 1. The moisture would cause the one way valves to stick and it will also accumulate moisture in the NRB bag.
      2. The majority of NRB flow will be brought through the nose passively and heated/ humidified by the nasopharynx.
      Great question
      If you patient needs humidity in these cases, the best bet is a large volume nebulizer.

    How do they breath out in such mask? I think they will breath the same high co2 content air again

      The flow is set high enough to flush the exhaled co2 put of the mask between breaths. If the flow is set below the recommended amount, some co2 may be rebreathed. It is important to know the recommended flow for every device.

    Do you have a great way to connect a non rebreather with a neb treatment? Normally I just plug one end and slide the mouthpiece into the side of the mask.

    What about those tiny holes on the sides ? Mine are covered with something when I put the mask out of the bag

      Those holes are covered by a rubber cover to allow the patient to pull 100% oxygen from the inflated bag instead of from the air around them. The one way rubber covers increase the amount of delivered oxygen

    does the non rebreather have 2 valves and a partial with one valve???

      A non-rebreather has three one-way valves. A partial rebreather has two one-way valves on the mask, but the opening between the mask and reservoir does not have a one-way valve. The fiO2 delivered is greater with a non-rebreather, but the clinical effects are not usually significant. A very good question Philly B !

      I do, but you do not have to. It is more of a safety measure for me. Sometimes there are two of them, just remove one. 😁

      Yes. We have alternated these two devices before. The FiO2 will be slightly lower on an oxidizer, but I would recommend turning it up to 15 lpm and watching their SpO2 for 5 mins.

      I've never checked the package for sterility before. We just make sure the package is unbroken before applying it.

      It isn't mandatory and many bottles can't handle the flow rate of a non rebreather. The pop off valve will be activated on the bottle if you turn the flowmeter up to 15 LPM.

    I was put on a non rebreather mask last week and they had it set at 25lpm. Was that too much? It was on the portable oxygen not a wall mounted one.

      You really can't put 'too much' on a non-rebreather. The only problem with 25 lpm is how quickly you'll burn through an e-cylinder. It would probably last about 20 mins if it was started at 2200 psi. 15 lpm is best (will last 44 mins), but the key is to keep the reservoir bag inflated during the inspiratory cycle.

      We attach them to a heliox. 70% helium, 30% o2. It doesn't affect the function of the device.

    Non-rebreathers are probably the most misused piece of respiratory equipment there is. I can't tell you how many times I've seen the bag not inflated or had to pick up a patient who had been on one for way too long without any kind of definitive treatment.

      As long as you need to keep oxygen saturations to an adequate level. Long periods of high Fio2 levels can cause release of free radicals and pneumocyte damage.

      Philly B : 12 liters for a simple mask should deliver close to 0.60 FiO2. A 12 liter NRB would also work, but clinically 15 LPM is more common. Thanks for the question!
