I retired a few years ago after 40 yrs as a dentist. First of all I commend you on excellent teeth and surrounding bone. Rest easy on the possibility that this issue could affect your heart health, that only happens on cases of severe gum disease. I know your gums aren’t visible on the X-ray but the bone under the gums would show damage as well and your bone shows no damage. The hole where the smelly substance was found is an issue but on the other hand all the bacteria living there has caused no damage to the back of the second molar nor has it caused a localized infection to the gum around the hole or above the impacted third molar according to the history you shared. My advice is to not have the third molar removed. To keep the hole as clean as possible I would invest in a water pic, the water stream can help dislodge any food particles that get trapped there. And go to a dentist for a good cleaning more often than once every 10 years! Enjoy your videos, thank you for them , fair winds and following seas always!
Good advice, and those "water pics" work very well. I recently had 2 teeth that required root canal work, not pleasant but I had no choice due to infection. As Parlay gets into some remote destinations and maybe a week or two from any assistance, would it be reasonable to carry a supply of a suitable antibiotic in case of an onset of infection as it could end up quite bad if left unattended for several weeks to seek help.? My pain was excruciating.
@@aurelioroca6801 I carry 500mg of amoxicillin on my commercial fishing vessel as well a cephalexin as standard in my medical kit. More than one time I've been a long way offshore and had guys with an abscess tooth.
Perhaps have the opposing (and useless) upper 8 removed (really straight forward) so that if an infection of the gum around the lower 8 tooth flares up (pericoronitis) there would be no additional drama from the upper tooth chewing on the now swollen gum and prolonging the pain.. Agree with the water pic recommendation. (probably a bit late with the discussion as you are now North of Papua New Guinea!)
Definitely should have a supply of various antibiotics on hand on a bluewater cruising boat. There is a website that lists the types of meds to have, just in case. I would add opioid painkillers to that supply, no matter what the current anti-opioid attitudes may say. OTC NSAIDs just will not cut severe pain in an emergency situation. Or during the first days after the type of oral surgery that you need-and especially if infection has set in before the extraction! I speak from personal experience.
I am currently sailing in Colombia. I went to the dentist this week and it was the best dental experience I have ever had. The cleaning and X-rays cost me $65. I went back the next day for 3 fillings, $150. Next week I have a gum surgery scheduled which will cost less than $150. After 12 years of no dental care the Colombians have fixed me up for about $350 total. I am very happy with the service😁

Someone else mentioned a water flosser/Water Pik would keep area clean. Deal with the extraction when you are going to be someplace for a bit of time that has good dental care.

I had that operation at all four. It's definitely crazy when you hear tooth cracking or the doctor trying to pull it out with a lot of force. Painkiller are your best friends the next days ;-) Finally all went well and after a couple of days only eating soup it was all fine.

When I had to have some dental surgery, I searched for a dentist that used total sedation for the procedure. They give you a little poke through a vein in your arm, you gently go to sleep and when you wake the worst is over. In my case it worked out very well, strongly suggest you find a place that uses that method. Glad to see Britany loosen up a little, she is my secret crush 🙂
That dentist, “Felipe”, is one of the best in the world, surrounded by state of the art equipment and a great team. Surprising noting the location but he’s exceptional. I’ve had plenty of work done by him during visits and know of expats that travel just to see him.

