Excellent discussion of the HINTS testing. It should perhaps be emphasised that HINTS testing should only be undertaken in those with ongoing vertigo AND nystagmus. If applied to a patient who had a brief episode of vertigo and vomiting, and currently have no symptoms and no nystagmus (eg BPPV) the head impulse will be normal, which would erroneously point to a central cause of the vertigo. Also, in patients with vestibular neuritis, the nystagmus does not necessarily disappear when looking the opposite direction. It may merely lessen in amplitude. However, the fast component will not change direction as was stated.
