What is better B1 sublingual or capsule? As well as a dose
B1 in the b1 therapy in Parkinson's has been administered via sublingual (tablets), oral or intramuscular way. The sublingual and intramuscular routes bypass the gastrointestinal tract, where absorption may be affected by many factors in persons with Parkinson's. However, all the 3 routes are reported to be effective if the right dose of b1 for each specific individual is found following the b1 protocol. Consult with your physician if you plan to start high-dose thiamine therapy.

I am recommending vitamin B1 and magnesium glycinate to my sister who suffers from restless leg syndrome and sleepless nights. My suggestion to her was to purchase benfotiamine instead of Thiamine B1, which I am understanding is better absorbed. However I can’t find much information on benfotiamine products sold in Italy which is where she lives. Could you provide some information on this form of thiamine, and if there is a reputable brand sold in Italy. Thanks!
This is our blog on benfotiamine on our website on b1 (thiamine) in Parkinson's. While benfotiamine represents an attractive, potential treatment option for testing in future clinical trials in Parkinson’s disease (PD), no clinical study have been conducted yet on benfotiamine in PD. So, there is currently no evidence to recommend its use in PD.

"Tissue transketolase activity decreased markedly in thiamine deficient and thiamine-magnesium deficient rats (Group I and II). Addition of thiamine pyrophosphate to tissue homogenates of thiamine deficient rats (Group II) resulted in recovery of transketolase activity, however this pheno menon did not occur when rats were also deficient in magnesium (Group I)." Magnesium deficient wistar rats could not recover transketolase activity upon thiamine replacement.

Intravenous route is not used in the B1 therapy protocol. It also carries There is also a small risk of serious adverse reaction (allergic reactions).

Do you know how to get in touch with Dr. Constantani’s group. My sister was diagnosed this week with Lewy’s Body Dementia. I know they have done work with HDT in that condition as well as PD. I want to discuss their results, dosage etc. Appreciate the help.

Can I use Thiamine instead of Benfotiamine?
No clinical trial on the use of benfotiamine in Parkinson's has been conducted to date, although it is a legitimate candidate, as ut has shown in pre-clinical studies to have antioxidant and anti-inflammatory effects and reduce microglia activation. The B1 protocol currently refers to b1 ( thiamine HCL).
Just started B1 therapy, not diagnosed with Parkinson's but it does seem to run in the family and I'm at the same age as when family members were diagnosed ... Hopefully the B1 will help my brain stay healthy and keep the dreaded Parkinson's at bay !!

What about magnesium carbonate which I’ve been taking for several years to help with muscle cramps and to help with sleep?
Magnesium carbonate is commonly used in preparations for heartburn relief and "stomach acidity". Although one drug site reports an absorption rate of 40% (which would be rather high), information on its bioavailability is not easily available as reviews of different forms of magnesium do not usually include magnesium carbonate.

Do you recommend fat soluble B1 benfotiamine over water soluble thiamine for Parkinson's?
There are no published studies on the use of benfotiamine in Parkinson's. A few anectodal reports have described mixed results. Benfotiamine represents an interesting form of thiamine, which has already provided encouraging results in diabetes neuropathy and mild cognitive impairment in Alzheimer's. Its mechanism of action is still not clearly understood.
Can I take mononitrate is it just as effective
The form of thiamine which has been used orally and in an open study with a relatively long follow-up on b1 in Parkinson's disease is HCL.
