I appreciate my surgeons skills, he repaired my cuff 7 weeks ago, the only complaint I have is with my 6 week exam he spent 2 min with me max and I had more questions of what to expect going forward. I have to come to utube to find a surgeon like you that will educate a patient a bit. I’m 73 yo male active and a bit apprehensive about this from the beginning. Again my surgeon has skills that puts him near top in our state, I just wish he would communicate with me like you. Thank you Doc.

I found that with my surgeon as well, very skilled. Bicep repair, rotator cuff repair, ac resection, bone spurs etc. I only had one office visit where he ordered the MRI, got a call and 3 weeks later had surgery. Then at 2 weeks a 15 minute visit where he flipped thru numerous photos and spoke at top speed before he said see you in one month , then rehab boom out the door he went. I know he’s very busy but darn I felt the breeze blow thru the room as he entered and exited. I had no idea about the surgery itself, everything he would be repairing or how long recovery would take, hell I didn’t even know I’d have an ice machine. No communication at all, if it weren’t for this doctors videos I’d have no clue what was happening!
I guess maybe I got lucky because my surgeon was great after my surgery. He even has an assistant that came in and they both sat with me and discussed everything that I needed to know or answered all my questions. It’s been three weeks since my surgery.
Good work! I’m astonished at the time you have taken to reply to these folks. Currently 2 weeks post op. Thank you for the work you put in these informational videos!

I’m a 56, very active outdoors, DIY, chainsaws, yard work, etc. plus I’m State LEO. I’m 2.5 weeks after a full thickness and 2 partial tears as well as a bicep tendonesis. The first couple days were rough, I’ve been pain free for about 1.5 weeks, I’m sure that will change when therapy starts. I’m hoping to return to full duty, I am eligible to retire, but don’t want to. I’m especially hoping this does not hinder my off duty activities. Thanks for the videos, very informative.
Hey Brad, Thanks for the comment. Good luck with your recovery. Lets us know how your first PT visit goes. Please consider joining our private FB group facebook.com/groups/409240847744184
Was wondering how you feel now. I had the same thing done, I'm at 3 months post op and feel like im at a stand still. Its better but nothing like before.
@@erickpeterson537 I’m a year out from surgery. I was back to full duty after about 4 months. I am back to “normal” for the most part. It took a while for my strength to come back, but I’m better. It still aches occasionally when I sleep on it wrong or over work it. I used to chuckle at the old timers who said they could predict weather changes by their aches and pains…..I am a believer now.
Just had a grade 3 repair done (two anchors). First surgeon post-op visit in a couple of days. Originally told 4-6 weeks before PT. Keep sling on and minimize movement. This is my second RC surgery (opposite shoulder). PT (range of motion) began much sooner for that one. Same surgeon so this is interesting...I definitely want to avoid any "frozen shoulder" issues. Thank you for the video.
Hey Dave, good luck in your recovery. Some times it depends on the size of the tear and the quality of the tendon. Also, just double check, sometimes we make a mistake on what we want the patient to do after surgery.

Your videos are so helpful. My surgery was 2 months ago and I am still very sore but pushing thru. I feel like im breaking through scar tissue. Therapy is brutal but necessary.
Great video. I'm on day 7 of recovery. My surgery went well.... Biceps Tenodesis Labrum repair Supraspinatus repair Bone spur removed Subacromial decompression to shoulder Everything fixed except infraspinatus- full tear retracted and muscle atrophy, so I basically only have 3 rotator cuff muscles now. Hopefully enough to play hockey again!
Jason, I watched your video showing your scope pics. That was a big tear. Certainly it is most important to have the supra reattached. Make sure you go slow and follow your docs advice. It was a big tear! Good luck and let us know how you do in your recovery.
@@rotatorcuffexpert Thanks Dr. Orcutt. I appreciate all you do. I'm so glad they were able to reattach. It was retracted on MRI. I really went into the surgery thinking he was only going to make repairs to labram and biceps. As seen from the scope he didn't have much tendon to work with so I am taking it very easy.
I had a biceps tenodesis 2 months ago now. My physical therapist said I can't hurt it now. I still have quite a bit of pain when I try to raise my arm. Can you point me to any information on how much discomfort I should tolerate during my physical therapy and perhaps how much raising and using my arm during my job each day. I'm not sure if I should just keep pushing through the pain or take it easy (if that's even an option at work.)
Ajax, to me 2 mo out from surgery is not a lot of time. It is typical for people after shoulder surgery to experience pain and discomfort for many months. Go slow, listen to your body, good luck and thanks for subscribing.
Hi doc. I am at the 5 week mark and have just removed sling. Rotator cuff repair with anchors as well as the bicep tendon repair and anchor. I am fine when my shoulder is in a certain position but when I bend over to do the pendulums I feel a sharp pain and almost clunk. If I hold the shoulder in place I can do the pendulums with no pain at all

