If you have any questions, just leave a comment below.
If your insoles are glued down, just pull them up, that's what I did for these shoes. When you tie the knot under the insole, put it towards the inside of your foot so you don't feel it.

I came up with and used something like this to lace my combat boots in the USAF (mid '70s). Except I laced from the bottom of the eyelets up so the side lacing ran a line along each side and the middle lacing only showed between the leather. I had no name for it but others in my flight called it French Windowpane lacing. I seem to recall I also had zippers installed on the inside side of the boots to facilitate getting them on because it just took too long to get the lacing right if I was in a hurry.
