Wonderful case and discussion. The presence or absence of reticular aggregates in the endothelial cells by EM may also support HIV as playing a role in the glomerular injuries. How much podocyte foot process effacement was seen? Were all capillary loops equally thickened or were there areas with more normal basement membranes yet disturbed podocytes?
No TRIs were seen on EM. Although I believe that reticular aggregates simply represent an "interferon footprint" and also could be seen if syphilis were the pathogenic mechanism.