Dear Dan, Following up on your UA-cam cast on Physical therapy after surgery. I have a question. You mention 3-6 weeks where fokus is on mobility. And 6 weeks - 3 months on strength. I got a two-in-one operation : bankart surgery w. 2 anchers, together with initial brissement / manipulation of frozen shoulder. The advice form the surgen is start therapy with caution and follow the bankart program. after that focus on mobility to unfreeze the shoulder. It make sence, but i am very Worried to miss the window the opptunity for reganing motion. Leaving me “frozoned" for ever. I therefore want to start motion excercise also. I am 4,5 weeks in after operation. Do you have any good advice? I know it a very rare case to have bankart and frozen at the same time. Best regards Karim

Hey there! Great channel and very helpful information. I’m 90 days post op, and just had another follow up with my surgeons’s pa yesterday. Mentioned that I’m still very stiff and so she got me into a pt today. PT commented that your surgeon must be very conservative since I’m not just doing home exercises but pt now too. From what I’ve learned, recovery path depends on the specifics of the injury and repair, but wondering if waiting to start pt may delay recovery. The pt said I am very stiff, which explains a lot of my post op pain. Thoughts?

I wish I knew this a year ago, especially about the PT . I had rotator cuff surgery and my surgeon had to weave the tendon back together and connect it to an anchor, the therapist took my arm up and twist it so bad, that I made a remark to her "that I don't think my good arm can do this" it hurt so bad ,that I went from therapy to Doctor office and told them what happened, I went in for shoulder MRI and back to Doctor and was told it was too early to go back in and repair the damage that was done. I was astonished to learn from the Therapist that when they get fax all it says patient had rotator cuff surgery on date so and so but didn't say it had an anchor. Thank you for this video it was very informative.

Love your vids - thx for all the great info as it really is the only site needed to be informed - I have surgery in 2 weeks for a 11 by 9 mm tear in the Supraspinatus.....High grade partial thickness involving 75% of tendon. I am not in much pain anymore unless I go swimming. I am highly active and a fitness teacher so the main problem is function as the shoulder kind of gets stuck - I will push through it but it doesn't feel good when I do that.....lots of crunching. My main ask is - am I crazy to put myself in all this pain and discomfort when I feel like I could probably get by but with minor issues that may linger?... I am starting to get second thoughts but then I think for the long term I will be better off - esp because I am so active.......BTW Male 54 - Phys. Ed Teacher
Hey User.. That is a great question to discuss with your surgeon. A 75% torn tendon has a increased likelihood of tearing further in to a full thickness tear. So this may be the easiest time to repair it. Definitely talk about the pros and cons of surgery vs waiting with your doc. Good luck! Please consider joining our FB group or join our newsletter. Hi Dr. Orcutt! I'm 7 weeks post op this is what I had done OPERATIONS PERFORMED: Arthroscopic subacromial decompression, arthroscopic distal clavicle resection, arthroscopic completion of rotator cuff tear with repair with 1.4-mm tape and a single 4.75 Stryker anchor. When should I expect the discomfort to go away? Do you have any suggestions on how to sleep better at this point? Sleep has been the worst uggh! Thank you for your time!
Hi Madeline, it is common to feel all sorts of strange things after surgery including tightness. Hope this helps. How far along are you in your recovery?
I’m 4 weeks tomorrow right now the nights trying to sleep are a bit rough . I have been waking up two to three times because of the achy full pain mainly on the scapula . I’m still icing and now taking two Tylenol and two Motrin when I wake up at night. I had a large full thickness tear plus he worked on my bicep tendon did some debridement of the acromonium and glenohumerol head . 1 hour and 30 minute procedure. So I guess it was a bit more of a surgery.

I only still have around 120 degree and 25 degree with help of other arm. I am worried I got post op complication like frozen shoulder. Is this a normal post op stiffness or frozen shoulder? 😢
Hi. Doctor, I just had my partial tear of subscapularis repaired plus subcromial decompression 4 days ago. Only tyrenols and ice and the pain is minimal. I hate to wear the sling and I take off while resting. Is it okay not to wear only wear in sleep?

