CLOT vs THROMBUS A CLOT is a normal physiologic response to an injury. The clot forms to maintain hemostasis. he clot does not clog any vessels, and the clot dissolves itself when its function is no longer needed. In contrast, a THROMBUS is an abnormal, pathological reaction to prolonged stasis in the setting of hypercoagulable state. A THROMBUS forms even when there is no injury, ie it forms inside a vessel thus clogging it, and a thrombus does not go away by itself. But the worst part is that--unlike a clot--a THROMBUS can embolize, ie break off, travel through the circulation until it wedges into a smaller artery where is causes ischemia (MI, PE, CVA, etc). Lines of Zhan are NOT how you distinguish clot from thrombus. Lines of Zhan are how you distinguish thrombus formed while patient was alive (DOES have lines of Zhan) versus thrombus formed while patient was dead (NO lines of Zhan). Lines of Zhan are relevant at autopsy if you are trying to figure out whether this big thrombus was the cause of death or not. If no lines of Zhan, the thrombus formed after death, and could not be the cause of death. If lines of Zhan exist, the thrombus formed while the person was still alive. Lines of Zhan are alternating layers formed during laminar blood flow. You would not need lines of Zhan to distinguish between clot vs. thrombus, because location, location, location (like real estate) tell you whether you are looking at a clot or a thrombus. The thrombus is smack dab in the middle of a vessels, clogging it up. Whereas, the clot minds itself well, forms only where there is an injury, and seems to inherently know how to keep itself out of the way so that blood continues to flow past it. Lines of Zhan can be seen in thrombus, wherein clot they are absent. Lines are made of lighter ( platelet + fibrin ) nd dark (rbc ) layers alternatively.
