Can a Thyroid Gland Recover?
We're back for another edition of Thyroid Thursday and today we're asking a question I get asked a lot. Can a thyroid gland recover and can someone get off thyroid medication? It depends on how bad your thyroid gland has been damaged and it also depends on whether you actually address the things that cause the thyroiditis or the glandular hypothyroidism to begin with.

    Regarding the conversion of T4 to T3, I have seen people say the conversion happens in the liver and if this conversion is not happening you need to look into the liver. Does this make sense?

      Some conversion occurs in the liver, but not most of it. Most of it is via deiodinase 2 in other tissues.

      The polymorphism can cause some reduction in T4 to T3 conversion but the bigger cause of reduced conversion are epigentic regulators.

    so in what units should ft3 and rT3 be in order to calculate the fT3/rT3 ratio? If I want my fT3/rT3 to be 0.3, my fT3 would have to be 56 pg/ml while the maximum measurable limit for fT3 in my lab range is 4.12

    Eric have you ever seen cases of people developing symptoms like paranoia and hallucinations due to poorly treated hypothyroidism, as in symptoms that are very similar to schizophrenia? There are quite a few case studies about this condition that is called myxedema madness (or myxedema psychosis) but sadly there is very little information about it. Based on your detailed studies of thyroid physiology, do you see this as something that could indeed happen in some rare cases, perhaps with individuals who for instance have poorer sensitivity of thyroid receptors in the brain, or another similar process?

    I think one of the main reasons Hashimoto is so under looked is due to the fact many people live with it without even realizing they have a condition, or even worse, they blame it on something else, whatever it may be, it is easier to blame it on something else which there is a cure for, like stress, depression or anything. Having Hashimoto myself, I'd say one of the worse part of having an autoimmune condition is not being sure of what the heck is really going on, the lab panel comes up alright, and many people still don't feel well, and then, everything we try to keep our symptoms under control is a matter of trial and error, and most of the time you can't even tell if not eating something specific really worked or if it was just something else that made you feel better.

    I think I have a crazy nurse practitioner. I’ve been on thyroid medication for 24 years and I don’t like it I don’t feel good. And she offered me to just quit the medication. I told her no I think I have to be wended off & she said I had Hashimotos. I don’t know. I have had low thyroid for years. But I was diagnosed right after I had had a baby.

    Excellent explanation. After being severely chronically ill from mold exposure my rt3 went from way too high to 5, a little low, now as I am finally healing. Great explanation. BTW, in the last year treating mold/cirs I have carefully under doc review gone from 40 to 10 liothyronine. Fascinating how mold affects thyroid.

    Yes! It's not the weight number, it's how you wear the weight. I hardly ever weigh and when I taught Jazzercise, I'd tell the class to go by how your clothes fit - not the number on the scale. With my T4-T3 conversion deficit, I've gone from an hour glass figure to just a water glass figure. There's no curve, but I don't really "weigh" a lot more. Clothes still fit but body shape has changed.

    If someone has a problem converting T4 into T3, you get reversed T😢3. That does not work for the bodycells. And T3 also regulates the insulin production and the function of it. So with too little T3, there wil be a problem with the bloodsugar.

    100% agree with what you're saying here, but when you've been feeling like absolute garbage with hypo symptoms for years, and 10-15+ doctors keep telling you nothing's wrong with you because your labs are "normal", at some point it's either try to take some T3 to try and shunt the system, or stick a plastic bag over your head and move on to the next world. Finding the root cause is easier said than done, and most doctors aren't trained to think like that, even functional MDs.

    I took a high dose, 75 mcg 2x per day and I felt really well, I was carnivore and intermittent fasting to be able to absorb it well. Then I began eating carbs and dairy, the medication didn’t absorb anymore and I felt terrible. You need a higher dose than your body would make because you are now taking a full replacement dose of T3 only.

