  • PSA: The fuel tank explosion is captured on video at 2:24, and again by another camera at 6:24. The second surveillance camera video begins at 4:20.
    On October 28, 2016, about 1751 eastern daylight time, FedEx Express (FedEx) flight 910, a McDonnell Douglas MD-10-10F, N370FE, experienced a left main landing gear (MLG) collapse after landing on runway 10L at Fort Lauderdale-Hollywood International Airport (KFLL), Fort Lauderdale, Florida. The left wing subsequently caught fire.
    The airplane came to rest off the left side of the runway and the two flight crewmembers evacuated the airplane. As the flight crew was preparing to evacuate, the nearly empty left main fuel tank exploded after its exterior surfaces were heated by burning fuel, which flowed from a broken fuel supply tube in the left engine pylon.The captain reported a minor cut and abrasions from the evacuation, and the first officer was not injured. The airplane sustained substantial damage.
    The first officer was the pilot flying, and the captain was the pilot monitoring. Both flight crewmembers stated in postaccident interviews that the departure from Memphis and the en-route portion of the flight were normal. About 1745, air traffic control (ATC) cleared the flight for final approach to the instrument landing system (ILS) approach to runway 10L at KFLL. Recorder data indicate that the first officer set the flaps at 35º about 1746 when the airplane was 3,000’ above ground level (AGL). The first officer disconnected the autopilot about 1749 when the airplane was 1,000’ AGL.
    In postaccident interviews, the flight crewmembers reported hearing a "bang" as the first officer applied the brakes, and the airplane yawed to the left. About this time, the CVR recorded the sound of multiple thuds, consistent with the sound of a gear collapse.
    About 1750:48, the captain stated, "I have the airplane," and the first officer replied, "you got the airplane." The captain applied full right rudder without effect while the first officer continued braking. About 1750:53, the captain instructed the first officer to call and inform the tower about the emergency. The No. 1 engine was initially supporting the airplane after the left MLG collapse when a fire began near the left-wing tip. The airplane eventually stopped off the left side of runway 10L, about 30º to 40º off of runway heading.
    Interestingly - and atypically - the NTSB explicitly stated in its report that "The application of braking by the flight crew did not initiate or contribute to the landing gear fracture."
    About 1751, the flight crew began executing the evacuation checklist. The pilots reported that, as they were about to evacuate, they heard an explosion, at which point they immediately evacuated the airplane through the right cockpit window.
    Following extensive metallurgical testing and analysis, the NTSB determined the probable causes of this accident to be: "The failure of the left MLG due to fatigue cracking that initiated at a corrosion pit. The pit formed in the absence of a required protective cadmium coating, the cause of which could not be determined from available evidence. Contributing to the failure of the left MLG was the operator's overhaul limit, which exceeded that recommended by the airplane manufacturer without sufficient data and analysis to ensure crack detection before it progressed to failure."

      @@maxieduardoapariciom.3181- The emergency vehicles aren't lined up along the runway with motors idling, crews at the ready. This crash and fire occurred on the runway with no advance warning.
      The emergency crews have to suit up, leave their quarters, board the trucks, start up, go through a checklist, then drive to the scene. They have to go around to approach upwind.
      The fire suppressant is started before arriving to ensure that it works. No matter how recently it was tested, they have to make sure that it works as the suppressant spray may very soon be all that is between the truck and the fire.
      They are at ground level and don't have the view of the scene that is had from the vantage points of the monitoring cameras.
      The wind can change at any moment, blowing the poisonous fumes toward the emergency response teams.
      The emergency responders may have seen the explosion, which was an additional reason to approach from the side of the aircraft away from the fire.

      @@killingthedeath96 they did nothing wrong. Just FedEx running cheap and fails to do maintenance. As experienced as those guys are, I highly doubted that from the beginning that they did had a hard landing.
      »On August 23, 2018 the NTSB reported that "the failure of the left main gear was the result of a metal fatigue crack that initiated within the gear," and cited FedEx's failure to overhaul the gear at the manufacturer-recommended eight-year interval as a contributor to the crash.«

      The MD-11 is about the same as an early Porsche 911. It has a bad handling characteristic that can bite you in one situation, but if the driver is educated on this, it isn't a problem.

