    Mignonne asks:
I tore my ACL 10 years ago. I’m very active, though. I have stopped playing sports, which I love. When my knee feels strong, I have the “one off” game and inevitably injure the knee again. Now, at 49, and after another injury, I resent the fact that I can’t participate in sports. I’ve heard that reconstruction does not always work, in that the knee will always be vulnerable. Of course my age now is against me with NHS. Can you tell me whether braces work and if so, which ones? Would I be able to protect my knee wearing a brace?
In this Ask Dr. Geier video, I discuss who usually does and doesn’t need surgery. I also discuss what type of braces a patient can try and how effective they might be.
Please remember, while I appreciate your questions, I cannot and will not offer specific medical advice by email, online, on my show, or in the comments at the end of these posts. I have torn my ACL completely 2 months back after having a poor jump from height and was not even able to walk. I went to my physio and she said I can go without surgery due to two reasons: 1) my meniscus was intact
    2) I had very few episodes of buckling
I am now undergoing rehab focusing on my strength and stability since 9 weeks and now I can jog and run at more than half the speed I used to run. I still have extremely few buckling but that's when changing direction but my physio is confident I can go back to playing soccer may be by changing my style of play. And the way I was confirmed that I can continue without surgery by passing a test (triple hop test).
I wanted to share my story as I want to help others who may also see these days and to give them some hope. Hope this helps.
    Stay strong all!

I'd like to share my own experience which is encouraging for people who want to avoid acl surgery. I'm not a doctor, this is just my own case and may not apply to your situation.
I get my acl completely torn 2018/02, and it's confirmed by MRI. I consult 3 doctors, all of them told me that it's a completely torn and need acl surgery if I still want to play soccer.
However, I decided to delay the surgery as my knee feels better and better. After 1 year, I try to go back to recreational soccer with a brace. To be safe, I did the 2nd MRI and it shows that my acl get connected.
I was told there's a chance that acl may heal naturally. I feel lucky and back to soccer with a donjoy defiance III brace. Now I can play soccer every week and with over 2 hours each time.

I have either twisted or torn an ACL or MCL in my knee, its been 3/4 months and I haven't gone back to playing but I plan on doing so in a few more months when the season starts. I definitely have done damage as of course its weaker than my other knee. However, i was always led to believe that the ACL cannot heal on its own? this reassures me as although I want to get an MRI scan done i would be willing to take a year out which the chances of the tear or damage healing on its own

24 years after acl tear I had knee surgery, I was 19 at the time, I was ok for the first few years after tear, I couldn't afford surgery at the time and being young and fairly fit was able to get by ok. However I was never able to compete in athletics the same even when wearing braces.
In my experience as I've aged, and my activities have significantly slowed down, my knee started to feel more unstable. I've had a couple of falls due to my knee buckling.
I had mri and the results were not good. After 24 years without acl I shredded my miniscus and developed arthritis. At 43 years old I wish I would have had surgery sooner.
I'm currently 5 weeks post op and looking forward to getting back to as close to 100 % as possible. I'll probably get a custom brace down the road for additional protection but I would recommend you think about future damage if you're considering not getting surgery. In my experience I had to get my miniscus removed due to the damage occurred from years of having an unstable knee.

I had torn the acl in my right leg 3 years ago. Already had some instability in my reconstructed hip so basically posture and movement patterns went south; lots of pain and instability everywhere, but bearable.
Over the last few months I've been able to regain some knee/shoulder/torso stability and have recently purchased a used donjoy xl brace with the intent retraining how I walk/move etc. Does this sound feasible plan? (I mean I'm gonna do this anyway, just want to setup realistic expectations)

Braces are good for less active people. However for active and crazy active it will be slipping off, heavy on one leg and light in another and you still freaking out on damage even more. Never few really comfortable doing all you can do. I use brace for a long time and still falling and embarrassing myself in front of groups and friends when I forgot I have a bad knee.
Surgery is the solution to feel free, happy and not afraid to do all you want when working out or playing sports with friends. Bummer I need to have a surgery.

I have a 14 years old daughter that plays travel softball. According to the ortho, she has a full thickness tear of her acl but everything else around the knee is solid. After the news I thought she was done for. But he said she could still play albeit with the use of a ACL brace. I don’t know if I should get a second opinion. I thought an athlete was done once they tore an acl yet he’s saying she ok to play. Her knee has buckled once chasing a fly ball...after like 30 min, she was ok to move. I think it’s just the sudden start and stop movements

