There's a bug I've encountered in the graveyard chapter where if you hold onto the grenade launcher, and decide to use it later on the group of mobsters that Passos distracts, then the mission immediately fails for some reason with an alt cutscene triggering, in which a lone mobster slightly further away guns down the running Passos. In a retry, I managed a few times to blow up that same crowd of mobsters with one grenade shot, then with a follow up shot I immediately fired it towards where that single mobster stands (presumably after the game registers a "fail" in the mission), and he gets blown up too. The game fails me again, but as Passos doesn't get shot in the cutscene, he survives and runs away successfully