I did 4 mo of formal PT for my surgery (sub scap full tear retracted into chest cavity and stuck on pec muscle) had to detach long bicep to do surgery (open surgery) My pt folks basically released me but did not send me home with an at home program. They told me to just do the exercises i was currently doing and just add higher resistance bands/weights and increase it to gain strength. Is this correct or should they have sent me home with a "actual program"? Thank you
Well i think it depends on the doctor, the PT and the patients desires. I think in most cases the PT thinks that the person is "healed" after 4 months and they have done their job. Typically I hear them say slowly get back to life. As a surgeon, i would want to evaluate my patient and understand their goals in the context of their exact surgery so I can advise them of what to expect going forward. Hope this helps

Is this normal the day after therapy if so how long does this last?. Day 3 p.t. Was the same. Thoughts on heat vs ice and tens unit to help with spasms?
Michele, sorry that you are having that much pain. It is common to have increased pain after therapy, make sure you are communicating this to the therapist. I think there is a role for heat, ice and tens. They all can help with the pain. Talk to your surgeon, they may be able to add other meds to decrease your pain and spasms. good luck

Thank you so much for these videos. I am having rotator cuff, SLAP repair and bone spur removal. It is so frustrating to think about this long recovery. I am 58 and fairly active for my age. Is the timeline for healing much longer than just rotator cuff? When do most people return to work? I’ve put the surgery off a few months because of a vacation trip that was already planned. Any advice on shoulder activity up until surgery time? Thank you so much 😊
A link to my video about return to work is at the end of this reply. Good luck in your recovery, let us know how you are doing! Thanks for your video. Very helpful. I had my supraspinatus surgery repair 4 months ago and currently ;I am struggling with the repaired shoulder being higher when I elevate my right arm during the exercises. My right shoulder is higher than the left one. Is this normal? I know everyone heals differently. I am 50 years old of age and I feel like a 25 year old. I am very active and always been active all my life and due to weight lifting, I injured my right shoulder. I am also receiving physio and probably will for at least another year or forever but this condition in which my right shoulder is higher than the other when raising my arm is concerning. The surgeon just said to continue physio and that should fix the problem. Does anyone here had have that same problem?
Cindy, thanks for your comment. It is very common for the surgical shoulder to be higher than the other due to the accessory muscles around the shoulder working overtime to "help" the injured muscles. Once the injured muscles recover the shoulder often begins to look more normal. Good luck Cindy.. Im 53 and feel like 25 (most of the time) ;-)
@@rotatorcuffexpert Thank you so much for your swift response 🙏 you have no idea how better I feel now knowing that this condition will improve as time goes by. Hi Dan I had arthroscopic massive rotator cuff tear repair plus biceps tetonomy 2 weeks ago in my wright shoulder. My dominate arm is in a sling. Occasionally I take the sling off and do very light motion for a few minutes. I still have some shoulder pain. Is it normal or could be failed repair? How can I know that my repair is going well? Please reply back and Thank you so much. Joe
It is common to have pain for months after shoulder surgery. Without a new injury it would be unusual to retear it. Make sure you go to your follow up appointments so your surgeon can evaluate your progress. Good luck and let us know how you are doing.
What happens if you stress your shoulder after surgery? How will you know? Just had that balloon artheoplasty today. I'm feeling fine. Still has the same normal pain. Nothing big. I'm guessing that it is from the surgery. But, just curious of what happens if the shoulder repair is stressed?
CJ, we as surgeons anticipate some stress after surgery in moving and bathing etc. It has to be placed in a way that can tolerate that stress. Typically what I see, is a significant increase in pain if the surgery fails due to a specific stress. Hope this makes sense and good luck !

I really enjoy your videos. I am 5 weeks post op for a high grade partial thickness tear repair, with a distal clavicle excision using an open incision on my dominant arm. I am very concerned about being sent back to work before I am able to do my job fully. My injury was caused by wear and tear as I am 66 and have done physical work for over 40 years. When school lets out for summer we have to strip every floor in the entire school and lay finish. The work is really hard and I have had shoulder pain for years, and am now worried that I will reinjure my shoulder as soon as I go back to work. I don't want to have to change jobs at my age and just want to protect my repair so it will last. Any suggestions would be greatly appreciated.

