When delirium takes it's original form, the really big head, in it's animation you can see that it does a sort of gasp for air animation on loop, like it is suffocating, that is really morbid. Also when you defeat delirium, near the end of the death animation you can hear rumbling inside the chest which then subsides, this is probably the final struggle of isaac just before he dies from asphyxiation. Really morbid stuff there. At least thats what i think.

the Delirium is Isaac's mind going insane in his toy chest which explains the definition of the boss' s name, but the Delirium takes the form of every boss ( including the Hush and his second form ( if including the Delirious item) and possibly Ultra Greed and Ultra Greeder) which is Isaac's memory of all those boss which the the Void, the Delirium's kingdom is either a conjunction of Isaac's memories or a destructive mutant world through a gateway of time ( the Void entrance)
to me, delirium would have been a more sensible boss for the chest level, but ???/blue baby makes more sense cuz you see him in the chest in one of the endings

Delirium: The only thing immune to the Chaos Card, since it is chaos itself. It transforms wildly, unpredictably, and faster as it becomes weaker. It could appear anywhere, as anything, in a place that is an absolute mess. Even broken runs have to stay sharp. I wish it was crazier, personally. Have the room change aesthetics more often. Give a bit of damage-scaled resistance. Give Delirium itself more attacks and animations. Just no patterns. Why give order to chaos? At least to make him more predictable, have Delirium’s head proper spawn in select spots in the room, like the center or the corners. Those are just my thoughts, though. I love fighting Delirium, like during my Lilith run with two 48 Hour Energy pills.

The floor is kinda like Isaac’s imagination is melting together, until that melting manifests itself as a giant glob. There was also ??? (the floor), which shows a lump seemingly behind the floor, trying to escape, like it’s suffocating (representing Isaac), leading into the Void, a place representing that delerious state I mentioned. I know this is all obvious to everyone else, but I just figured it out.

You are right about delirium being isaac's insanity but wrong with alot of stuff here. first off when isaac wakes up in his chest in the Hush ending, that is the afterlife, shown by the dead trees and white ground. in the delirium ending isaac is the one you see walking away. he is exploring/accepting the afterlife after all the pain and torment he had before then. i know this because McMillion confirmed that the figure was isaac and the grass is white and there are dead trees just like the hush ending. unlike the subtle green you would see outside the house. The ending with hush is simply isaac's death and the delirium ending is a sequel to it as well since the void only has a 100% spawn rate after killing hush because that is when isaac dies.

A theory i thought of: what if all the enemies we were fighting weren't exactly who we thought they was? In reality, they were machinations created from Delirium itself

I see a fight, but it's a one-sided one. One of the fighters is merciless, consumed by the sensation of fighting, by the feel of their power. They think they're unstoppable now, and even the fact that their opponent still somehow manages to dodge most of their attacks, doesn't bother them. They fight just because they want to fight. They want to play with their prey, killing them slowly, watching them die. This is exciting for them, and that's why there's not so much possibility to defeat them. They will never stop.
Curse of the tower with scatter bombs and glitter bombs. First ever delirium kill. You get so much red heath from this combo that slamming you face into delirium becomes a valid strategy. For extra chaos, add butt bombs, bobs curse, and hot bombs. Thank you chaos, for replacing the boss item pool with the bomb beggar pool. Edit: Repentance nerfed it :(
my first time beating delirium I had absolute monster items and amazingly, melted him dead in less than a minute but that was also on normal mode so whatever.
Pov: you're playing Repentance where sometimes in Delirium fight happens glitch that plays boss-cleared theme when the fight is still going on and you don't want to fall out of form

I feel like this game has to have some inspiration from DQ6 since Satan's statue form is basically Mortamor's first form, his buff form is the 2nd form, and Mega Satan is literally a reskin of the 3rd plus Delirium being a broken bullshit boss that attacks way too fast that lives in a dungeon made up of bits and pieces from other parts in the game is reminiscent of Nokturnus and his Fungeon.
So I played DragonQuest 6 a lot as a younger kid and between Mega Satan being what looks like a reskin of Mortamor in all of his forms and the final optional floor being a mix of all the other floors in the game with a bullshit boss... I see a lot of similarities, at least in the endgame.

Once you unlock It Lives beat it within 30 minutes and you will reach the blue womb where you fight Hush. Beat him to unlock the void. If you mean the item beat Mega Satan as any character to unlock Apollyon and then beat delirium as him.

no, Delirium is the word used to describe an unstable mind, which explains his unpredictable behavior, he doesn't have any patterns either, basically Delirium is Isaac losing control of his world; the Chest he hides in
also Delirium can only become bosses you've encountered during the run, which can be explained as to Delirium using that to drive the player and Isaac insane

Actually delirium can become bosses you have not encountered on that run, it just priorities the ones you have encountered.
A theory i thought of: what if all the enemies we were fighting weren't exactly who we thought they was?

Redimaniac I can see that, considering that the Void is filled with bosses, some being from the last legs of the game, though seeing as all enemies are figments of Isaacs imagination according to Bumbo, perhaps Delirium was an after thought after being in the chest for so long

They want to play with their prey, killing them slowly, watching them die. This is exciting for them, and that's why there's not so much possibility to defeat them. They will never stop.

Curse of the tower with scatter bombs and glitter bombs. First ever delirium kill. You get so much red heath from this combo that slamming you face into delirium becomes a valid strategy. For extra chaos, add butt bombs, bobs curse, and hot bombs. Thank you chaos, for replacing the boss item pool with the bomb beggar pool.

What about Magolor ' s 1st phase theme, or Star Dream 3rd phase "Super Nova", or Galacta Knight's theme "Warrior From Another World", or "Masked Dedede"
I feel like this game has to have some inspiration from DQ6 since Satan's statue form is basically Mortamor's first form, his buff form is the 2nd form, and Mega Satan is literally a reskin of the 3rd plus Delirium being a broken bullshit boss that attacks way too fast that lives in a dungeon made up of bits and pieces from other parts in the game is reminiscent of Nokturnus and his Fungeon.

So I played DragonQuest 6 a lot as a younger kid and between Mega Satan being what looks like a reskin of Mortamor in all of his forms and the final optional floor being a mix of all the other floors in the game with a bullshit boss...

Once you unlock It Lives beat it within 30 minutes and you will reach the blue womb where you fight Hush. Beat him to unlock the void.
If you mean the item beat Mega Satan as any character to unlock Apollyon and then beat delirium as him.
You need to beat hush after every ending is acheived
