Joel Hurt, MD - Austin Orthopedic Institute
United States
Приєднався 19 чер 2019
This channel features Dr. Joel Hurt, a board certified fellowship trained orthopedic surgeon, answering common orthopedic questions in an engaging and patient-centered fashion. No quoting studies or citing complex anatomy without explanation....we keep it simple. Sure, you can read a lot on-line but who is writing those articles? (Hint- probably not an orthopedic surgeon) And why are they so basic and overly general? Because the author likely just summarized what they read in a book. Healthspeak is different. I'm a surgeon who hasn't just studied the material but has actually treated these orthopedic issues in real life. So much of medicine is judgment and nuance that is only learned by first hand experience...on this channel I'll share mine. Most of the time will be spent on common knee and shoulder conditions, though general orthopedics is also something we will cover. Our goal is one video per week and with time we will customize content for your questions.
Dr. Joel Hurt explains Osteochondral Allograft
Dive into the world of advanced cartilage restoration with Dr. Joel Hurt's detailed explanation of osteochondral allograft surgery. This in-depth video covers:
1. Understanding Osteochondral Allografts
Dr. Hurt explains how this unique transplant surgery uses donor bone and cartilage to replace damaged joint surfaces, primarily in knees. Learn why this technique provides normal cartilage architecture that other methods can't replicate.
2. Ideal Candidates
Discover who benefits most: typically younger patients (teens to early 40s) with isolated cartilage lesions, often due to trauma. Dr. Hurt discusses the importance of intact menisci and ligaments for optimal outcomes.
3. Surgical Process Unveiled
Follow the journey from MRI sizing to graft implantation. Dr. Hurt details how donor tissue is selected, prepared, and surgically implanted to restore joint surfaces.
4. Mechanical Alignment: Key to Success
Understand why proper knee alignment is crucial for graft survival. Learn about complementary procedures like high tibial osteotomy and distal femoral osteotomy that correct alignment issues.
5. Real Patient Case Studies
Dr. Hurt presents several cases, including complex knee procedures, a rare patella allograft, and even a humeral head allograft for a young chemotherapy patient. See the transformative impact on various joints.
6. Recovery and Rehabilitation
Get insights into the post-operative journey, including weight-bearing restrictions, physical therapy, and the gradual return to activities over 6-12 months.
7. Long-Term Outcomes
Dr. Hurt shares impressive survival rates: 85% at 10 years and 55% at 30 years post-surgery, demonstrating the durability of osteochondral allografts when properly performed.
8. Comparisons with Other Techniques
Learn why osteochondral allografts offer benefits that procedures like microfracture can't match, particularly in preserving normal cartilage structure.
Whether you're a patient exploring treatment options or a medical professional expanding your knowledge, this comprehensive video offers invaluable insights into this advanced joint preservation technique.

Pros and Cons of Using Your Patellar Tendon to Reconstruct Your ACL
I had an anatomical replacement shoulder fitted in 2012 and it was never very good. In November 2023 my surgeon decided to fit a reverse shoulder. The surgery was carried out in sept 2024 but infection was found so an antibiotic spacer was fitted. The reverse shoulder will be fitted next week, 7th Feb 2025 and my doc also does the surgery in the computer first. Hoping for a good outcome.
Very professional! Excellent explanations. Great training aids. Voice and rhythm just right. Thank you so much for a wonderful video.
Excellent video dr Hurt. Probably will require a reverse in the near future. This video put me more at ease . I need to find a an orthopedic surgeon that specializes in shoulders and has the latest technology that he will be able to use for my surgery. Wish dr Hurt was located in central New Jersey. Did some research and found that orthopedic institute has many locations and one in New Jersey .
I’m 2 months post TKR. I literally walked out of recover and had 110* flex the next day. 10th day started PT and was at 128* and walking unassisted. 1 month and was at 138*. The functional recovery was great but the pain is lingering in my knee and quad muscle. Still swelling in knee but little to none in thigh or calf with numbing on outer side of knee. I was told that the avg turnaround from the initial surgical pain is 3 months, but of course, everyone’s recovery is specific to themselves. Then there’s the longer acclimation time for your body to adapt to this new hardware “intruder”. I’m hoping for the best. It seems the more time on my feet during the day leads to a more painful bedtime at night. I’m hoping for the pain to heal in pace with my knee function, but this video helps to make expectations a little more realistic. Best wishes to all whom Father Time has touched.
This is incredibly helpful for me to watch . I now understand what the surgery will look like. Or possibly look like . I can also speak to the surgeon in his language.thank you for this video 💛😊
Wow, you are right on! I had L knee replacement. I'm 73 and the first week was like; oh no what have I done! But I don't like to sit around. I dropped the PT because it was too expensive and followed exercising on u tube. The worst is the rebuilding the surgery damage to the quad muscles. One major key to recovery is, keep moving, flexing, etc! Doc is right on, it takes about a year! I'm 4 months along. I can walk pretty good but it still hurts somewhat off and on and gets stiff, but it's well worth it! Thanks Doc.
Thank You! for the information.
And a partial knee replacenent?
I had a total knee replacement and five weeks and one day from waking from the anaesthetic we went ballroom dancing at our club. Didn't do the Quickstep though, too many turns, that had to wait another week, just rumba, waltz and a few sequence dances. Took the blood thinning tablets but didn't take the painkillers as the pain was easily bearable without them. Do the physio, diligently, and it will work out fine.
I’m 90 percent sure I tore my acl after this video
How does that cause carpal tunnel? I had reverse shoulder surgery a year ago. My doctor gave me an injection in my wrist?
Thanks for this information. You are dead right it's a painful experience and it's not going away fast.
