Vucko100 got some... Theory about it actually. Back in the day... He even described why paramedics in GTA V are that useless.
But my theory here is this: RockStar intentionally ruined the cruel (like really cruel) fun for players that way. Just so players can't shoot infinitely (or while they have patience for that and are interested in that) pedestrian's heads and watch how paramedics will magically create a new head (or a whole body) to a dead pedestrian, just so player will shoot again that saved pedestrian and watch that "show" again. For real. Does that sound reasonable enough?
They could at least put them in the ambulance, just leaving the body on the street is crazy

I’m playing this old games unknown on the future and the games that are talked about for day that we played is Gta trilogy definitive edition.
I think GTA IV will continue to be one of those classic games that most games pare in comparison to.
Gta 4 took actuality to such level that it became downright boring to play just like life for real. Gta 5 had to encorporate fun elements and foolishness again to make playing gta enjoyable 😅

Actually ☝️🤓 gta iv paramedics are realistic, irl paramedic will try their best to save that persons life, IV paramedics will sometimes fail to revive someone which is realistic aswell, and as you saw revived ppl are not fully revived if they walk like that, that's why they taken to the hospital, but paramedics in all gta always forget how to get to the hospital 😂
Also, if you kill someone with a bullet to the head, the paramedics will do nothing cause, how the hell are you supposed to revive someone that was shot to the head lol

About food on GTA V: You can buy it in shops (you have to rob for 100% completion, but anyway, and not only in ONLINE tho), and even heal with it, but only of your health there is not at 100% level.
I know it's not the beefy burger or something like that from GTA SA, but it's still there. It's just lazy players who somehow always finding a reason to hate GTA V, so that's why nobody mentions it, if it's not ONLINE part
Restaurants have interiors, so they were probably functional in beta
They are mostly GTA IV fanboys, they have a personal crusade against GTA V.

But stores work, even in single player mode. And you can pay there for sweet snacks to restore your health (if we're not talking about vending machines, which is obvious)
