    If it sounds too good to be true, it probably is. This guy brags about elite athletes, kings, celebrities coming to see him, he’s making huge amount of money for giving advice that is, for the most part, common sense. Truth is that you cannot undo decades of body neglect, lack of exercise, poor nutrition, psychological problems with a few thousand dollar consultation. Clearly he is knowledgeable about the spine, as are many other physical therapists and orthopedic spine specialists. Anyone who tells you they have the answer to chronic low back pain is lying to you. This guy is a very well educated charletan.

      @derbye5369 I’m interested as to the origin of your comment. Charletan is an insult. Common sense? You’d be surprised as to the lack of it these days. Stuart McGill understands the human spine like no other. Yes, there are other good therapists, but I will guarantee they learned most of their useful knowledge from this man. This man does not brag. If anything he is too modest. He has fixed people who were beyond fixing, Brian Caroll is 1 such example amongst many many others.

      I’ll explain. I’ve listened to his many UA-cam material and several podcasts. He is not forthright and is anything but humble. Surely he must have many failures. He makes it sound as if all of his patients are cured of their back pain. Anyone in this field will laugh at such a notion. I have never once heard the magnificent Dr. McGill admit that he couldn’t help someone’s back pain. Not once. Is that possible ? Wouldn’t it be relevant to state this in any one of the hundreds of interviews he is in ? What clinician, physician, therapist, surgeon has a 100% success rate? He starts every answer with “it starts with an assessment”. How does it end Dr. McGill? A nice paycheck for the 3 hr assessment ? Do you follow up with your patients? Of course with the Olympic athlete/celebrity/king he likely does. Point is that chronic back pain is a very complex multifactorial condition, and anyone who charges thousands of dollars for a 3 hr assessment and claims to be the answer to the #1 cause of doctor visits in the country, and does not volunteer that some of his patients fail his methods, is a charletan. And believe me, there are many of those in the back pain industry.

      Maybe I haven’t listened to ALL of his stuff. Please, find me a single interview in which Dr. McGill admits that he couldn’t help someone’s back pain, that his “methods” failed. That would be humility. I’m not here to smear anyone. If I’m wrong, please correct me. Or perhaps Dr. McGill himself can respond to me if he can be bothered with us non-celebrity folks. He knows spine anatomy very well, and obviously has spent decades researching the topic. That does not mean he has the answer to chronic low back pain. Does he help many people? Yes I’m sure he does. But so do many physical therapists, chiropractors, many physicians and surgeons. Acute and subacute back pain has a high success rate, with any treatment…time probably being most important. It’s the chronic cases that are problematic, when someone is disabled for years and decades…and Dr. McGill’s claims that he can cure these patients with his methods, every single time? Well this is preposterous. But I’m sure quite lucrative.

      He actually does follow up with his clients. That’s how he claims such a high success rate. If they follow the prescription, they get better. It sounds like you believe chronic pain is something that cannot be fixed? Do you suffer from back pain? Have you had a McGill assessment? With regard to the cost of an assessment; He’s dedicated his entire life working as an academic to build a rock solid knowledge base (probably not on a great salary). He’s done more to educate the world in this field than anyone else. He’s worked to be an exceptional clinician and created exceptional clinicians. If you are the best in the world at something, you can charge what you like. Do you not think he has earned that?

      What is a high success rate? Have you seen him admit one time that his “assessment” and methods failed ? Not even once have they failed ? Would be refreshing to hear this from him just once. How long have his patients had back pain? Months? Years? Decades? Do they have actual spine pathology? a history of multiple osteoporotic compression fractures? Severe scoliosis? Severe kyphosis and malalignment? Severe stenosis ? Or are we just referring to the elite athlete and celebrity who has low back pain? In the real world of chronic low back pain, where PT’s and various physicians live, they don’t get the highly motivated athletes, celebs and royalty that Dr. McGill sees. He sees a very niche, tiny subset of very wealthy clients who are highly motivated and likely don’t have the severe spine pathology that exists in the real world. Try curing the obese 60 year diabetic who has 4 compression fractures and can’t stand upright because of spinal deformity. Yeah good luck with that Dr. McGill. Or the obese bipolar 30 year old smoker who is unemployed and can’t walk more than 30 feet. Oh yeah, those patients ain’t flying to Canada.

    People tell me that I use words that are too extreme, like “disgraceful.“ Hearing Dr. McGill referred to the ineptitude of clinicians in the year 2024 in treating something so common and life-changing as back pain as disgraceful… Was a huge relief for me.

    It’s so crazy how awful some people get treated in medicine. I’ve been a Union electrician for 23 years. I pay anywhere from $9/ hour to $13/ hour depending on the year I’m working. I calculate I’ve paid around $425,000 for healthcare in my 23 years. I’ve had 3 microdiscectomy surgeries in 12 years. All due to 24 hour pain from herniated discs that could never be relaxed. I get small flare ups where I’ll have lateral tilt and strong low back pain (I’ve had a numb right foot for 22 years) and a feeling like my back is bone on bone. I have NEVER had a dr even look at me over besides a MRI l, X-ray, Advil and send me to PT. My PT has always tried to spend some time with me, but they don’t pattern PT to me, they use the book and do the same for everyone. I feel like the micro movements are my issues. I wish so much I could take my $425,000 back and give all to Stu for a visit.

