So, when you're busting a regular brick, you're not killing anyone. The ones that have people in them are the question blocks, the multicoin blocks, the hidden blocks etc. And they don't get destroyed when Mario hits them. So, in short, no, you can't kill the people Bowser turned into blocks.
It's also stated this only applies to SMB1, not even the alternate universe that is SMB2 JP. So not even the Power Up blocks and the Coin blocks in this game are Toads either.
I'm in my 40s. I've full cleared Super Mario Bros. 3 more times than I can remember. I've one-lifed this game warpless. I've shown off some of its dumbest 1UPs to my friends, like those three you need a P-Wing for in 5-Spiral Tower. And I never knew about the coin heaven with a Cloud Jewel hidden in 3-7 until today.
There's also a few well hidden 1-ups in places like 4-4 and 3-5, both of which require a Frog Suit to get to. The Frog Suit is the least used in the game, although there are plenty of places to get one, including the end of world 2 (use a hammer to get past the rock, and enter the mushroom house)
Another thing that is taken for granted now was the then-new concept of worlds having more than four numbered levels. I thought it was so cool to see "1-5" and "3-7". I *was* just a kid at the time.

If you get 3 the same of anything at the end of a stage for fireworks everything pauses for the fireworks animation. If there was an enemy at the bottom I.e. The boomerang guy you didn't finish would that killing him if you get the 3rd match for fireworks?
Mario All-Stars Mario 3 has save files, you could stock up on Jugem’s Clouds and see how far you can push it.🤷🏻♂️ It is less pure than a new game, but more pacifism would be possible!
If you warp in this game, do you just not save the kingdoms you skipped over, or is it like a fast-forward where Mario does save those kingdoms but you get to skip over those parts?
The GBA port implies it's the former, because once you beat Bowser and unlock the world select screen, all the skipped worlds are still marked with "Help" instead of "Ok" (airship beaten) or "Clear" (all levels beaten).
Except the manual also says that the transformed Toads give out coins and items when you hit them. So all the brick blocks you destroy are just plain ol' bricks.

Warp whistle in World 1 lets you not kill that Boom Boom. Using warps, can you get yourself to the wrong warp skip in World 7 that ends the game?

The Killing happens the moment you touch any Card at the end of the level. Irrespective of the results of the total of the cards.
