All jokes aside, All Might punched him so hard that he basically crushed his skull. According to the "doctor" he's talking to, AFO got his hands on a powerful regeneration quirk. But it apparently only works on injuries you received after you got the quirk. So he can't restore his face. So he's entrusting Shigaraki to be his metaphorical and literal face.
This episode is arguably the most important episode of the entire story because it explians the two most important powers. All for one: the power to take power for yourself or to give that power forcibly onto someone else. There is no limit to how many powers he can take or give. Note: he is extremely old. Probably 150 years old because of An anti-aging quirk One for all: The quirk that was born by accident from all for one himself. by forcibly giving the younger brother" the power to stockpile power " which combined with the younger brothers quirk "to pass on quirks" both combined into a power that is transferred onto the next generation become stronger and stronger each time it is passed.
I love what is coming up next: the first term finals!!! Those are awesome!!! After that, it's the summer camp arc that will change everything. I hope you're ready because this anime can only get better.

Sadly, this is why I go back and forth with the English dub and the Japanese dub, both are good but the English dub quotes the manga better each season and helps the story flow when there are too many breaks and you have to watch both at the same time making it very easy to get lost in details