    Can this sort of physiotherapy actually fix a Spondy grade 2 (L4/L5) combined with a disc bulge ? Sure it might help but - it is surely never going to fix a mechanical problem like this is it ?

    Obviously Dr McGill thinks sitting at a desk causes back pain. But even if I retire, how can I avoid sitting at all? What am I supposed to do for the better part of the day? Stand all day? Sitting in a recliner causes more backpain. So, I am not getting this answer from Dr McGill's videos, what are we supposed to do all day if not sit. How about just lying on bed? does it help?

      The best position is the next position. Ive heard this once...i think the key is to switch position often throughout the day..i got a standing desk and switch often and stretch every so often in between task, hope it helps. I do miss sitting.... driving around and traveling is still an issue🤷‍♀️

      Hey. So for certain types of back pain - sitting will hurt. It is very much dependant on the structures that are at fault and generating pain. If sitting it a pain trigger then we need to look at posture, we need to look at muscular endurance around your spine and the time you are sat. Check out www.disc-pro.com

    If you have been diagnosed with mild osteoarthritis in the lower back - is there a way to reverse it?

      Clinically, Arthritis cannot be reversed. The bones/deformity that have overgrown, cannot shrink on their own. Arthritis also means bone-bone friction mostly because of reduction in joint space or loss of lubricating fluid or loss of cartilage. But when the condition is mild, one can manage it in much better way then someone who has moderate to severe OA.

      Osteoarthritis is not necessarily a pain generator. It depends on what triggers your pain and what makes it feel better. Have you had any input as yet?

    Did these for over a year. Read Back Mechanic and Gift of Injury. Had a video call with Brian Carroll. Saw a McGill Master Clinician in person for the 3 hour assessment and follow ups. Spent thousands of dollars. All this did absolutely nothing for my L4/L5 disc bulge. I am still in daily pain.

    I have an extra vertebrae in my spine.. L6 . I don't know if this is the cause of my back pain when I lie down or sit for extended periods.

      I also have the extra l6 and have extreme herniation disc pain at that joint. It has to be linked .

      We humans have wonderful differences. Just because it is different does not necessarily mean it is the cause of pain. It will be an environmental exposure to a physiological weakness. Modifying movements, postures and loads will help.

    Would stem cells work for a herniated disc? The science is new in that area from what I can tell.

      The issue is not the disc being injured but how the disc can to be injured. If you don’t fix that then you are just going to re-injure the disc - stem cells or not. Like giving an alcoholic a new liver but they carry on drinking.

    This is amazing! I want to be empowered too. The physio I have right now didn't even touch my back at all and said my own mind was amplifying my pain. I'm feeling really let down right now by the lack of diagnosis I've experienced. But I am now hopeful that I can fix this lower back pain I am suffering.

      Same here. Before injury i was a competitive fighter ever since i was 8yrs old. Parents had me in muay thai and japanese jiujitsu from age 5 due to bullying. first time i was ever punched in the face i just turned 5 and was hit by a 9yr old bully wearing a gold ring. Busted my eye wide open. Started doing mma at 18, at 21 got slammed real bad, whole spine felt like it was being cooked by a blowtorch and electrocuting me. Pain is 24/7. im 34 now and lost all of my 20s. Each day is just trying to survive. The physio person i saw treated me like i was an idiot who knew nothing about physical fitness etc despite them knowing my sports history. Not once did she ever even touch my spine. my sessions were her having me on spin cycle for ten mins then stupid stretches, nothing else. Absolutely ridiculous. My injuries are excessive. At t9-10 the spinal cord itself is twisted into the vertebrae and you can literally see on the mri the csf fluid being backed up below the point of impingement, above it you dont see much fluid. Think of it as a hose pipe with a kink in it not allowing water to flow properly. Its this thats causing all this pain, yet docs treat me like an idiot despite the fact ive spent all these years studying the spine and can clearly communicate with the docs with the terminology they themselves use. I get disregarded alot because of my age which is bs, i know for a fact if i was 60 this wouldve been resolved in months 😞😢 Theres also several endplate fractures and bulges, the bulges arent horizontal, outwards like normally do, theyre vertical into the vertebra and fractures themselves. Sorry for the rant guys lol im sure you guys understand how torturous and annoying it is

    Many clients get trapped in habitual postures and repetitive movement patterns that cause their pain. Ask yourself, "What motions, postures, loads and activities are triggering your pain?" Instead of paying clinicians to perform procedures, we need a health care system that trains clinicians to perform accurate, precise physical assessments and then prescribe functional, pain-free postures and movement patterns.