@@rotatorcuffexpert Thank you so much for your quick response and the link to your other videos. My short term benefits can last 6 months, but I am praying that I won't be off that long. At 66, my physical activity consists of 8 hours a day on my feet at work, typical housekeeping chores at home and not much else. My pre-surgery shoulder pain pretty much dictated my activity level. Does remplissage prevent osteoarthritis from advancing? Osteoarthritis is not genetic for me it was caused by traumatic dislocations and no surgery for a long time. If labrum is already worn down alot and arthritis already there, will I definitely need a replacement at some point. I'm only 35
Hey Jim, Good question! A lot of people with shoulder arthritis do not require replacement surgery. Having arthritis early is concerning though. Hi Doc if I had only known all the excruciating pain that I am experiencing I would never had this surgery... It is now 4am and I haven't slept with this burning pain.... On pain killers but lying down is a real killer.... 😩😢
Pranitha, I am so sorry for your pain. Please reach out to your surgeon. they can help. There are other meds that they can prescribe. Pls let me know how you are doing.

what are some exercises i should be doing for a tenodesis i had 3 months ago? I do plenty of rotator cuff stretches and exercises and i did the post op stuff for the tenodesis but im at 3 months and i need the next level exercises for the biceps tendon.
Ahh, great question! I think that is a great topic for another video. I think I would do low resistance and higher reps. BTW great guitar on your channel!
Hi Dan, I’m 5 weeks in after 80-90% full supraspinatus tear. Surgeon said out of sling at 4 weeks since he was confident. I’m now doing active assisted arm movement to 90deg, but since the surgery my front delt has had a hematoma and is a constant source of pain..Sometimes it hurts during these ROM exercises..The only real source of pain. My question is, is it normal to have hematoma at the front delt and it be a common source of pain in the initial six weeks?
Guil, the anterior part of the shoulder very commonly has pain after rotator cuff surgery. I wonder if it is truly a hematoma or just swelling and pain from the surgery. I would ask your doc to look at it. Good luck and let us know how you are doing in your recovery.
I've had rotorcuff surgery and biceps tendon put back in place it s been 3 weeks I don't ever want to go threw this ever ever again nothing work s at all for pain I do not think I made the rite choice for surgery I don't think it worked
Should have asked while I was in to see today😏.... while in the 6wk - 3mth recovery, how should pain be managed? I'm trying wean from the heavy meds, but DEFINITELY feel "the throbbing nawing" after PT, exercises, in the morning at home. When should I no longer need pain meds?

Same...

Thanks for you kind words Kathy. Glad you are progressing. PT can be very challenging for sure. Keep on Going!
Great video. I'm on day 7 of recovery. My surgery went well....
Biceps Tenodesis
Labrum repair
Supraspinatus repair
Bone spur removed
Subacromial decompression to shoulder
Everything fixed except infraspinatus- full tear retracted and muscle atrophy, so I basically only have 3 rotator cuff muscles now.

Karim that is a tough one. Your surgeon will need to watch you closely to make sure you are not getting stiff, but also protecting your repair.

I'm so sorry that you had pain after surgery from the PT. Hopefully it was just temporary and you can go on to have a successful recovery.

Is it normal to feel slight tugs and pulls when putting on and removing your sling while getting dressed?
Hi Madeline, it is common to feel all sorts of strange things after surgery including tightness. Hope this helps. How far along are you in your recovery?
I’m 4 weeks tomorrow right now the nights trying to sleep are a bit rough . I have been waking up two to three times because of the achy full pain mainly on the scapula . I’m still icing and now taking two Tylenol and two Motrin when I wake up at night. I had a large full thickness tear plus he worked on my bicep tendon did some debridement of the acromonium and glenohumerol head . 1 hour and 30 minute procedure.

My surgeon just said continue the physio and that's was it. He did not spend too much with further explanation. I know each individual heals different. I am going back to work in a few days providing that I don't do any overhead movements. ( I am a dental assistant) frankly ;I am very grateful I came across your videos and thank you for taking the time in answering my inquiry. I know I have to be very patience and trust the process as my body isn't 25 years younger and its only been 4 months post surgery. Is there a video about this condition? That I can learn more from? Thank you so much!
Hey Cindy I will post a link to a video about getting back to work. Also there is very little out there about after post-op. I am getting something together for this topic.