      Like the NTSB wouldn't want to view a half decent video of that 777 crash in San Francisco? I'm betting they would. And a $99 dashcam camera would not have cost them "millions" of dollars. Not saying that's what they should buy, but surely $2000 would get them an excellent system and I'm pretty sure that wouldn't cause them to go broke.

      It's not necessarily a problem with the image quality itself, usually the problem arises from the amount of footage they need to store at any point in time. It is much easier to lower the video quality to get a few more days in the recording (I think those systems store the past 30 days on a 24/7 recording) than to buy a larger storage device

      @@MatSil- In addition, the tower has an elevated position and can say exactly where the aircraft is, and the closest taxiway. Where as at ground level, you might at best be able to tell general direction until you get closer.

    All of complaining about ARFF's response time, which was amazing - they had extinguishing agent on the plane in 2:00, the regs are as follows;
    FAR139.319(h)(2)
    (i) Within 3 minutes from the time of the alarm, at least one required aircraft rescue and firefighting vehicle must reach the midpoint of the farthest runway serving air carrier aircraft from its assigned post or reach any other specified point of comparable distance on the movement area that is available to air carriers, and begin application of extinguishing agent.
    (ii) Within 4 minutes from the time of alarm, all other required vehicles must reach the point specified in paragraph (h)(2)(i) of this section from their assigned posts and begin application of an extinguishing agent.

      You are mischaracterizing the statute by omitting part of the text. A response time requirement only exists in relation to airport certification testing. It has to be demonstrated during testing, under ideal conditions. There is no response time requirement for actual emergencies. Just a goal.

    It should be noted that the ATC audio has had the blank gaps removed and the conversations didn't actually take place at the point that it appears they do as we get further and further in to the video.
    As one example, the conversation about opening the south runway certainly didn't come up within a few short minutes of this incident (prior to the fire even being extinguished). That was some time later after the fire trucks has already returned back to service - not while they were still extinguishing this fire on the north side of the airfield.

      You're wrong about that. The runway and airport closure was done by airfield operations, and that would have happened as soon as they saw what happened and that would have been within a few seconds to a couple of minutes of the trucks rolling.

      @@ImpendingJoker - I'm well aware of who initially closed the runway and you can hear them doing it over the radio. I was pointing out the fact that the conversation about re-opening the south runway (as I mentioned) wouldn't have actually occured as quickly as the audio here implies that it did. This incident occured on the north runway and it was not possible for them to re-open the south runway at a time when all of the airport's ARFF trucks were currently occupied with this incident on the north side of the field. The FAA requires ARFF trucks to be in service and available at a fire hall at a central location on the field if even half of the airport is going to be open - and at the point we hear them discussing reopening the south runway here, the fire wasn't even extinguished on the north side of the field yet.
      That was the entire point of my comment. This audio is compressed and the blank gaps had been removed, but it isn't apparent until we get further and further in to the video where what we're hearing doesn't match up with what we're seeing. Toward the end of the video, there is conversation about reopening the south runway, which didn't actually occur at the point where we hear it.
      The audio at the start of the video is pretty close to being in proper time. The blank gaps have been removed there as well and what we're hearing is still a bit ahead of what we're seeing, but that effect is much worse several minutes later in the video. I've edited my comment to explain in greater detail what I was explaining.

    FedEx ignored Boeing's recommendation for inspecting the landing gear struts for corrosion. There was supposed to be a cadmium coating to control corrosion but it was absent on this plane. It is not clear if the coating was never applied or if it was accidentally removed during a maintenance operation and never restored. So, FedEx saved a few bucks by not doing the recommended inspection and ended up writing off a valuable aircraft. FedEx NOW follows Boeing's recommendations.

      I would bet my left nut that their maintenance schedule was approved by the FAA. Just because they didn't follow Boeings recommendation doesn't mean they did their maintenance improperly.