As someone who’s had major dental surgery for double jaw surgery at 29 years old to fix a few issues including a severe overbite (and had 4 wisdom teeth removed when I was 16), my advice is to plan to take proper time off. You’ll heal much better/faster. Dental work like this is not something to rush through, as it can make life absolutely miserable. I couldn’t imagine trying to recover from it while sailing on a boat. Our mouth is literally our life, so you use it so much more than you even think about. Having to recover from major surgery means there’s no real way around having to take really good care of yourself to keep up on proper nutrition, fluids, etc. Give yourself a proper few weeks to a month off. Don’t recommend trying to travel or do any other major life plans while recovering. Find a place to hunker down, preferably with people who can provide good care, and use the opportunity to rest. Not to scare you, but just to say that I did end up with nerve damage. The roof of my mouth (pallet) was split and widened, and both sides of my jaw were cut and extended forward. Nerve damage was one of the risks. The entire roof of my mouth has zero feeling, and the left side of my chin never did fully regain feeling. Having said that, I would do it again 100x over. I look and feel so much better. The loss of feeling doesn’t really bother me. I got used to it. I just have to be careful when foods are too hot as I can’t feel it. Otherwise, it hasn’t affected my quality of life at all. So that’s my two cents. Don’t wait for that tooth to become a pain. Make a plan to get it done. I also feel you for not having health insurance. Even if I had insurance, it wouldn’t have covered it because it was considered a cosmetic/elective surgery. Cost $16K CAD out of pocket to get it done, and 3 years of my life total (including braces). Plus an additional 6 months-1 year before I could eat solid food again. You wouldn’t experience that downtime, though. That was only because they had to cut my jaw so I had to be very careful as I could have re-broken it by eating a steak or something difficult to chew. You’ll probably be eating solid foods again within a week or so. Dental work sucks in the moment, but it’s usually worth it in the long run, in my experience.
Suggest in the meantime purchasing a waterpik (not cheap but well worth the peace of mind) to keep the area around the tooth clean. Rinse daily with salt and/or antimicrobial mouthwash. Had a similar issue once but fortunately it was a small simple extraction. Good luck.
Hey Colin. I've been there before with the back tooth. Here's a top tip for that tooth and every other tooth in your head regardless of whether you have that one removed or not:::: Brush your teeth with heavily heavily salted water twice a day. Always have a bottle of heavily heavily salted water made up in the bathroom. Add few drops of mint oil or clove oil for taste. Brush and clean and rinse with that mixture every day, morning and evening and you will never have those bacterial build ups that cause that smell and ultimately rot your teeth. Salt is the number 1 cleaner and kills all the bacteria that would otherwise be having mardi gras in your mouth every night. This is a game changer. I'm telling You. Because if that back tooth abscesses on that main nerve you will want to take a screwdriver to your jaw. Take it from me. Salt your mouth morning and night and after eating during the day too if you like and You wont have those problems. Heavily salted water medicinal quantity. Your mouth will be fizzing but you will know those bacteria are wiped out and You will feel the difference in how clean salt makes your mouth and teeth feel and never want to use toothpaste again. Because all your doing with toothpaste is brushing bacteria around your mouth. Do yourself a favour get a bucket of salt and you wont look back. Toothwise anyway. The thing is, toothpaste has triclosan which an anti bacterial agent. It also has fluoride, without which the tooth enamel depletes in hardness. And surfactants to actually clean and break down lipids, not smear food around. In villages in my country, they use salt water but nobody over 40 has any teeth left 🤷♂️

Climbing an active volcano is by far one of the coolest things I have ever done myself as well. So hard to put into words how wild that experience really is. You can just feel the volcano rumbling in your diaphragm. Smell it, hear all around you. You feel small but ecstatic. Unforgettable. I actually named my dog after the volcano, haha. Stromboli. When you come through the Med the Aeolian Islands north of Sicily are nice place to anchor and hike around as well.
The Xray clearly shows the tooth fully developed. If its not hurting or bothering you now I wouldn't worry about it. Go live your life and if you start to feel something go in, listen to your body bro! Good luck and cant wait till next Sunday!

Medical and dental is always a challenge when traveling abroad because you never truly know what the training and skill level of the providers is. I think you made tge right decision to wait, if for no other reason than to get somewhere you plan to stay for a few weeks. 😊
I think you made a sound decision not to have surgery, only because of time restrictions due to the looming cyclone season. It might also be best to book an apartment post surgery for quiet comfort and remaining close to dentist if needed, ensure you have antibiotics suitable for dental infections on board regardless Colin. The tooth issue made me forget about that volcano real quick, ouch!
Hi as a practicing UK dentist we would approach the situation differently , by all means section the wisdom tooth but then leave the rest of it in place . The bone will grow over it and it is unlikely to cause a problem in the future . Removing the crown or top of the tooth is called a de-coranation . That is all that is required . the food trapping issue is then removed and the nerve and jaw bone are not disturbed .
It's awesome how you invested time in the local kids. So critical that cruisers do that to kind of pay back the amazing places they allow us to visit. It's their land we just sail in it. Congrats. That is one of the many things that make you different than other channels.

Having visited as a kid, i went back to that volcano with my wife last year and confirmed every memory i had of it being as scary as it was awe-inspiring.
Another great video. Tanna is one of my most favorite memories. The proper word to describe the experience of Yasur is AWESOME! Whenever you have that tooth, or any other pulled, put a teabag on the hole in your gum. The tannic acid in the tea causes blood to coagulate. Once the bleeding stops, the pain all but goes away. Fair winds.

