Antipsychotic Drugs & Pharmacology: Typical (1st Gen), Atypical (2nd Gen), NMS/EPS | USMLE | COMLEX

  • Master the Antipsychotic drugs & their pharmacology for USMLE & COMLEX (great for NCLEX as well). Typical / 1st Generation Antipsychotic Drugs & Atypical / 2nd Generation Antipsychotic Drugs and their pharmacology are covered in detail as well as Extra Pyramidal Symptoms (EPS) that are high yield for these exams.
    Side effects of antipsychotic medications should not be memorized whenever possible, but should be as intuitive as we can make it. Mechanism of action of antipsychotic drugs along with an understanding of physiology can help us predict side effects and truly understand these drugs.
    Of course, antipsychotic pharmacology requires SOME memorization, and memory devices in this video make this process much easier.
    Note: Neuroleptic Malignant Syndrome is not necessarily an EPS symptom, however rapidly increasing signs of EPS occurs in upwards of 95% of patients with NMS, so conceptually it is helpful to think of NMS in the context of EPS, especially with comparing the differences in TIMING.
    Note 2: Didn't mention specific receptors D2 and 5-HT2a in video. Atypical Antipsychotics work by blocking Dopamine D2 Receptors and Serotonin 5-HT2a.

    It’s so weird to hear that aripiprazole is the lowest in terms of the side affects. My brain reacted. Completely opposite to it. I took it for a good 3 years and I didn’t have problems till I hit the third. I’d have muscle convolutions right after taking it before bed- made it impossible to calm down & sleep. - took the bens for Parkinson’s & still no progress. Switched to ziprasidone. Works wonders on me for some reason.pretty strange.

      I had intense akathisia on abilify. Once, I biked so hard I almost passed out from exhaustion, but I still had an intense urge to keep moving my legs.

    D2 receptor is g inhibitory right then why do we have to antagonize the receptor if it is inhibitory?

      Not that simple - the dopamine pathway in the brain is very unique and acts on several different promoters and inhibitors - but you bring up a good question !
