Severus' father was of Libyan/Punic origin, his mother was Roman. Ancient Libyans were not the same as modern African Americans. There are also tons of films/ TV series where they inject African American actors to portray European characters - Bridgerton, Vikings, Anne Boleyn, Troy: Fall of a City to name a few. Due to the Hollywood casting quotas, it's impossible to make a historically accurate film today.
Severus was not black, and probably neither was macrinus. There are some berbers who are mixed with sub-saharan africans, but MOST of them are not. Is it possible? sure. Likely? Not very.
