Nadim, when you look up at the ball, do you look at it to the right of your upstretched arm, or to the left? I find that if you look at it to the left, tilting your head to the left (like Federer, Sampras, pretty much all the great servers) the tilted head position helps unlock the shoulder-over-shoulder rotation. Your tossing arm has to be almost parallel to the baseline.
