    I think Ron is right here. Speaking from personal experience, avoid tooth-borne expanders! This created a nightmare for me and I didn’t finish treatment (two front teeth died).

    My then 9 year old daughter had a crozat. After 1.5 years her cross bite and gummy smile was corrected. Since her treatment ended, has had significant regression as her palate has narrowed quite a bit. She has a lip and posterior tongue tie, both of which we were told not to release and that myo would be enough. Now she is 15 and we are pursuing Marpe and release. Back in 2018, no one was doing Marpe, so I think we did the best we could do.

      You are a great parent. MARPE done right alongside other treatments at age 15 is amazing.

    I think it depends on what the goal is of the treatment - for example, with a Schwarz/biobloc or any other of these tooth borne appliances, many people aren’t looking for big changes in their airway or pure skeletal expansion. What if a wider smile is what they want? Because there is a study showing slow removable expansion works in adults. What if the treatment goal is to correct a posterior crossbite (that has occurred due to a narrow upper arch)? That’s also a reasonable goal for the treatment.
    Many people have reported that their nasal breathing slightly improved with these appliances too. Yeah sure, it won’t be anything like MSE nasal changes but for young adults (18-25 years) you can acknowledge this as a possibility at-least with the ‘roof of the mouth/floor of the nose’ analogy
    I think people really just need to write down what they want, and be honest with themselves about what will get them there.

      But adding to that comment, I think the AGGA in particular should not be used in ANY case as the risks are FAR greater than any rewards (there are none)

      In my case im using biobloc because in my city and country ( madrid, spain ) i cant find a mse/marpe ortho specialist so…

      @@JADELACdoes it work for you? Have it tipped the teeth, pushed them out of alveolar bone or caused gum recession? Or is it actually working? Remember to use very little spring force and over a very long time period (at least 5 years) if you are an adult. Probably more to take into consideration such as where the force is pointed, but your doctor probably knows.

      @@bertodreable yes i am, currently 6 months in with bioblock. Thanks, ill go check them out, do you know if they do piezo cuts? Im planning on doing myofunctional therapy after im done with bioblock along a frenulectomy and then dr pedroche wants me to invisalign but id rather do mse first and then will see

    To add to the conversation on if MSE/MSDO makes the jaw slightly longer or shorter: Dr Yousefian (I'm a patient of his) has mentioned it's possible to get *up to* 3mm (if I remember correctly) mandible length gain with his version of MSDO. The way he's talked about it, it seems to vary by person and is not super predictable. As for MSE, he explicitly told me NOT to get a generic MSE/MARPE because I don't have crowding of my upper teeth (despite my narrow palate) and my front teeth would fall back and reduce lip support. His appliance can overcome this issue. So I don't know if the maxilla itself would lengthen slightly, but regardless the overall effect for me would be a reduction of tongue space (length-wise) and lip support.

      @@arshg9318 I'm not sure yet, I'm waiting on an ENT consultation and eventually I'll choose either TAPES or MMA surgery - he gave me both options.

    I didn’t watch your whole stream. But I have heard you have to use tooth borne expanders very carefully over a very long time. I.e very low spring force and over several years. Traditionally, tooth borne expanders have been used over a too short time period, usually too much force over months, and treatment ending after 1-2 years. I have heard you need to do it over much longer, 5years and so on. This is also very dependent with the skills of the practitioner of course. So there is a good idea to ask for previous results of several former adult patients that used tooth borne expander by this practitioner. Also asking for duration, spring force and where this spring force is pointed towards.
    There is also a good idea to think about current lifestyle during expansion. It’s important to get enough nutrients, sleep well and exercise. And of course all the good habits that comes with mewing (suction hold mewing/tongue in palate, breathing through mouth, lips closed, good body posture, etc).
    Another important step is to have an osteopath (posturologist, myofunctional therapist, cranio sakral therapy) work with your body to help with the changes the tooth borne expanders cause. And the changes need to happen slowly, very important.
    There is maybe other stuff to do also, but if these things do not work and the tooth borne expander causes issues; Important to stop treatment with tooth borne expander right away so it won’t cause damage.

    I think the shortening of the arch with mse must be related to how the gap is closed, the back teeth must be brought forward to not flatten the arch overall, unless combined with sfot advancment of the front 6 creating enough space to implant where premolars were extracted in PER cases

    My kids are almost finished with the Biobloc phase 1, and their mouths are visibly wider. Let's see what the next phase will bring. Our dentist is trained by Dr. Mew.

      It's where the adult claims come in that evidence hasn't been generated
      The children's cases tend to be good, but since the kids are still growing it can be hard to tell how much growth is natural vs due to intervention
