    Can I ask if you have a paid partnership meant to endorse and promote BioHorizons implants? You seem to think they the best in the world. Here in Europe they basically are non existent compares to the competiton. Also the sleeves make the guided surgery unnecesary difficult. they should just have made universal drills for the master cylinders like other systems so you did not have to switch those sleeves for each diameter. Also laser lok is no Sci Fi tech, no offence.😊

    Spot on advice for clinicians. It is better to use your brain power in improving your protocols thus making your treatments more affordable than using cheaper not so well indicated solutions that will cost you lots of money in wasted chairtime.

      @@Myrussell67 Oh! I understand now. Where are you located? At our office we do all of this under sedation so you won't feel a thing. and we are able to do this procedure fast and safe. We have people around the country travel to have us do this procedure for them!

    Hi Dr Stanley I completely agree with a lot of what you are saying but feel like freehand should not be disregarded. A couple of questions for you please: Firstly- have you had the situation where you end up drilling deeper than you wished for, whilst using a guide, which could have been avoided had the guide been taken off and the implant placed 'freehand'? What us your view on pilot guides, made for the initial osteotomy only? Secondly- what if your guide is not tooth borne or only partially tooth borne? Would you feel happy using a guide say on an edentate case with flat anatomy for fully guided protocol? What if your arch has a free end saddle which allows for flexure (albeit minimally) which introduces errors of positioning, which could have been avoided had the guide been used for initial osteotomy only with the rest of the procedure performed with totally clear and unmasked vision? I'm interested to hear your thoughts on this.

      Freehanding increases risk of iatrogenic events. Unfortunately many older dentists resist changing with the times and these poor patients trust us to know better..his title is simply an attention grabber to spread a valuable message.

    I agree. Why eyeball it when you can digitally plan and get exactly what you want? Put together your plan with the end in mind and the patient will get a far better outcome. Those unwilling to move to guided is why I started placing my own implants. I got tired of telling my OMFS he had to take an implant out because nobody could restore it.

    Your assumption that doctors that place implants "freehand" have no idea where the correct position should be, is woefully misguided. Certainly guided is more accurate, but don't disparage doctors knowledge of correct implant location.

      Also we should mention that an implant doesn’t have to be in the perfect position. Slightly less than perfectly placed implants provide function, aesthetics, and longevity

    My question is - how do you do guided when from premolar to premolar is all zirconia crowns and you wanna implant the mollars? How do you merge the scan/model to the cbct in the SW accurately with so much scatter from the zirconia??

      There are bite plates with opaque indicators built in that you place in the mouth while taking the CBCT. This adds an additional analog step, but it allows you to merge the CBCT and digital scan when there are lots of opaque restorations…

    little worried about using a non-hex for the temporary. You ever get these loosening up and having a tough time getting it re-aligned in the correct orientation with the wings being gone?

    We custom abutment everything but just because we mill all our own work (I own and operate an in-house lab). I prefer the emergence profile of a polished custom abutment over stock anything, even if placement is mint with our guides

    Whether you use a stock or custom abutment, if your crown is ceramic it’s still bonded to the abutment, it’s just a matter of either the lab or provider doing it correct? A screw retained crown from the lab is actually just screwmentable to my understanding

    First of all , we are not talking about natural tooth , we’re talking about dental implants in ZBLC we need to consider many factors like type of the implant conical connections ,gingival thickness,depth of placement, abutments gingival hights , polish zr , etc..

    I am afraid that the cyano-acrylate will wick along the implant and cause implant not to integrate. Or if i am at 2nd stage surgery, to cause it to lose integration. Any thoughts on that. What has been your experience?
