I dont have the car icon when I want to schedule a ride in advanced. Help

I've never taken ride share. Is there an option if you prefer male/female driver? If you know that you are going to board a plane, cruise, train etc. and you have luggage, can you put in comments or request a car that will meet your luggage needs? What is the tipping protcol? What do people traditionally tip? 10%? A couple of dollars? Do majority tip?
my app doesn't appear to be updated, I don't get the saved locations that I saved under settings for the calendar options. how do I update the app when it's not appearing in my apple updates?

currently i can log in to Uber using Malaysia Sim Card and phone number and key in password..
How about if i travel abroad in Europe or Japan?
Should i buy new Sim card in those country and sign up again to log in Uber?