    Where I see the utility of T3 (synthetic, NDT or from some glandulars) is in giving a burned out, overworked thyroid a break for a period of time that could be months to weeks, and this could be based on assessing the degree of thyroid pain/aching the patient is experiencing and how even they feel energy wise. So I would get them to a level where they literally feel that their thyroid has calmed down and isn't being overworked and then pad in another month or so (two if need be) of exogenous T3. Is the gland itself more supple when palpated? Is the neck less swollen and stiff? These are things that can be scored and their trajectory recorded. TSH can be elevated even when T3 and T4 are normal, possibly due to RT3 of course, and the pituitary is signaling to the thyroid to work harder. This can cause the thyroid to become quite inflamed and progressively more so over months and attenuating this by asking less of the thyroid temporarily can take a fair bit of time as a result. Often enough a hypothyroid state follows a hyperthyroid one. I agree with the general idea of the body not making a lot of mistakes, but what I would call a judicious and short term intervention using T3 (or T4 with conversion assisting nutrients if this is the only option available) /could save the thyroid by allowing into heal/ rather than being on overdrive for longer and longer periods of time. If the cause of the inflammation that is causing elevated RT3 cannot be found and attenuated then the only real way to do it is with exogenous T3 and cofactors like tyrosine, selenium etc. to make conversion easier. If you don't intervene you run the risk of the thyroid being damaged beyond any recovery. Yes, it can "atrophy" but it can also burn out, and this, to me, is why the idea of just focusing on reducing RT3 without HRT is a bit problematic. A lot of people won't be able to make the modifications needed (due to the severity of chronic illnesses for example), so just doing away with HRT in these cases really sends them on a path of no return. We don’t live in an ideal world sadly. None of the research that I've seen (that I recall at the moment) discusses using HRT short/er term in the fashion I describe, and to me it seems like a fairly obvious take, but I know the guidelines and the “algorithm" most Endos employ doesn’t allow for alternative approaches: they prescribe levo, or anti-hormone depending, and get your TSH "normal" and call it a day. There is simply no reason that just because most people take thyroid HRT long term that this should be the approach. The Endo guidelines are frankly idiotic pretty much across the board. I think you can get to a place where the HRT is no longer needed pretty quickly, particularly when broad spectrum anti-inflammatory methods are being used such as sleep prioritization for 8 hours or more, broad spectrum anti-pathogens like oregano oil and cat's claw, broad spectrum NSAIDs like devil's claw etc., so what you do is tackle both: calm the thyroid while supporting better systemic conversion and uptake of T3. The whole idea I am offering is to use HRT during the acute/acute'ish syndrome and take the workload off of the thyroid temporarily and then fairly quickly wean a patient off of the T3 replacement and get them to a point where they are using a supplement that contains precursors and cofactors to hormone creation, conversion and uptake and/or mild glandulars. After a year or so of this approach coupled with low inflammatory marker tests, optimal RT3, and clear good health, the patient may no longer need to employ specific supplements for the thyroid but instead focus on diet and lifestyle. That’s my (current) two cents, but as I learn more I am sure my stance will be modified accordingly. I have been enjoying your lectures. Thanks for your work.

I've gained some weight and I'm struggling to get that off.I did go see a functional doctor who has put me on T3. I will watch my numbers as these videos are so helpful.     Once again you blow me away. I love it when you show your thought process on how you figure this out. I do have a couple of questions. 1. If you are manipulating the meds to only 5; then 4 times a week. When do you test? On a day meds are taken or on the off day when meds are not taken? Or Wait a set number of hours? 2. Did your patient experience weight gain do this? 3. I find it interesting you chose to skip medication on some days but remain on the same dosage instead of lowering the meds amount she takes each day. I think I can see why. But can you explain your thought process? Sorry if I bug you too much, I love to learn and have been trying to fix my thyroid on my own. You are a great teacher!

      It depends on what you and your doctor are trying to determine. If your are trying to determine if the T3 you are taking raises your T3/fT3 levels to a specific range, a few hours post dose. If you are trying to see how your body is making it's own T3, 24 hours.

    Well, u did not define stress. My observation of populations in 70-90 yo range w diet based on CHON & lipid w modest carbs & trim bodies in a non-stressful environment doesn't fit ur description... something is missing.... suggestions? considerations?

    Dose with t3. The perpencity to only dose with t4 is lacking.