      They failed to discover that the cadmium coatings were not present on the strut for 5 years and the strut became so badly corroded that the gear collapsed. If they had followed recommendations, they would have discovered the issue and avoided the failure and destruction of the aircraft and endangering the crew.

      If I am correct, THAT is an old DC10..NOT a Boeing Airplane/product. Maybe Boeing bought McDonald Douglas out.

      It started life as a DC-10 as built by McDD. It was converted to a "MD-10" by removing the engineer's position and updating the avionics to allow for a two person flight crew by Boeing or a contractor.
      Officially Boeing and McD Douglas merged. Reality = Boeing bought MCDD.

    The crew did an excellent job saving their own asses and landing in high winds - this can be a hard plane to land. Seems pretty clear that these landing gear have maintenance issues. Always love a good outcome.

    Its a whole lots easier to get the gear to the fire when all runways are closed. A lot of time is spent delayed at taxiways and runways until ATC can clear them to cross. When you have all flights hold position and all arrival and departures halted the responding gear can make a beeline to their destination.

    Report says the RIGHT main collapsed with a fire on the right side of the aircraft and the aircraft went off the right side of the runway, but everything was LEFT in the video. What's up with that?

    8:21 another ruptured fuel storage area with zero sign of fire damage. So what exactly was it that was fueling all these flames and explosion?

      What you see at that time stamp is not the fuel tank. the fuel tanks are forward of the landing gears only. behind the rear spar is the wheel well, a lot of cables going to the ailerons and spoilers and wires to the fuel pumps, sensors and actuators. at 8:49 you can see a schematic of the wing. there is no fuel stored aft of the rear spar

      In this case it probably was. It was a fairly windy day, and there was a risk the slide would be blown under the aircraft. So they used the next best option.

      It was parked on the north side of the field (near the other end of the runway, right of camera perspective) next to a burnt out Dynamic 767 (Dynamic Airways flight 405, caught fire on a taxi way a year earlier). Both planes were scrapped on site about a year ago. Interestingly, the 2 good engines from the FedEx are still sitting next to the hanger used by UPS wrapped in plastic.

    n the DC-10 all series that collapse is common here in MEXICO to the dc-10s aeromexico so it happened several times from 1973-1982 also in the A-340all series it is usual that your train is not saved

      Correct, that is the left side of runway 10L. Though technically its the right side of runway 28R

    I wonder how far off the ground their jump from the window was. I'm glad it turned out ok for them. I thought there'd be more crew members though. Navigator or flight engineer for example.

      Some of these cargo planes appear to fly with just the cockpit crew and with no-one in the back to monitor the cargo and to be ready when the fire starts (or what-have-you).

      A lot of DC-10 aircraft were refitted to eliminate the flight engineer, there are some seats in the back (the first 2 rows I think) but they're only used if an employ needs to hitch a ride.

      have you seen that fire? all the fuel that didn't burn would have been evaporated due to the heat. if there was any liquid visible in those images it would be water from the fire fighters

    I’m not a pilot but I can tell that by looking at the landing gear’s condition you can see that the tires are ripped and that can only mean that the plane’s landing gear broke off because of the overload.

      They ripped due to being pinched under the wing after separation. They weren't overloaded, if you read the whole reports you'd know that a cadmium plating was supposed to be done around the air-fill valve during maintenance on the gear but was not, resulting in corrosion and a hairline fracture that weakened the gear to point that when brakes were applied it simply couldn't handle the forces and completely let go

      I live near this airport, it was definitely concerning as I saw this aircraft fly over my house before this accident. And a year before another aircraft a 767 caught fire on the taxiway.
      Also the airport Delta 191 left from, before it crashed near Dallas.

    Nickle plate cause of failure does not make sense. I am sure the part was made out of 300M which if ground incorrectly can cause burns which should of been pick up through the nital etch process. The burns can cause stress issues resulting in failure. The other possibility would of been inclusions and cracks that were not picked up during NDT, mag particle inspection.
