Excellent case!! A true head scratcher as you said. I thought of post infectious GN (because of pacemaker) but no humps, also of AIN but Immune complex deposits ruled that out. Nice case for brainstorming Enjoyed it!
Hi just went through the discussion - I was having all along diagnosis of IRGN/PIGN. Elderly presenting with pulmonary edema and renal failure , most imp diagnosis to rule out is PIGN All the symptoms and lab criteria fit to this It’s more of RPGN - class II