I partially tore my ACL 5 years ago during a hs basketball game when I was 16. The surgeon had told me that I needed 2 surgeries but the meniscus one was the more urgent one so I did that and didn't return for the ACL reconstruction because when I went back to my initial sports doctor, he told me that I didn't have to get surgery at all even though he's the one who recommended it in the first place. He claimed that things could've resolved with physiotherapy alone which I highly doubt. Anyway I went to rehab a few times after the surgery but what helped a lot was my chiropractor. At the time the sports doctor accidently discovered that I had scoliosis and leg length discrepancy (good thing came from bad I guess) So I had to wear this insanely expensive scoliosis brace and an insole under one foot. I couldn't handle the brace so that's where the chiropractor came in to the rescue. He made me wear an irremovable plaster brace 3 times for 2 weeks at a time. For my ACL he maneuvered something around in my knee and with huge relief I could finally bend them all the way again. He unfortunately died a few months later of leukemia. I'll always be grateful for his help. Yesterday I played my first basketball game in years and after I landed from this one jump I knew I shouldn't have joined the game. I've forgotten what the chiropractor had told me, that I could play again without any risks if and only if I warmed up enough for about 30 minutes.. I didn't injure myself or anything but it was a narrow one. Still, I don't regret playing when I've been dying to for so long.. Which brings me to this video where I'm exploring my possibilities. Should I just get a brace or should I go see a rheumatologist? Let's be honest the right choice is clear as day: rheumatologist. I want to end this rumination with a quote:
“I do not believe that sheer suffering teaches. If suffering alone taught, all the world would be wise, since everyone suffers. To suffering must be added mourning, understanding, patience, love, openness, and the willingness to remain vulnerable. All these and other factors combined, if the circumstances are right, can teach and can lead to rebirth.”

I had a partial ACL tear three months ago. Some doctors advised me a MPFL reconstruction surgery. I am just trying to heal with PT. Not very happy with my progress. For the first one and a half months, couldn't even keep my foot on the ground as the knee was so swollen. I have started walking or I should say limping now. I walk around 500-600 steps per day, but that is inside the house. I am too scared to go outside, as my knee feels wobbly, this has badly affected my career and I am losing hope. I don't play sports but I would like to return to my normal life, workout, travel, and commute without fear. I had a really bad knee dislocation eight years ago when I was fourteen, was bed ridden for 3-4 months then gradually recovered in a year with hot fomentation and PT. That time I consulted a doctor in a government hospital, but could not afford even the MRI so I was just trying to heal at home with exercises. After that my knee has dislocated almost every three four months, swollen up a bit and then recovered in a week. There is a history of arthritis in my family as well. I could really use some advice on whether to go for surgery or not.

I wish you'd explain a bit more about what an ACL tear is and how a brace or surgery can impact it, whether it's a full or partial tear. Please, always stress that conservative treatment is best. Not once is PT mentioned. My ACL was torn almost a year ago in a fall and hasn't healed. So many factors must be examined.
Based on my particular circumstances, a brace is not practical, PT is questionable, water therapy is a great option, and surgery is last. My biggest fear is not being able to surf again but I have to have hope and believe there is a way. Lots of twisting and odd angles for ankles and knees.

So I'm going to chime in on this ACL party...I tore my left ACL back in 2008, had the reconstructive surgery and love love loved my new knee so much more than the healthy one. I just found out i blew out my right ACL during karate and plan on going through the surgery as soon as feasible. I have a 7 degree hyperextension on both knees which caused my knees to always feel wobbly.
My question for you is this: it may take until spring for me to do it, so can a brace and still participate in karate and strength training until then?

I wouldn't recommend contact sports with a torn acl. It's enough of a mental battle to worry about it not slipping while doing sports yourself but it adds much higher chances for it to happen often if it's caused by something someone else was doing while playing with you. That's just asking it to get knocked out of place every time. Hope everything worked out for you!!

I completely tore my ACL and MCL racing motocross when I was 19. The leg was so damaged and swollen I had to spend a week in the hospital and drop out of school. The doctor only did the MCL repair and there was no cartilage damage. He told me to not do any tough sports. I stopped the serious motocross racing to stay with college. I stayed fit most years and exercised a lot with weights. I am now 59 and training for my 13th Boston marathon. I have race motorcycles on trails, played in lots of hockey and soccer leagues, ran over 25 marathons including Comrades ultra, snow ski expert terrain, water ski and many other activities. No ACL and no brace. I can tell the knee is always less stable and when starting a new exercise sometimes a slight warning pain but never any resting pain. Do those squats and lunges. I am now for the first time going to PT rehabbing a calf strain so I can get another qualifier this October in Boston. Stay with it and build the muscles all over. Marathons no problem.

I had an MCL complete torn and ACL severe torn (based on doctor's comments) 3 years ago during an amateaur soccer tournament. I didn't choose to do surgery and I haven't got reinsured again so far. After the injury, I still went to gym, jogging, playing ping pong and soccer ( non-competitive) with brace and my injured knee seemed ok.
However, if I step on a hole or a lower level place, occasionally I felt my knee was a bit not stable but NOT that much of feeling of buckled or "give away". Also sometimes after mild soccer practice, I felt a bit pain on my right knee which was not injured. Should I continue doing some non-competititive sports, or should I avoid those sports?
    Thanks.