I had all 4 wisdom teeth extracted at the same time. The bottom ones were horizontal like yours. The dentist was sweating so bad, his assistant had to keep dabbing his brow. The noises were horrible too. The top teeth sounded like wood splitting as he pulled them out. My whole jaw swelled, my entire neck and even the tops of my shoulders were swollen for about 10 days. Take care of your teeth, your back, and your feet! These are the 3 most important areas of the body that keeps us happy and pain free.

If you are in no pain, I see no problem in waiting a few months for a more convenient place and time to pause your adventures for a few weeks. I've been through a very similar operation, in my case both of my wisdom teeth had the same problem, the roots were growing like horns curling out and away from each other, pulling them like this would have been like trying to pull hooks straight out. This forced my oral surgeon to saw both teeth in half and pull one half at a time. I was awake through the whole thing, surprisingly the pain wasn't nearly as bad as I thought it would be. I was given enough pain meds to last 5 days and stopped taking it after the 2nd day. There was a considerable amount of swelling, my cheeks were stretched to the limit, my girlfriend kept teasing me about my chipmunk cheeks. That had to be like 45 yrs ago and I remember it like yesterday... lol

The problem with teeth is when the pain is uncontrollable it takes a toll on your over all health and infections can cost one their life. Get that taken care of asap. We love having you around Colin. Nothing effs up a sailing adventure like tooth problems, you've been given the heads up, now you need to follow through. If you have not experienced tooth pain, there is nothing like it. To be avoided at ALL costs.
I am glad you are staying up on your health Collin. If it were me, I would have taken the opportunity to get your tooth extracted if the price was reasonable and you trusted the surgeon. Infections can come out of nowhere and can impact more than just a tooth. It can get into the gum and cause tooth loss, it can damage your nerve and cause Bell’s palsy (facial paralysis on one side) or it can get into your bloodstream. All bad things when you are far away from land. A few months can turn into a few years when you are dreading something! Either way… good luck and thanks for such great content!
I had the same thing Colin! I too pushed it off my whole life, and with the help of a water pick, would clean out that hole. I got it removed this year and it was the best thing I've ever done. My doctor mentioned that if it's starts to absys or get a bad cavity, you'll have to deal with it at some point. It's better to just get it over with while you're still young and can recover easily. Plus - you can then sleep through your watch guilt free while recovering 😂
Great video, that volcano was insane. The experience you gave the kids will live on in their memories but the surfboard will change their lives. The fun they have with it is going to be immeasurable. You guys are legends. Gargle hydrogen peroxide morning and night to ward off any infections. Tip given to me by a dentist many moons ago.
get it done... couldn't imagine having issues out in the middle of the ocean. Problem with dental problems, if you get any infections they can spread quickly to your heart and that's it.

Your mouth will be fizzing but you will know those bacteria are wiped out and You will feel the difference in how clean salt makes your mouth and teeth feel and never want to use toothpaste again. Because all your doing with toothpaste is brushing bacteria around your mouth. Do yourself a favour get a bucket of salt and you wont look back. Toothwise anyway. Peace

The thing is, toothpaste has triclosan which an anti bacterial agent. It also has fluoride, without which the tooth enamel depletes in hardness. And surfactants to actually clean and break down lipids, not smear food around.
In villages in my country, they use salt water but nobody over 40 has any teeth left 🤷♂️

Climbing an active volcano is by far one of the coolest things I have ever done myself as well. So hard to put into words how wild that experience really is. You can just feel the volcano rumbling in your diaphragm. Smell it, hear all around you. You feel small but ecstatic. Unforgettable.
I actually named my dog after the volcano, haha. Stromboli. When you come through the Med the
Aeolian Islands north of Sicily are nice place to anchor and hike around as well.

Medical and dental is always a challenge when traveling abroad because you never truly know what the training and skill level of the providers is. I think you made tge right decision to wait, if for no other reason than to get somewhere you plan to stay for a few weeks. 😊

I think you made a sound decision not to have surgery, only because of time restrictions due to the looming cyclone season. It might also be best to book an apartment post surgery for quiet comfort and remaining close to dentist if needed, ensure you have antibiotics suitable for dental infections on board regardless Colin. The tooth issue made me forget about that volcano real quick, ouch!