Hi Dan
I had arthroscopic massive rotator cuff tear repair plus biceps tetonomy 2 weeks ago in my wright shoulder.
Thanks 🙏
What happens if you stress your shoulder after surgery? How will you know?
Just had that balloon artheoplasty today.
I'm feeling fine. Still has the same normal pain. Nothing big. I'm guessing that it is from the surgery.
But, just curious of what happens if the shoulder repair is stressed?
CJ, we as surgeons anticipate some stress after surgery in moving and bathing etc. It has to be placed in a way that can tolerate that stress. Typically what I see, is a significant increase in pain if the surgery fails due to a specific stress. Hope this makes sense and good luck !
I started watching all of your videos two weeks before my surgery, I had no idea what to expect or even the recovery. I’m now two weeks post recovery from a large year, bicep tear and ac joint osteoarthritis repair. I’m grateful to you for the information.
Im glad it was helpful and thank you for subscribing. I hope you are recovering well.
i had rotator cuff surgery 5 weeks ago and I fee my shoulder is still still. I do go to PT 3 days a week and do all my exercises. I guess i am worried i will never get better. any advice you can give me would be great..
Just keep up with your therapy and make sure you are communicating with your surgeon. Good luck, Stiffness can be a big problem after shoulder surgery.
Hi
I had rotator cuff surgery 4 weeks ago. I feel the same way. The pendulum exercise really helps. I am still applying ice before bedtime. It gets stiff above and below the elbow throughout the day.
Laurie, thanks for your input! Good luck let us know how you are doing in your recovery.
I really enjoy your videos. I am 5 weeks post op for a high grade partial thickness tear repair, with a distal clavicle excision using an open incision on my dominant arm. I am a custodian who does a lot of mopping, sweeping, overhead window, door and mirror cleaning at a school. I clean 16 bathrooms, offices , classrooms, sand a library.

Great information and I have to tell you that I started some strengthening exercises today that showed me how little strength my surgical arm and shoulder have in it....was a bit of a blow to my day realizing this recovery process will not be over in a matter of weeks as I understood it to be. My short term benefits can last 6 months, but I am praying that I won't be off that long. At 66, my physical activity consists of 8 hours a day on my feet at work, typical housekeeping chores at home and not much else. My pre-surgery shoulder pain pretty much dictated my activity level. As I said in my earlier comment, I want to keep from reinjuring myself because I don't want to go through this painful process again. Thank you again for your assistance and I will sign up for your newsletter.
Thanks Teri!
Really nice
Thanks a lot! Hope it helped.
In general surgery to fix the instability ( remplissage) does not reverse the arthritis, but they may decrease the instability that causes additional arthritis. Hope this helps. @@rotatorcuffexpert will a debridement or clean out to shave off the bone spur put a stop to the osteoarthritis or bbn will it just get worse no matter what?
Jim, clean out of shaving of a bone spur may improve symptoms, but do not reverse arthritis. Hope this helps.
@@rotatorcuffexpert what is it that causes the osteoarthritis? If I get it all repaired will it still break down even if theres no instability anymore?
My surgeon is having me to do PT the same day after the surgery and 5 days a week for two weeks after that. My surgery is Monday and I’m am so anxious 😢😢😢
@godchild1271, did you have a frozen (stiff) shoulder before surgery? Sometimes if you are really stiff before surgery then PT may start sooner and be more often. Good Luck, Please tell us how you are doing.
@@rotatorcuffexpert Had my surgery today. Went to PT and it wasn’t as bad as I thought. I have full range of motion. I’m in a little pain but I was given numbing medicine in my shoulder. So we shall see once that wears e. Thank you for your reply ☺️
I had a small rotator cuff tear a labrum tear and bicep tendinitis
I am glad it wasn';t that bad. Keep up the good work!
appreciate yr videos dr dan.
my left r c surgery is 27th Feb. i also have a tear and spurs in my right shoulder. my surgeon told me to deal with left first. is that kind of standard.

Are you a PA or an MD?
Anthony, I am a MD. Why do you ask?
@@rotatorcuffexpert lol I noticed in your library a reference book for the Pance. The PA certification exam. Also was wondering how the heck you have the time to maintain a surgical practice and make all these informative videos. I just had rotator cuff surgery 10 days ago and I must say your videos are super helpful and informative. But man do you ever have time to sleep! lol

what are some exercises i should be doing for a tenodesis i had 3 months ago?
I can not be down long...
Trust the process and your surgeon.
I've had rotorcuff surgery and biceps tendon put back in place it s been 3 weeks I don't ever want to go threw this ever ever again nothing work s at all for pain I do not think I made the rite choice for surgery I don't think it worked
Kris, be patient, it is tough in the beginning but it will get better. Did you watch my video on pain control after surgery?
ua-cam.com/video/z_8uKZHdCrQ/v-deo.html
Should have asked while I was in to see today😏.... while in the 6wk - 3mth recovery, how should pain be managed? I'm trying wean from the heavy meds, but DEFINITELY feel "the throbbing nawing"
after PT, exercises, in the morning at home. When should I no longer need pain meds?
typically people require pain meds for around 3 mo. with decreasing frequency over time
@@rotatorcuffexpert What kind of pain meds are we talking about? Tylenol and ibuprofen or percocet?
Perfect description of the pain I'm dealing with. 9 weeks post op and my description is deep wisdom tooth pain.
Right inner thigh muscle pain unbearable!
It is unusual to have thigh pain after shoulder surgery. I would touch base with your surgeon.
