What a sad loss Gilels was. A remarkable pianist, nay an astonishing musician. The Soviet Union of this era had some outstanding musicians but for me Gilels is supreme with only Richter close.

Yes. That is the tradition well-established with this piece. Carl Fischer has published an anthology of Siloti transcriptions, including this one, and much attention is paid to the question you raise.

It is played through twice - the second time it is voiced differently.

Yes, like mentioned, it's played twice. First time with emphasis on the left hand, 2nd with the right hand.