I tore my ACL skiing 10 years ago. At the time I was an amateur soccer player. I also played squash. I have never had surgery. I tried soccer again but the knee was unstable for that. I never tried squash again although I expect the same result. However, I have no problems with sports that essentially lack a dynamic twist and push component so I can still ski and run with no problems. I recently started to play tennis again although I do get very occasional instability with that. I have never worn a brace although I probably should. I'd be interested to hear of people with No ACL who think a brace aids stability. I am considering it. Hope this helps

Yesterday MRI report came it's showing mild or partial ACL tear , there is low grade strain on the lateral ligaments, synovitis, mild amont of fluid in patellar pouch. Now the question is on drawer and other tests it looks near to normal and I can bear weight on my leg , should I go for surgery or look for alternatives

Yes, but there's a lot to consider so you should get a second opinion. You don't want repeated injuries. Right now it might be a partial tear but if you don't allow it to heal it could become a full tear. Just sayin' you should discuss your options with your primary care physician first (they have to take care of issues after surgery if you go that route.) Talk about recovery time and what's most important to you.

It has been my experience that a knee with a rebuilt ACL is much better than suffering through life with a damaged ACL. Over time it gets worse. The knee grows more unstable over time, as the small injurys, shifts, and give outs will start to add up. It's my opinion to get it fixed while you're young and you will have a better knee for the rest of your life. It might also help avoid arthritis later in life.

This is my story, I do not know if it will help but here it is. My daughter is 16 and just had the surgery with a hamstring graft four days ago. She had no acl for a while (chronic tear from what they tell me, that means she tore it a while ago and has an unstable knee) and we did not know. Her muscles were doing a great job of making up for it (but not 100% like her knee would be with an acl). She does martial arts and when she fell she tore her meniscus. that is when an mri showed all the damage. Thank God they could save her meniscus. The point is that an unstable knee can cause damage to other thongs. I do not know how much of your acl is left or how stable your knee is but God bless you and I hope you make the right choices.

Hello Dr. I watched your video and I felt bit stronger. I am facing similar situation right now. I am sharing my problem to you. Please help me.
On 4th July 2023, I met with an accident while I was riding my bike, going to work. The MRI observations are as follows -
    1. There is fracture reaching upto the articular surface seen in lateral tibial condyle with surrounding marrow edema and contusions.
    2. There is subchondral fracture with cortical buckling and ill-defined marrow edema and contusions seen at mid aspect of the lateral femoral condyle.
    3. Ill-defined areas of marrow edema and contusions are seen in medial femoral and tibial condyles.
    4. Fracture with marrow edema and contusions seen in proximal fibula.
    5. High grade partial thickness tear of ACL noted.
    6. Gross joint effusion with myofascial thickening and edema noted.
    7. Grade 3 tear in posterior horn of lateral meniscus reaching upto tibial articular surface.
I consulted doctors, some told me to take rest for 45 days after that we will go for surgery, some told me that we can go for surgery now.
I didn't went for surgery till date. Right know, I am using a knee brace, which keeps my leg straight. I use this brace for around 23 hours daily. I am not bending my leg when I use brace. I walk around 100 mtrs using a stick. I can stand without support, can walk without support. I don't want to go for a surgery.
My question is- Can it be healed naturally? If not then, instead of going for a surgery can I use knee brace as a solution for walking?

Find a different doctor and get a second opinion on what's best for you and your circumstances. If your knee is really bothering you then you should discuss it with a doctor you trust that has the same or similar philosophies when it comes to treatment plans. You won't always agree with your doctor but the first time you meet you should be asking questions about their approach to treating their patients.

If you are active and play sports, I recommend having the surgery. I tore my ACL in Jan 2019 but I didn't get the surgery until July 2019 as I wasn't finished growing so had to wait. Throughout my time waiting for surgery, I continued playing sport with a special knee brace (it was bulky but I got used to it). If you don't have issues with instability (as my knee dint feel unstable as such) and you really really don't want the surgery then I guess you could go down that route but I also recommend doing PT as well as I felt like it really helped my knee feel more stable and by the time I had my surgery I was doing big changing of direction without my brace as I built up my muscles around my knee. So if you really don't want surgery then you could go down that road but I personally feel like the surgery is the better option, especially if you are active

I completely tore my ACL, sprained my mcl and fractured my tibia at 29 years old in May 2019. I am a semi active male. I have no pain at all after the first month, other than aching at time. I can live a normal life for the most part. The only time I find it difficult is if I'm playing with the kids outside, running quicker then a light jog. Pulling a heavy object or jumping. but I am at about 2 1/2 years with this injury and have avoided surgery at this point. But would like to get more stability out of it to be able to play recreational sports or jumping and not having the fear of coming down on the knee wrong or it popping out of place. But like I said I live 80% a normal life with this complete tear I just don't want to get surgery at this time awaiting medical technology to advance to potentially avoid this and recover. I. Doing this to avoid the medical costs, downtime at work and physical and to this day I don't see the benefit considering I'm basically able to live a normal life with basically no pain.

I had a complete ACL tear years ago and my doctor recommended me to do physiotherapy instead of surgery because I don't play sports that require me to twist my legs. The first year was incredibly tough, and if I could go back in time, I'd wear a brace and concentrate on making my leg muscles stronger. These days, I'm hyper aware of what might be a bad move to make on my knee. But I'll only wear a brace when I feel like my leg muscles won't be enough support (like when I'm trying a new fitness routine and my muscles are tense).