Hi as a practicing UK dentist we would approach the situation differently , by all means section the wisdom tooth but then leave the rest of it in place . The bone will grow over it and it is unlikely to cause a problem in the future . Removing the crown or top of the tooth is called a de-coranation . That is all that is required . the food trapping issue is then removed and the nerve and jaw bone are not disturbed .
It's awesome how you invested time in the local kids. So critical that cruisers do that to kind of pay back the amazing places they allow us to visit. It's their land we just sail in it. Congrats. That is one of the many things that make you different than other channels.

Having visited as a kid, i went back to that volcano with my wife last year and confirmed every memory i had of it being as scary as it was awe-inspiring.
The best part of a long journey is the high amount of "this is THE wildest thing ive ever seen", - theyre all 100 % legit. Every single one.

Another great video. Tanna is one of my most favorite memories. The proper word to describe the experience of Yasur is AWESOME! Whenever you have that tooth, or any other pulled, put a teabag on the hole in your gum. The tannic acid in the tea causes blood to coagulate. Once the bleeding stops, the pain all but goes away. Fair winds.

Service to others is such a higher calling and so glad you guys could share with the kids that way, amazing. I grew up with my father as a leader in the boy scouts and sea scouts and learned how amazing it can be to give back to others. Glad to see Jaimie using the binoc's and hope the night vision comes in handy. Yes if the tooth gets impacted, pushing on the next tooth, it will really hurt and you should get it removed when the opportunity shows it self. Bali is a great place to get dental work and there is an amazing volcano where you can cook eggs from the factures in the ground the steam cooks your egg, so fun. I had all 4 wisdom teeth extracted at the same time. The bottom ones were horizontal like yours. The dentist was sweating so bad, his assistant had to keep dabbing his brow. The noises were horrible too. The top teeth sounded like wood splitting as he pulled them out.
My whole jaw swelled, my entire neck and even the tops of my shoulders were swollen for about 10 days.

Hey Colin I had the same problem with my wisdom teeth. I had mine decay in the same position as you. Once It hurts look out. It's agony. Keep enjoying life
If you are in no pain, I see no problem in waiting a few months for a more convenient place and time to pause your adventures for a few weeks. I've been through a very similar operation, in my case both of my wisdom teeth had the same problem, the roots were growing like horns curling out and away from each other, pulling them like this would have been like trying to pull hooks straight out. This forced my oral surgeon to saw both teeth in half and pull one half at a time. I was awake through the whole thing, surprisingly the pain wasn't nearly as bad as I thought it would be. I was given enough pain meds to last 5 days and stopped taking it after the 2nd day. There was a considerable amount of swelling, my cheeks were stretched to the limit, my girlfriend kept teasing me about my chipmunk cheeks. That had to be like 45 yrs ago and I remember it like yesterday... lol
I had the same kind of tooth, and a water pick really helps. The dentist didn't get all the tooth out, which I did with pliers several months later.

Make sure you carry clove oil in your medicine kit for sore teeth. Works a treat. You will love Vanuatu.

The problem with teeth is when the pain is uncontrollable it takes a toll on your over all health and infections can cost one their life. Get that taken care of asap. We love having you around Colin. Nothing effs up a sailing adventure like tooth problems, you've been given the heads up, now you need to follow through. If you have not experienced tooth pain, there is nothing like it. To be avoided at ALL costs.

I am glad you are staying up on your health Collin. If it were me, I would have taken the opportunity to get your tooth extracted if the price was reasonable and you trusted the surgeon. Infections can come out of nowhere and can impact more than just a tooth. It can get into the gum and cause tooth loss, it can damage your nerve and cause Bell’s palsy (facial paralysis on one side) or it can get into your bloodstream. All bad things when you are far away from land. A few months can turn into a few years when you are dreading something! Either way… good luck and thanks for such great content!

I had the same thing Colin! I too pushed it off my whole life, and with the help of a water pick, would clean out that hole. I got it removed this year and it was the best thing I've ever done. My doctor mentioned that if it's starts to absys or get a bad cavity, you'll have to deal with it at some point. It's better to just get it over with while you're still young and can recover easily.
Plus - you can then sleep through your watch guilt free while recovering 😂
Great video, that volcano was insane. The experience you gave the kids will live on in their memories but the surfboard will change their lives. The fun they have with it is going to be immeasurable. You guys are legends.
Gargle hydrogen peroxide morning and night to ward off any infections. Tip given to me by a dentist many moons ago.
get it done... couldn't imagine having issues out in the middle of the ocean. Problem with dental problems, if you get any infections they can spread quickly to your heart and that's it.