Had a knee replacement in Perth, Western Australia in 2010, did lots of exercise and eleven months later flew to Nepal and did the Everest trek from Lukla to base camp, at age 74, now 88 and knee still going strong.
I just had Reverse Shoulder surgery on December 5 th and I felt like it came out of socket some but my surgeon isn’t in the office until Tuesday. Does this always mean I will have to have surgery again?
This is my third time tearing my same acl
Damn I wish you were my doctor. I'm on over 3 years and 4 surgeries with another one coming. All on left shoulder.
Did you have Reverse Shoulder Surgery?
Not yet. I had a CT guided aspiration to see if shoulder is still infected. I have about another week before results should be in.
Excellent summary of the expectations and answers to some common questions.
No matter how you cut it, it takes one year!
This video is totally fascinating. I had a reverse replacement about 2 months ago. I knew about the modeling from my doctor but I didn't realize how developed it was. You have a great job. You get to help people and you get to play with power tools!
Most excellent review. I have quite a bit of damage and am having this reverse procedure on 12/5. Your video has clarified all of the question points I have had and I thank you so much. Very best regards.
55 yrs old and I’m 7 weeks post op from TKR. The 1st month was definitely the toughest part for me with pain and limited mobility. I noticed a significant reduction in the pain and swelling between weeks 4-5 as I switched from the aspirin to an anti-inflamatory. I was able to start riding a stationary bike 3x per week around week 5 and that has been a tremendous help for my mobility. I feel very fortunate to be ahead of where I thought I would be at this point, based on what other people’s experience. It certainly varies from person to the next. My PT told me the people that are more focused on getting rid of the pain tend to take longer to recover than the people that are focused on getting better. I could not agree more.
My going through this right now and the pain is terrible am dealing with this since August I can't even walk
I enjoyed this video. It was very informative and I feel safe in having Dr Hurt as my orthopedic surgeon. I saw him today and I was very impressed with his techniques which is very refreshing.
Thankyou so much for this video
Had my knee surgery in the middle of February 2024 and worked very very hard during rehab. Dr told me to take 2 pain pills before every rehab. I went there 3x week. And `no knee pain for awhile. Need the other one done but will wait til I can forget how bad the first three months were. Worse pain ever. But now fine and just had L5S1 surgery on on my lower back. Day 3. Woke up this morning with my butt doing mild twerking so I guess the nerve block is wearing off. Took pain pill and it stopped. I am 72 years old. And taking it easy today. Walked a little yesterday. Looking forward to tomorrow!
Thank you for this video! I am almost 3wks out of TKR and was given so little information. I applaud you for your heart and effort to educate us.
Thank you for your great information. I had a cortisone shot 3 days ago and was almost going to write it off as not working but you have given me hope with the 3-5 days for it to begin to work. Fingers crossed!🤞🏼✌🏼✌🏼✌🏼
Now how was ur knee is it good and can u run easyily without pain please give me a reply 😢i am also taken steroid my events are come that's why
@@nanduuu3754 The cortisone did not work on my knee pain but I do have arthritis in my knee too and I do not run at all.
Thanks for this video. Very informative information. Im out 1 week post opp. My pain level so far has been 0 to 3. Just did my first pt at home. Is it common for the pt person to just watch you do your exercises after telling you how to instead of working with you to help you push through? For me it seems thats why you have one. Have a good day
Good summary
Wow! This doctor is good! A perfectionist who knows what he is doing and not a one-size-fits-all surgeon. I like that he plans ahead and shares a video of the plan with the patient. I am a candidate for rTSA, and I sure wish I lived near Austin to have Dr. Hurt do the surgery.
I’ve had steroid injections in my shoulder, lower back, hip, knee, foot (plantar fasciitis), and big toe…the shots helped…still had to have meniscus surgery on my knee but that was just to “clean out” the torn/frayed cartilage…
Mahalo doc. Question. So I finally got my replacement after 5 years of suffering. 6 drs said I'm too young. Now at 55 a dr did it. I'm at 7 weeks n knee bends 86-91 . My problem was my knee has never bent 5 years prior. I'm getting nervous won't bend. Pt did some help but I'm stuck at 88 degrees. What can I do
How long will it before I go back work as a school bus driver?
The side effects are often horrible.
Doctor, Joel H, successfully repaired my newly injured knee. I greatly appreciate his excellent work. I recommended it!
Doesn't answer questions. Suspicious.
It's a scam not going to work
Incredibly informative
Four months post op, best description yet Thanks. perfect
Thank you
Incredibly intuitive! Thanks for the explanation.
This was a great video thank you. It calmed a lot of my fears. I guess I thought I was gonna be up in two weeks maybe three.
ty for the knowledege Dr..jazakallah
if i got one cortizone injection to tendon under pinky will i perminately damage healthy joint in pinky ? only had one shot on day 2/12 but im scared cuz i feel a slight pain in joint ajcent to tendon shot
i just got one going on 3rd day and i dont feel a done of difference yet have rsi and pretititve pressure from gaming injurined below pinky so far i regret getting the injection but will see how it goes on the comming days only day 2 1/2 for me
yes! true! one year back to life without thinking about it!
And my left knee done in 2015 and this fall and my right knee down
I have a knee replacement but my feet is not okay. I can't pick up my tones. And walk not well.
7 mos post TKR 90 percent back still having some stiffness pain is gone but in a occasion a flare up
Please take precaution of testing for metal allergies before surgery. I found out I'm highly allergic to nickel. Surgeon had to use a different knee replacement or I would have never healed and been in serious pain. It would have had to be removed and no replacement could be put in until it healed, leaving me bedridden for weeks! Not good. Be safe!
Finally the truth.
