How about patient who does not have a hand grip. Isn't it possible to grasp the patient's hand and follow. How about chances of shoulder subluxation chances ?

Interlace the fingers of the patient with yours in that case.
Subluxation probability occurs beyond 90 degrees of shoulder elevation and that too in open kinetic chain exercises.
Also not mobilizing and freeing the scapula before working on GH joint will lead to shoulder pain and subluxation chances.
