I am a physician. This was an extremely clear presentation. I don’t understand why we physicians cannot present this clearly. Seems like Pharm. D lectures do it better.
    Thanks doc!

I have been suffering from COPD for the past 3 years and 8 months, and ever since then i have been taking series of treatment but there was no improvement until i came across testimonies of @Dr_irabor2 on how he has been curing different people from different diseases all over the world, then i contacted him as well. After our conversation he sent me the medicine which i took according to his instructions. Dr. Bollmeier, Thank you for your insight and devotion to care. I have a question: my mother who suffered from COPD was having shortness of breath. We called the ambulance for transport to the hospital, and begged them not to have her lay back on the gurney because the pressure on her lungs was causing her severe pain, panic, discomfort. They INSISTED that she had to lay back and she tried to calm down and measure her breathing in order to get to the hospital 20 minutes away. Trailing the ambulance, we watched through the door window as she struggled to pull herself upright to catch her breath, then she laid back and we hoped she'd found a way to calm herself. We parked the car as they raced her in the hospital ER. She was pronounced deceased 15 minutes later, never regaining consciousness from her struggle. We were devastated, despondent and deep down knew her transport position was a factor. Is there ANY protocol for transporting patients with COPD the paramedics should have known and provided? Thank you.

Is there any point in doing the staging assessment of airflow limitation (GOLD 1-4) since choice of treatment is based on exacerbation and symptoms questionnaires (ABCD)?

I'm struggling a lot with exharsebations in a daily basis. My current lung function is about 30%. I have a permanent dual pacemaker. My hands and feet are often blueish. I'm tired nearly all the time. I'm wheezing a lot and have to tell myself not to sleep most or all of the day. I was diagnosed in 2016. My FEV1 was 47% a year ago and it's now 30%.

Plus they chose to take out of the therapeutic algorithm FEV1, while now in groups C and D one severe exacerbation equals to 3 for example exacerbation and 30% FEV1...Not enough data on frequent exacerbators. I wonder why they made such a big change in 1 year only without sufficient data.

Ciccio Bello a very greed driven disease. Pharmaceutical companies (probably do have a cure, they can't be that stupid after all these millenuims...) or there would be no advancements in any disease. This is the only disease a stock broker will mention to buy stocks in respiratory pharmaceutical companies and oxygen and respiratory equipment.....They are not doing that to any other disease...They are helping the opioid addicts...that they created...it's a human travesty....we all need to speak up loud and often.

My primary is borderline incompetent. His nurses are just as bad. Gave me spirometry less than 2 hours after I took my bronchodilators. (I tried to tell her this, she refused to listen.)

Min 9.40...I think you mean tiotropium has shown better effect on exacerbation, not LAMA in general. The algorithm is flawed. Until further proof the recommendation should be tiotropium.

You can also be diagnosed with this when you have diarrhea and have the flu...my life was a nightmare for 3 years because of being wrongly diagnosed. I hope if you are nurses you pay attention to the patient...I have great insurance through my employer, I feel it was a big sign, to abuse also. As, much as people with copd are not diagnosed, they are a lot that are diagnosed and they do not have it. And, why isn't there a cure for a disease that killed Henry the VII in the 15th century...if I was in the medical field I'd be ashamed of this, not giving people stigma. Smokers are stigmatized, whilst opioid addicts who leave their kids starving, dirty alone you save them over and over and over again. Explain that, in a vlog. I have become enraged after seeing what you do to people. This is a greed driven disease, that's the reason it's not cured, it pads a lot of pockets. The medical community can not have any better advancements ? Diagnostic ability, spiriomety is usually given by someone who hasn't a clue what there doing....finally I was told to take a deep breath and blow..thats a big difference, and demanded a CT scan...thank God I'm persistent. But, if your dealing with Copd patients they are humans, they will probably out live half of you. Have you seen them on threadmills?? They are health freaks, and they are germ phobic....I'm betting on them. Find a cure...do something...

Yes, so very true. I have been treated like crap by nurses, talked to like crap. I have dealt with ignorant nurses, incompetent ones, been yelled at...Had my inhalers I need, cancelled by my primary.
