- In this Acute Shoulder Injury Evaluation with Paul Marquis PT I evaluate this gentleman who slipped on the ice one day ago and injured his shoulder. I'm suspecting a supraspinatus and subscapularis injury. I demo the belly press test, the bear hug test, empty can test and perform manual muscle testing to come to my conclusion.

thank you so much for having a real patient and for explaining the "why" so well. Far more helpful than watching videos of UE tests performed on people with no issues
Please keep these videos coming. Truly appreciated.
Thanks Jon! Feel free to share, LIKE and Subscribe. thanks.
I love your evaluation videos!!
I am a PTA student in my first outpatient clinical rotation and your videos help a ton! Thanks!
You are welcome! My goal is to help students and new grads see some of the stuff you may not see in class and put a few pearls from experience in there also. Please spread the word. Thanks.
So helpful Paul! I appreciate this so much as a new grad about to start in the OP ortho setting!
Glad it was helpful Anna! Lots more to come. check out the website too. orthoevalpal.com
Great explanation
This is so helpful and an amazing source to have as a DPT student - thank you!
Thank you Ashley! More to come. Spread the word! orthoevalpal.com/
Amazing video. This is great 👍
Thank you very much Hari!
Recently injured and found your channel for some at home tests (I will go to Dr if symptoms continue). Thank you for the information and thank you to all the patients willing to be filmed to see their reactions for the evaluations.
Thank you Michael! Good luck with your appointment. This guy actually did quite well and regained full function in about 2 weeks. It's amazing at how inflammation can look like a full cuff tear.
@ortho Eval Pal with Paul what did you do for treatment or as management?
@@OrthoEvalPal what did you do for treatment or how did you manage? What was the home care plan for this particular case?
Great fan of this guy
Thank you Sakshi!
Good night
Same injury me
Hi Paul , Thanks a lot for your amazing videos always with real patient
I am really interested to know after 2 months if that patient able to elevate his arm after his inflammation subsided as you said or he was needed to do MRI
Thanks appreciate
Hi Mina, Hope you are well. This guy had FULL ROM after about two weeks. He did very well. avoided an MRI and back to work. Thanks for watching! Best, Paul
Ortho EVAL Pal With Paul Marquis
Hi Prof again , you mean he made full ROM actively ?! And the cause of he didn’t make it first because of RC shut down because of the inflammation as you said?!
Thanks
@@monyd7roog Yes. Full AROM
5:31 if there is a tear, what do you actually do with the gentleman.
A splint, surgery or different approach to exercise??
I'm unsure
I've had a shoulder injury, my Xray and MRI are normal. My shoulder looks fine but as soon as I work with it, it starts paining, also I can hear a crack sound coming from the joint while rotating my shoulder. What to do ? Which physio exercises to do to get over it?
From your clinical experience what do you trust more for diagnosing rotator cuff tears: the special tests or direct palpation of the tendons?
I'll put it in order of how I like to evaluate shoulders...1. MMT 2. Special Tests 3.AROM 4. MOI 5. Palpation. Great question
@@OrthoEvalPal I noticed you like to palpate the bicipital groove area. Is there a reason you like to palpate this area versus others? Are there any other areas you like to palpate during your shoulder exam? Are there areas you do not place a lot of emphasis during palpation?
Unable to perform S flex. AROM but has full pain free PROM. Is this classic for rotator cuff injuries in the acute phase?
Yes, but not always. This one looks really bad right?? He was done with me in about 2 weeks with full pain free AROM and strength. Had a severely inflamed cuff. Going to throw this out at you in case you are interested. www.meorthopedicseminars.com/shop/comprehensive-evaluation-and-management-of-shoulder-dysfunction/
Thank you for this. Im in pain and just wanted to get a slightly better understanding before being fobbed off at the doctors.
You are welcome!
I think you made the infraspinatus muscle upset by forgetting to test it for a potential tear too 😃.
What was the outcome of x-ray etc
Negative x-ray. Back to normal within 3 weeks with PT. He was just inflammed.
@@OrthoEvalPal full strength back with Sub scap testing?
Im from Morocco north africa , too bad we dont have doctors that can define my injury , i dont know what kind of injury i have , i hurted my trapezius because of a heavy barbell squat , i feel weird in my trapez , and hurt my shoulder doing pull ups

You may have injured a nerve. There is a lot going on. You need to see a medical provider an identify where the issue is coming from.
